上記のviral outgrowth assayは感染性HIVの培養操作を必要とするため、感染の危険があり、操作が複雑で、長時間を要し、さらに費用が高額である。Droplet digital PCR for HIV-1 DNAは不完全なプロウイルスも含めて定量するため、リザーバーを正確に評価できない。
(1)準完全長HIVプロウイルスを含む、HIVの病態を示すマーカー。
(2)HIVの病態を検査する方法であって、HIV感染細胞内の完全長HIVプロウイルスの量を評価することを含む、前記方法。
(3)感染細胞内に準完全長HIVプロウイルスがある検出限界以下であると評価された場合には、HIVの機能的治癒が達成されたと判定し、感染細胞内に準完全長HIVプロウイルスがある検出限界以上であると評価された場合には、HIVの機能的治癒が達成されていないと判定する(2)記載の方法。
(4) HIVプロウイルスの全長に点在する3~7箇所の領域を検出することにより、HIV感染細胞内の準完全長HIVプロウイルスの量を評価する(2)又は(3)に記載の方法。
(5)抗HIV薬の薬効を検査する方法であって、HIV感染細胞内の準完全長HIVプロウイルスの量を評価することを含む、前記方法。

1.HIVプロウイルスの全長に点在する数個の部位を同時に増幅する。
2.1の増幅物をそれぞれの部位につき別個に増幅する。
3.2の結果をCt値と融解曲線を基に陽性・陰性を判定する。
〔実施例1〕
実験方法
(1) 準完全長プロウイルス定量法(表1、表2、図1)
血液からFicoll-Paque(GE Health care)を用いて末梢血単核球を調製し、そこからDNA Blood Kit(QIAGEN)を用いてDNAを抽出した。
(2) 定量値の正確さ
制限酵素を用いて直鎖状にしたHIV-1サブタイプBのプラスミド(pREJO.c、pCH077.t、pNL43、すべてNIHから入手)各々について定量値の正確さを検討した。制限酵素は、pREJO.cに対してはSal I、pCH077.tに対してはNot I、pNL43に対してはNco I(すべてNew England BioLabs)を用いた。プラスミドの濃度は260 nmの吸光度から、1 OD = 50 mg/mlを用いて求めた。
(3) DNA抽出段階におけるDNA損傷
HIV-1非感染者の血液から調製した末梢血単核球に、制限酵素Nco Iを用いて直鎖状にしたHIV-1サブタイプBプラスミド(pNL43)を混和し、DNA Blood Kitを用いてDNA抽出を行った後、本検査を行った。
(4) 抗HIV治療開始前後の準完全長プロウイルス量
未治療患者5例と抗HIV治療開始後3年以上ウイルス未検出の患者5例の血液検体を用いて本検査を行った。
(5) 抗HIV治療開始後9年間の追跡調査
ある患者の抗HIV治療後9年間にわたる7血液検体(図2)を用いて本検査を行った。血漿中HIV RNA量の測定は、コバスタックマンHIV-1「オート」を用いて行った(BML(本社 〒151-0051 渋谷区千駄ヶ谷5-21-3、総合研究所 〒350-1101 埼玉県川越市的場1361-1)に依頼)。CD4陽性リンパ球は、フローサイトメーター用いて測定を行った(慶応義塾大学病院中央検査部へ依頼)。
(1) 定量値の正確さ(図3、図4、図5)
全プロウイルスに対する準完全長プロウイルスの割合は、pREJO.cは16/16、pCH077.tは14/15、pNL43は18/18であり、平均98%であった。本検査法を用いて準完全長プロウイルスの定量が可能であることが示された。(表3)
(2) DNA抽出段階におけるDNA損傷(図6、図7、図8)
全プロウイルスに対する準完全長プロウルスの割合は、1回目は17/20、2回目は18/20、3回目は14/18であり、平均88%であった。DNA抽出の操作によるDNAへの大きな損傷は見受けられなかった。(表4)
(3) 抗HIV治療開始前後の準完全長プロウイルス量(図9~図18)
全プロウイルスに対する準完全長プロウイルスの割合は、治療前は平均26%、治療後は平均2%(ウェルチのt検定、P=0.003)であり、統計学的に極めて有意であった。(表5)
(4) 抗HIV治療開始後9年間の追跡調査(図22-29)
抗HIV治療を始めて年数が経つにつれて、欠損プロウイルスを含めた全プロウイルス量も準完全長プロウイルス量も減少したが、抗HIV治療を始めて約5年以上が経過した検体では、全プロウイルスが80~150copies/μg DNAが存在するにも関わらず、準完全長プロウイルスが検出されなかった。この結果は準完全長プロウイルス量の方が全プロウイルス量よりも抗HIV療法の効果をより精確に反映していることを示している。
本発明を用いて、HIV-1プロウイルス1コピーを5か所(U5、gag、vif、env、U3)の部位で定量することが可能であることが確認された。また、増幅可能感染個体内のプロウイルスは、抗HIV治療によりウイルス複製が長期にわたり抑制される間に断片化されていくことが示唆された。準完全長プロウイルス定量は、培養操作を必要とせず、PCRだけで実行可能であるため、多数の抗HIV治療患者のリザーバーサイズのモニタリングとして有望な方法と考えられる。
実験方法
(1) マルチプレックス・ネステッドPCR(図30)
マルチプレックス・ネステッドPCR産物のアガロースゲル電気泳動を行った。具体的には、1 × TAE緩衝液を用い、2.0%アガロースゲルで、8 V/cmの電圧をかけて15分間電気泳動を行った。1回目のPCRは、1 × PCR緩衝液(インビトロジェン)、4 mMのMgCl2、0.2 mMの各dNTP、0.12 μMの各プライマー(U5-1FG、U5-1RG、gag-1FG、gag-1RG、vif-1FG、vif-1RG、env-1FG、env-1RG、U3-1FG、U3-1RG)、0.1 μlのPlatinum Taq DNAポリメラーゼ(インビトロジェン)、および50 ngのヒト細胞DNAを含む25 μl溶液中で行った。2回目のPCRは、1 × PCR緩衝液、4 mMのMgCl2、0.2 mMの各dNTP、0.12 μMの各プライマー(U5-2FG、U5-2RG、gag-2FG、gag-2RG、vif-2FG、vif-2RG、env-2FG、env-2RG、U3-2FG、U3-2RG)、0.1 μlのPlatinum Taq DNAポリメラーゼ(インビトロジェン)、および1回目のPCR産物のうちの1 μlを含む25 μlの溶液中で行った。1回目のPCRの温度設定は、97°Cで2分間の後、97°Cで5秒間、58°Cで10秒間、72°Cで15秒間からなるサイクルを5回行った後、94°Cで5秒間、58°Cで10秒間、72°Cで15秒間からなるサイクルを35回行い、最後は4°Cで冷却した。2回目のPCRの温度設定は、94°Cで2分間の後、94°Cで5秒間、58°Cで10秒間、72°Cで15秒間からなるサイクルを30回行い、最後は4°Cで冷却した。レーン1は陰性対照としてTEをPCRに加えた。レーン2-15は1反応あたり平均して1コピーのNotI切断HIV-1 DNAプラスミドpTRJO.cを加えた。レーン16は陽性対照として10コピーのNotI切断HIV-1 DNAプラスミドpRHPA.cを加えた。レーン17は100 bp DNAラダー(タカラバイオ)である。

個別2回目PCR産物のアガロースゲル電気泳動を行った。具体的には、1 × TAE緩衝液を用い、2.0%アガロースゲルで、8 V/cmの電圧をかけて15分間電気泳動を行った 個別2回目PCRは、1 × PCR緩衝液、4 mMのMgCl2、0.2 mMの各dNTP、0.12 μMの次のプライマー対のいずれか(U5-2FG/U5-2RG、gag-2FG/gag-2RG、vif-2FG/vif-2RG、env-2FG/env-2RG、U3-2FG/U3-2RG)、0.1 μlのPlatinum Taq DNAポリメラーゼ(インビトロジェン)、および1回目のPCR産物のうちの1 μlを含む25 μl溶液中で行った。PCRの温度設定は、94°Cで2分間の後、94°Cで5秒間、58°Cで10秒間、72°Cで15秒間からなるサイクルを30回行い、最後は4°Cで冷却した。レーン1-5は陰性対照として1 回目のPCR産物(1)(図1のレーン1)をPCRに加えた。レーン6-10は1 回目のPCR産物(8)(図1のレーン8)を加えた。レーン11-15は1 回目のPCR産物(10)(図1のレーン10)を加えた。レーン16-20は1 回目のPCR産物(12)(図1のレーン12)を加えた。レーン21-25は1 回目のPCR産物(15)(図1のレーン15)を加えた。レーン26-30は陽性対照として1 回目のPCR産物(16)(図1のレーン16)を加えた。レーン31は100 bp DNAラダー(タカラバイオ)である。レーン1、6、11、16、21、26にはU5-2FG/U5-2RGのプライマー対を使用した。レーン2、7、12、17、22、27にはgag-2FG/gag-2RGのプライマー対を使用した。レーン3、8、13、18、23、28にはvif-2FG/vif-2RGのプライマー対を使用した。レーン4、9、14、19、24、29にはenv-2FG/env-2RGのプライマー対を使用した。レーン5、10、15、20、25、30にはU3-2FG/U3-2RGのプライマー対を使用した。レーン31は100 bp DNAラダー(タカラバイオ)である。
結果
マルチプレックス・ネステッドPCR産物のアガロースゲル電気泳動を図30に示す。また、個別2回目PCR産物のアガロースゲル電気泳動を図31に示す。
図30において、陰性対照のレーン1には予想通りバンドがなかった。陽性対照のレーン16には予想通り、レーン17の分量マーカーと比較して、約100塩基対付近にバンドが認められた。レーン2から15の14個のPCR反応のうち10個で、陽性対照と同様なバンドが0.71の割合で検出された。ポアソン分布式によると、1反応あたり平均1個の対象DNAが存在する場合、陽性反応が検出される割合は0.63と計算され、両者はよく一致している。したがって、本実験の結果は、1分子のHIV-1 DNAが本法によりほぼ必ず検出されることを示している。
図31において、陰性対照のレーン1-5には予想通りバンドがなかった。陽性対照のレーン26-30には、レーン17の分量マーカーと比較して、それぞれのプライマー対から予想される通りの塩基対をもったバンドが認められた。図30で陽性が確かめられた4個の1回目のPCR産物を用いた場合の結果である、レーン6-10、レーン11-15、レーン16-20、レーン21-25には、陽性対照と同様に、それぞれのプライマー対から予想される通りの塩基対をもったバンドが検出された。この結果は、本法により1分子の全長のHIV-1 DNAをU5、gag、vif、env、U3の5カ所の部位で同時に検出することを示している。すなわち、この結果は、完全長のHIV-1 DNAとを断片化したHIV-1 DNAを区別して定量できることを意味している。
考察
これらの結果は、我々の開発したマルチプレックス・ネステッドPCRと個別2回目PCRを用いることにより、単一コピーのHIV-1 DNA分子を5カ所の部位(U5、gag、vif、env、U3)で同時に検出できることを示している。このことは、本技術により準完全長HIV-1プロウイルスDNAを、部分的なプロウイルスDNAと区別して定量できることを意味している。

Claims (12)
- 準完全長HIVプロウイルスを含む、HIVの病態を示すマーカー。
- HIVの病態を検査する方法であって、HIV感染細胞内の準完全長HIVプロウイルスの量を評価することを含む、前記方法。
- 感染細胞内に準完全長HIVプロウイルスがある検出限界以下であると評価された場合には、HIVの機能的治癒が達成されたと判定し、感染細胞内に準完全長HIVプロウイルスがある検出限界以上であると評価された場合には、HIVの機能的治癒が達成されていないと判定する請求項2記載の方法。
- HIVプロウイルスの全長に点在する3~7箇所の領域を検出することにより、HIV感染細胞内の準完全長HIVプロウイルスの量を評価する請求項2又は3に記載の方法。
- 抗HIV薬の薬効を検査する方法であって、HIV感染細胞内の準完全長HIVプロウイルスの量を評価することを含む、前記方法。
- 抗HIV薬投与の前後及び/又は抗HIV薬投与継続中のある期間の前後で、HIV感染細胞内の準完全長HIVプロウイルスの量を評価し、準完全長HIVプロウイルスの数が減少した場合には、HIVの機能的治癒が達成された、あるいは達成しつつあると判定し、準完全長HIVプロウイルスの数が減少しなかった場合には、HIVの機能的治癒を可能としないと判定する請求項5記載の方法。
- HIVの機能的治癒を可能とする薬剤をスクリーニングする方法であって、HIV感染細胞内の準完全長HIVプロウイルスの量を評価することを含む、前記方法。
- 薬剤投与の前後及び/又は薬剤投与継続中のある期間の前後で、HIV感染細胞内の準完全長HIVプロウイルスの量を評価し、準完全長HIVプロウイルスの数が減少した場合には、HIVの機能的治癒を可能としうると判定し、準完全長HIVプロウイルスの数が減少しなかった場合には、HIVの機能的治癒を可能としないと判定する請求項7記載の方法。
- HIVプロウイルスの全長に点在する3~7箇所の領域を検出することにより、HIV感染細胞内の準完全長HIVプロウイルスの量を評価する方法。
- 配列番号1~40のいずれかのヌクレオチド配列の一部又は全部を含む20~30塩基対のプライマーを用いて、HIV感染細胞内の準完全長HIVプロウイルスの量を評価する請求項1~9のいずれかに記載の方法。
- 準完全長HIVプロウイルスの量を評価するためのキットであって、HIVプロウイルスの全長に点在する3~7箇所の領域を増幅できるプライマー及び前記プライマーにより増幅された核酸を検出できる試薬を含む、前記キット。
- HIVプロウイルスの全長に点在する3~7箇所の領域を増幅できるプライマーが、配列番号1~40のいずれかのヌクレオチド配列の一部又は全部を含む20~30塩基対のプライマーである請求項11記載のキット。
