Insgesamt wird aufgrund des bei normalen Patienten ohne Anorexia nervosa vorliegenden Appetitlosigkeit durch MPH davon ausgegangen, dass bei Gabe bei Patienten mit Anorexia nervosa eine Verstärkung der Appetitlosigkeit vorliegt. Auch bei übergewichtigen Patienten findet sich eine Gewichtsabnahme bei den Patienten. Überraschend fand sich die paradoxe Wirkung von Methylphenidat auf Verhalten und Gewicht bei Patienten mit Anorexia nervosa. Es kommt eben nicht zu einer weiteren Gewichtsabnahme, sondern entweder zur Stabilisierung des Gewichtes oder sogar Gewichtszunahme. In einem Fall wurde bisher von Veränderung von geriatrischen Patienten, die nicht essen, ausgegangen. Dies sind jedoch explizit keine Patienten mit Anorexia nervosa, sondern Patienten, bei denen Anorexie im Sinne von Kachexie zu übersetzen ist (Reversal of Anorexia by Methylphenidate in Apathetic, Severely Demented Nursing Home Patients). Die vollständig überraschende Therapieoption ergab sich aufgrund der Nichtbeachtung einer Kontraindikation bei einer Patientin. Die schwer anorektische Patientin nahm Methylphenidat, da sie selber abhängig davon war, zur Leistungssteigerung ein. Nach Vorstellung und Erstellung der Diagnose Stimulanzienabhängkeit und Anorexia nervosa wurde das vorher durch den Hausarzt aufgrund eines vorgeschobenen fehldiagnostizierten ADHS verschriebene Methylphenidat abgesetzt. Aufgrund der wöchentlichen psychiatrischen Überwachung fiel eine deutliche Verschlechterung der Anorexia-Symptome auf.

Bei erneuter Überprüfung ergab sich eine Differenz Realität / Selbsteinschätzung von 30%. Die Einnahme von Diuretika und Laxantien ging nachweisbar zurück. Das Nutzen des Fitnessstudios ging auf zweimal pro Woche für eine Stunde zurück. Somit ergab sich insgesamt eine erhebliche Verbesserung des Krankheitsbildes.
