Description

DESCRIPTION
A PREPUBIC NEEDLE SYSTEM NOT CAUSING BLADDER INJURY Technical Field:
This invention is related with the passing rack stripe between vesica, in other words, bladder and pubic bone in a secure way during urocleusia operations.
Background Art:
Retropubic rack techniques are being applied for a long time for the purpose of supporting urinary exit tubulus, namely urethra for the purpose of preventing urocleusia. This rack is being located by means of a needle. Needle, which is emplopyed for progressing rack from both sides of the urinary tubulus till stomach wall, progresses between bone and bladder. Howver, this sharp needle has the potential to damage vesica, in other words, bladder.
Technical problems aimed to be solved by the invention:
Invention is opening the region, through which it shall pass, without damaging by means of a needle special mechanisms. One of the mechanisms is the mechnical system, keeping organ/bladder away from needle axis by means of a button movement and covering both sides of the needle, and other one is the tubular system, spraying liquid assisting to push organs away with widening the operation area with liquid.

2. Internal structure of the system in lateral section view (zoomed section of the tube inside the circle, zoomed view of pusher arm at lower section).
3. Button movements and needle protection movements accompanying thereof in lateral section view.
4. It is needle protector, and respectively in form of oblique, cross-section and installed onpusher arm(s) (1 , 2 and 3 numbered small pictures)
5. It is showing that needle is pushing organ away from needle axis by means of a button movement of the protector (dark colored section is bone, emply line is organ) and liquid is being sprayed. 6. It is shown on the cross-section that it allows sharp needle point with remaining at back while spring needle protector was passing through flesh and the most darkest areas are showing the hard tissues, to be penetrated, and after passing through hard section, it comes back again so as to protect the needle end (1 > 2 > 3 > 4 numbered small figures).
7. Rack material installed onto needle.
8. Another form of tube in the cross-section (in "u" form anf protector pushing arm and water channel are within this rift).
9. It is needle protector, adapted to single puser arm, and there is one single hole

11. An alternate solution view to joint of pusher arm and needle protector, and rounded pin of the puhser enters into the channel under protector. There are rifts for stretching at both sides of the channel (No. 1 is rounded puhser arm, no. 2 is the protector part, manufactured with channels according to pin, and no. 3 is interlocked side view in the figure).

While in starting position, needle (1) is within protector walls (2) and in form of not being abl§ e to damage any other tissue or hands of the surgeon (Fig. 1 , 3 and 6). Protector walls are continuously being pushed forward by pusher arm/s (3). Pusher arm/s progress till protector walls through body, and within the pipe (4), installed to the body during intermediate section^ Figure 2). Pusher spring (6) within body (5) maintains continual forward pressing position of pusher arms (Figure 2, 3). Needle, except single usage model, is installed to the end of the pipe and fixed. Protector walls (2) can only move forward from starting position if button (7) is pressed (Figure 3). Button movement is transmitted to the walls via pusher arm (3). Wall movement occurs in a channel (8) within needle, and this channel is inclined (Figure 2, 5, 13). This inclination is for pushing walls forward.
After grasping the body manually, needle end is applied to the hard tissue region, where it shall pass through, and tool is pushed. Blunt walls (2) first stopped by the tissue. Sharp needle progresses and penetrates into the tissue. Walls also enter from the slot, opened by the needle. Edges of the walls are slightly rounded in order to facilitate entrance. After passing through hard tissue, walls return to their original positions by means of spring (6), and protect needle so as not to damage around (Figure 6). When button (7) is pressed at this stage, walls progress forward and onwards. Thus, they keep sensitive organ (9) away from shap needle's line (Figure 5). There is water spraying installation as an additional system for protecting organ (9). Pressurized serum, injected from the body rear entrance (10), reaches to spraying end (12) via hose within pipe (11 ), and while exiting from there, it pushes organ (9) without hazard (Figure 5). Rack (14), which is installed onto the hole over needle (13), is also securely progresses from this tunnel (Figure 7). Parts of system, which enter into the patient, namely, needle (1 ), walls (2) and pusher (3) or needle and pusher along with whole pipe, can be manufactures so as to be installed to and demounted from body (5) easily. Groove like system (15) with "U" shape cross-section can also be used instead of pipe in order to facilitate manufcturing.
Joining point of pusher arm (3) to protective walls can have various shapes in order to facilitate manufacturing and usage. There is an upper slot (16) at the very end of pusher arm on one of these and protective wall axle (17) is entered into this with stretching (Figure 4). In another way, protective wall axle is in the middle and 1 . (Figure 9). In such a case, pusher arm is also one. In another model, there is no axle slot at the end of pusher arm (Figure 10). Edges are internally inclined and protective wall axles enter with lateral stretching (Figure 10). In another model, there is no connection location with revolving point, pusher arm's end is in form of a slenderized and rounded axle (18) (Figure 11 , 12). There is also a channel (19) within protective wall region against this, and axle enters into here (Figure 11 , 12). There are slots (20) here in order to maintain entrnce with stretching (Figure 12).
For single usage system, if only end section shall be manufactured replaceable, there can be 2 different systems available. One is the system without spikes, which enters with compressing (Figure 13), and other one is the system with spikes, and . removed with compressing laterally after completion of process (Figure 13, in circle). There is also an electrical circuit and warning system, which alarms if entered into the bladder accidentally, within the prepubic needle system, not causing bladder injury. This system is composed of electrodes (21 ), isolated region around electrodes (22), cables (23), battery (24) and warning apparatus (25). An electric transmitting liquid is injected into bladder prior to the operation. If bladder is penetrated during operation, circuit between electrodes is completed by this liquid, and warning appartus (with audio, light, vs.) activates, and warns.
The most important difference of the invention is that needle (1 ) is fixed, and needle protector walls (2) are mobile. Sections, which contact with patient, are for single usage (use/dispose). A battery system is suggested, so it has a warning system.
