Each discrete axial aperture 615 includes a pair of opposing flat inner surfaces 618 corresponding to a pair of opposing longitudinally extending flat sections 630 of each prong 603.
  • Flat sections 630 of each prong 603 are configured to mate with respective flat inner surfaces 618 of each discrete axial aperture 615 to rotationally fix lock washer 608 when prongs 603 of tulip head portion 604 are inserted in their respective discrete axial apertures 615, while allowing axial movement of lock washer 608 along the prongs 603 of tulip head portion 604 to secure rod 605 within the transverse channel 607 between lock washer 608 and tulip head portion 604.
