对于骨折碎块的钢丝固定(包括临时固定及长久固定), 一直没有专门的 器械(或设备)。 过去, 为了达到功能复位、 有效固定碎骨块的目的, 须在碎 块所在的局部切开至少 4 - 5cm的皮肤切口, 方能用手直接置入钢丝进行环绕 固定, 基本上是在直视下操作, 这样, 必定会在局部对软组织 (包括骨膜) 造成不必要的 "副损伤 (主要是滋养血管的损伤) ", 进而影响骨折愈合, 甚 至导致骨折不愈合。 但在开放复位手术前提下, 这种副损伤的存在是可以接 受的。 正所谓: 手术是一把双刃剑, 在更好地使骨折复位的同时, 却使骨折 明显地延迟愈合了。
随着骨折内固定设备及临床医疗技术的不断发展, X线引导下小切口闭合 复位内固定微创手术技术已经广泛应用于临床, 但对于在闭合条件下骨折碎 块的功能复位及稳固固定一直是临床上的一个技术难题。 本实用新型可以有 效地解决这个问题, 使骨折的闭合复位内固定效果达到开放复位固定的效果。 实用新型内容
本实用新型要解决的技术问题在于: 针对现有技术中没有合适的器械或 设备来协助完成骨折碎块的钢丝固定的问题, 本实用新型提出一种钢丝环扎 固定装置, 有效解决这一难题。 为了解决上述技术问题, 本实用新型提出以下技术方案: 一种钢丝环扎 固定装置, 包括一个第一钢丝引导装置、 一个第二钢丝引导装置、 一个钢丝 旋紧绞断装置;
该第一钢丝引导装置包括一个第一套筒、 一个第一刺枪;
该第一套筒包括一个第一外筒、 一个设在该第一外筒的后端部的第一套 筒手柄;
该第一刺枪包括一个第一内筒、 一个设在该第一内筒的后端部的第一刺 枪手柄;
该第一内筒上开设有一个插孔;
第一内筒插入该第一外筒中, 该插孔露在该第一外筒的前方;
该第二钢丝引导装置包括一个第二套筒、 一个第二刺枪;
该第二套筒包括一个第二外筒、 一个设在该第二外筒的后端部的第二套 筒手柄;
该第二刺枪包括一个第二内筒、 一个设在该第二内筒的后端部的第二刺 枪手柄;
该第二内筒的前端设有一个插接块;
该插接块包括一个扁平部位以及一个凸鼓部位;
该第二刺枪的第二内筒插设在该第二套筒的第二外筒中, 该第二刺枪手 柄与该第二套筒手柄接触, 该插接块露在该第二外筒的前端;
该钢丝旋紧绞断装置包括一个外套筒、 一个内芯;
该外套筒包括一个套管、 一个把持部;
该套管的前端部设有一个挡片, 该挡片上开设有两个通孔;
该把持部连接在该套管的后端;
该内芯包括一个旋转手柄、 一个安装在该旋转手柄上的工作杆; 该工作杆的两侧分别各有一个与杆的中心轴线平行的凹槽, 该凹槽沿纵 轴方向直线贯穿旋转手柄; 该工作杆的前端分为三部分, 中间是一铰刀部, 该铰刀部的前端面与工 作杆的杆体相垂直, 该铰刀部的两侧是两个钢丝锁定部, 该两个钢丝锁定部 的前端面都为凸弧形斜面, 该凸弧形斜面的远端与铰刀部的前端面相连, 该 凸弧形斜面的近端与铰刀部不相连, 该凸弧形斜面的近端与工作杆两侧的凹 槽相连续。
上述技术方案的进一步限定在于, 该把持部是一内部中空的圓柱体, 该 把持部的尾端设有一锁定板, 该锁定板开设有一个圓形的开孔, 沿开孔的边 缘开设有若干个凹槽;
该内芯包括若干根锁销, 该若干根锁销安装在该旋转手柄上, 该锁销包 括一个销杆、 一个设在该销杆端部的销帽。
上述技术方案的进一步限定在于, 该第一外筒和第一套筒手柄上开设有 一条平行于中心轴线的直线形的第一外筒开口, 该第一外筒开口贯穿该第一 套筒手柄, 在该第一套筒手柄处形成一个第一套筒手柄开口;
该第一内筒上在该插孔和该第一刺枪手柄之间开设有一条平行于中心轴 线的直线形的第一内筒开口, 该第一内筒开口与插孔相连并贯穿该第一刺枪 手柄, 在该第一刺枪手柄处形成一个第一刺枪手柄开口;
该第二外筒和第二套筒手柄上开设有一条平行于中心轴线的直线形的第 二外筒开口, 该第二外筒开口贯穿该第二套筒手柄, 在该第二套筒手柄处形 成一个第二套筒手柄开口;

上述技术方案的进一步限定在于, 该插接块是半球体形的。
上述技术方案的进一步限定在于, 该第一内筒的前端设有一个尖头, 该 尖头是在该第一内筒的前端部与该中心轴线呈一定角度切削掉一块而形成 的。
上述技术方案的进一步限定在于, 该第二钢丝引导装置包括一个填充芯, 该填充芯包括一杆体、 一设在该杆体的端部的握柄, 该杆体对应插入该第二 刺枪的第二内筒中。

1、 本实用新型产品的结构简单、 清洗方便、 操作简单、 效果可靠。
2、 钢丝旋紧绞断装置可以极为方便地伸入肢体深部, 在非直视前提下, 仅仅通过微小切口达到深部骨折碎块处, 将固定之钢丝环扎、 旋紧、 绞断三 个步骤一气呵成。
3、 第一钢丝引导装置、 第二钢丝引导装置可以通过微小切口插入患者肢 体中, 创口可以减到最小、 把副损伤减少到最小程度。
4、 第一钢丝引导装置、 第二钢丝引导装置从两端分别插入后可以方便的 锁住和分开, 钢丝通过第一钢丝引导装置、 第二钢丝引导装置内部穿过后环 扎在骨折处, 使用方便, 稳定可靠。

填充芯 26的作用主要有两点: 1、 增强第二钢丝引导装置 2的整体强度, 防止使用时装置变形折断; 2、 防止软组织嵌入第二钢丝引导装置 2内造成堵 塞和意外损伤。
该钢丝旋紧绞断装置 3包括一个外套筒 32、 一个内芯 34。
该外套筒 32包括一圓形的套管 322、 一个把持部 324。
该套管 322的前端部设有一个挡片 3222 ,该挡片 3222上开设有两个通孔
3224。
该把持部 324连接在该套管 322的后端, 该把持部 324是一内部中空的 圓柱体, 方便人手握持。
该把持部 324的内部中空, 其尾端设有一个锁定板 346。 该锁定板 346开 设有一个圓形的开孔 3462 , 沿开孔的边缘开设有两个凹槽 3464。

下面介绍实际手术过程中应用本实用新型钢丝环扎固定装置的过程: 患肢需在牵引状态下手术, 首先通过牵引恢复肢体的长度。
第 1 步: 在常规手术前准备后利用微创技术, 沿骨干长轴骨膜下穿入适 长锁定钢板(图未示), 钢板的长度确定一般为可以在完整骨干处固定三枚锁 定螺钉(图未示)。
第 2 步: 在需要复位固定的骨折块范围确定切开皮肤的位置, 尽量使之 与预定的钢板固定螺钉皮孔重叠, 这样可以减少重复的切口以减少软组织损 伤。 皮肤切口长约 lcm。
第 3步: 首先将第一钢丝引导装置 1的第一刺枪手柄延伸片 1442旋转到 与第一套筒手柄延伸片 1242相背的位置, 第一套筒手柄开口 1244和第一刺 枪手柄开口 1444相背,同时该第一外筒开口 126和第一内筒开口 148不对齐。
第二钢丝引导装置 1的第二刺枪手柄延伸片 2442旋转到与第二套筒手柄 延伸片 2242相背的位置, 第二套筒手柄开口 2244和第二刺枪手柄开口 2444 相背, 同时该第二外筒开口 226和第二内筒开口 248不对齐。 在皮口处插入本实用新型的第一钢丝引导装置 1 直达骨折块的前侧, 尽 量沿骨膜下绕过骨折块的近侧 1/4周。
在此皮肤切口的肢体对侧, 同样切一长约 1cm皮口, 在第一钢丝引导装 置 1的相对位置处插入第二钢丝引导装置 2 ,并同样使之尽量于骨膜下绕过骨 块对侧的 1/4周。
将第二钢丝引导装置 1的插接块 246插入该第一钢丝引导装置 1的第一 内筒 142上的插孔 146 内并旋转约 90。 , 使第一钢丝引导装置 1、 第二钢丝 引导装置 2相互锁定固定连接成一体。
第 4步: 抽出第二钢丝引导装置 2的该第二刺枪 24 , 沿该第二钢丝引导 装置 2的内腔顺利穿入 1 ~ 2根直径 1mm的引导钢丝备用。 旋转第一钢丝引导 装置 1的第一刺枪 14以及第二钢丝引导装置 2的第二刺枪 24 ,使第一套筒手 柄开口 1244和第一刺枪手柄开口 1444对齐, 并且第二套筒手柄开口 2244和 第二刺枪手柄开口 2444对齐, 释放出引导钢丝。
注意: 引导钢丝的数量根据需要环扎的圈数决定, 每根可以环扎一圈。 一般情况下环扎两圈为佳。
第 5步: 反旋第二钢丝引导装置 2的第二刺枪 24约 90。 , 第一钢丝引导 装置 1、 第二钢丝引导装置 2解锁分离开, 分别后退第一钢丝引导装置 1、 第 二钢丝引导装置 2 , 使得第一钢丝引导装置 1、 第二钢丝引导装置 2的前端刚 刚抵达钢板或 (及)骨干表面, 如同第 3、 4步骤操作, 再次穿入 1 ~ 2根直 径 lmm的引导钢丝。 全部撤出第一钢丝引导装置 1、 第二钢丝引导装置 2。
第 6步: 利用上述引导钢丝, 顺利穿入环扎钢丝(图未示)(钢丝直径根 据局部受力的大小决定, 最大直径为 2匪), 环扎钢丝的尾端外露于钢板侧的 皮孔外。
第 7步: 将两根外露的环扎钢丝头端插入该钢丝旋紧绞断装置 3前端当 片上的两个通孔内, 环扎钢丝贯穿该钢丝旋紧绞断装置 3后尾端外露于装置 外, 以利于初步收紧环扎钢丝。逆时针锁紧该钢丝旋紧绞断装置 3的内芯 34。 第 8步: 连同外套筒 32继续逆时针旋转, 同时提拉绷紧环扎钢丝, 注意 用力均勾及适当。 环扎钢丝打结后逐渐放松提拉力度, 使得环扎钢丝的钢丝 结能够盘起以减少对周围软组织的刺激。
第 9步: 在保持外套筒 32相对固定状态下(顺时针)旋转其内芯 34 , 注 意避免锁销 348经过凹槽 3464时脱出, 剪断环扎钢丝的尾端, 撤出器械。
第 10步: 如有其它需要复位固定的骨折块, 如法操作, 待全部重要的骨 折块都处理好后再固定锁定钢板。
本实用新型具有如下有益效果:
1、 本实用新型产品的结构简单、 清洗方便、 操作简单、 效果可靠。
2、 钢丝旋紧绞断装置 3可以极为方便地伸入肢体深部, 在不需要直视的 前提下, 仅仅通过微小切口达到深部骨折碎块处, 将固定钢丝环扎、 旋紧、 绞断三个步骤一气呵成。

综上所述, 本实用新型实施例确能达到所预期的使用功效, 又其所揭露 的具体构造, 不仅未曾见诸于同类产品中, 也未曾在申请日前公开, 已完全 符合专利法的规定与要求, 恳请惠予审查, 并赐准专利。
