Definitions
- the present invention relates to a technical field of a blood pressure estimation apparatus and method for estimating blood pressure of a living body (in particular, blood pressure is continuously estimated).
- a blood pressure estimation apparatus of this type for example, as disclosed in Patent Document 1, a continuous change in the area of a blood vessel obtained from a reflected wave of reflected ultrasonic waves (reflected ultrasonic waves) irradiated on a living body is used as a physiological parameter.
- a device for converting the blood pressure into continuous blood pressure values has been proposed.
- a pulse wave signal is extracted by detecting light irradiated on a living body, and a time of a second-order differential value of the pulse wave signal is extracted.
- An apparatus for deriving a change in blood pressure from a change has been proposed.
- a blood pressure estimation apparatus for solving the above-described problem is a blood pressure measurement unit that measures blood pressure of a living body for each first period, and measures blood flow of the living body for each second period that is shorter than the first period.
- the blood pressure measurement unit, the blood pressure measured by the blood pressure measurement unit, and the blood flow measured by the blood flow measurement unit are used to estimate the blood pressure for each third cycle shorter than the first cycle.
- a blood pressure estimation method for solving the above-described problems includes a blood pressure measurement step of measuring a blood pressure of a living body for each first cycle, and a blood flow rate of the living body for each second cycle shorter than the first cycle.
- the blood flow measurement step, the blood pressure measured by the blood pressure measurement step, and the blood flow measured by the blood flow measurement step, the blood pressure for each third cycle shorter than the first cycle is estimated.
- movement of the blood pressure estimation apparatus which estimates the blood pressure of the desired time based on the blood flow volume of the past time from reference time, and the blood flow volume of the past time from desired time.
- Blood related to the operation of the blood pressure estimation device that estimates the blood pressure at a desired time based on the average value of the blood flow during a predetermined period determined based on the reference time and the average value of the blood flow during the predetermined period determined based on the desired time
- the blood pressure estimation device includes a blood pressure measurement unit that measures the blood pressure of the living body for each first cycle, and a blood flow that measures the blood flow of the living body for each second cycle that is shorter than the first cycle. Based on the blood pressure measured by the measurement unit, the blood pressure measurement unit, and the blood flow measured by the blood flow measurement unit, blood pressure estimation for estimating the blood pressure for each third cycle shorter than the first cycle A part.
- the blood pressure measurement unit measures the blood pressure of the living body. At this time, the blood pressure measurement unit measures the blood pressure for each first period. In other words, the blood pressure measurement unit measures blood pressure for each first cycle. More specifically, the blood pressure measurement unit measures blood pressure at the first timing. Thereafter, the blood pressure measurement unit measures the blood pressure again at the second timing when the period corresponding to the first period has elapsed with the first timing as a starting point. Thereafter, the blood pressure measurement unit repeats the same operation.
- the blood flow measurement unit measures the blood flow of the living body. At this time, the blood flow measurement unit measures the blood flow volume for each second period. In other words, the blood flow measurement unit measures the blood flow volume every second cycle. More specifically, the blood flow measurement unit measures the blood flow volume at the third timing. Thereafter, the blood flow measurement unit measures the blood flow again at the fourth timing when the period corresponding to the second period has elapsed with the third timing as a starting point. Thereafter, the blood flow measurement unit repeats the same operation.
- the second cycle in which the blood flow measurement unit measures the blood flow is shorter than the first cycle in which the blood pressure measurement unit measures the blood pressure. That is, the frequency with which the blood flow measurement unit measures the blood flow is higher than the frequency with which the blood pressure measurement unit measures blood pressure. In other words, the number of times the blood flow measurement unit measures the blood flow during a certain period is greater than the number of times the blood pressure measurement unit measures the blood pressure during the certain period.
- the blood pressure estimation unit estimates the blood pressure for each third cycle based on the blood pressure measured by the blood pressure measurement unit for each first cycle and the blood flow measured by the blood flow measurement unit for each second cycle. To do.
- the third period in which the blood pressure estimation unit estimates the blood pressure is shorter than the first period in which the blood pressure measurement unit measures the blood pressure. That is, the frequency with which the blood pressure estimation unit estimates the blood pressure is higher than the frequency with which the blood pressure measurement unit measures the blood pressure. In other words, the number of times the blood pressure estimation unit estimates the blood pressure during a certain period is greater than the number of times the blood pressure measurement unit measures the blood pressure during the certain period. Therefore, the blood pressure estimation unit can estimate the blood pressure at the time when the blood pressure measurement unit does not measure the blood pressure, for example. As will be described in detail later, the third period in which the blood pressure estimation unit estimates the blood pressure may be the same as the second period in which the blood flow measurement unit measures the blood flow.
- the blood pressure estimation device includes the blood pressure measured by the blood pressure measurement unit (that is, the blood pressure for each first cycle and a relatively discrete blood pressure) and the blood flow measurement unit. Based on the measured blood flow rate (that is, the blood flow rate in the second cycle and is relatively continuous), the blood pressure in the third cycle (that is, relatively continuous blood pressure). Can be estimated.
- the blood pressure estimation apparatus can continuously estimate blood pressure based on the blood pressure discretely measured by the blood pressure measurement unit and the blood flow volume continuously measured by the blood flow measurement unit.
- the blood pressure estimation device is based on the blood pressure measured by the blood pressure measurement unit every first cycle and the blood flow measured by the blood flow measurement unit every second cycle. Blood pressure can be estimated for each cycle (that is, blood pressure can be estimated relatively continuously).
- the blood pressure estimation apparatus of the present embodiment can estimate continuous blood pressure using a continuous blood flow that can be measured relatively easily using a laser Doppler blood flow meter or the like. Therefore, the blood pressure estimation apparatus of the present embodiment can estimate continuous blood pressure relatively easily.
- the “first period” in the present embodiment may be a fixed value or a value that is appropriately changed. That is, the “first cycle” in the present embodiment is mainly the cycle of each operation in the case where the same operation (in this embodiment, blood pressure measurement) is repeated periodically or aperiodically. Is intended to indicate. The same applies to the “second period” and the “third period”.
- the blood pressure estimation unit is configured to perform the blood pressure measurement at the reference time measured by the blood pressure measurement unit and the blood flow rate at the reference time measured by the blood flow measurement unit.
- the blood pressure at the desired time is estimated based on the ratio or change rate of the blood flow at the desired time measured by the blood flow measurement unit.
- the blood pressure estimation unit can estimate the blood pressure at the desired time based on the blood pressure at the reference time and the ratio or rate of change of the blood flow at the desired time with respect to the blood flow at the reference time. This is because there is often a certain correlation between the temporal change in blood pressure and the temporal change in blood flow. For this reason, there is a high possibility that the blood pressure changes in a manner similar to the manner in which the blood flow changes. Therefore, according to this aspect, the blood pressure estimation unit can appropriately estimate the blood pressure.
- the blood pressure estimation unit may handle a product of the ratio of the blood flow rate at the desired time to the blood flow rate at the reference time and the blood pressure at the reference time as the blood pressure at the desired time.
- the blood pressure estimation unit calculates a desired value obtained by adding a product of a change rate of a blood flow rate at a desired time with respect to a blood flow rate at the reference time and a blood pressure at the reference time to the blood pressure at the reference time. It may be handled as the blood pressure of the time.
- the blood pressure estimation unit has a predetermined reflection.
- the blood pressure at the desired time is estimated based on the ratio or the rate of change that is weighted according to a coefficient.
- the blood pressure estimation unit reflects not only the blood pressure at the reference time and the ratio or change rate of the blood flow at the desired time to the blood flow at the reference time, but also the reflection coefficient indicating the degree of weighting of the ratio or the change rate.
- the blood pressure at the desired time can be estimated based on the above. This is because the correlation between the temporal change in blood pressure and the temporal change in blood flow may vary depending on individual differences in the living body. For this reason, the blood pressure estimation unit adjusts the reflection coefficient so as to absorb or compensate for such individual differences, thereby taking into account individual differences in the living body (for example, absorbing or compensating for such individual differences). However, the blood pressure can be estimated appropriately.
- the reflection coefficient which is an example of information that can identify the individual difference
- the reflection coefficient is information that can identify individual differences between living bodies
- the reflection coefficient is stored in a manner that can be distinguished for each living body (for example, in a manner associated with a living body). Also good.
- the blood pressure estimation unit can use the reflection coefficient stored in the storage unit to improve blood pressure estimation accuracy.
- the storage of the reflection coefficient by the storage unit is preferably performed particularly when the reflection coefficient is corrected (that is, the reflection coefficient is appropriately updated) as described later.
- the storage of the reflection coefficient by the storage unit may be performed when the reflection coefficient is not corrected (that is, the reflection coefficient has a fixed value).
- the storage unit may store a reflection coefficient that is a fixed value (that is, a reflection coefficient as a default).
- the storage unit further stores the blood pressure estimated by the blood pressure estimation unit within a predetermined estimation period.
- the blood pressure estimation unit summarizes the weighting according to the reflection coefficient for the blood pressure estimated within the estimation period stored in the storage unit after estimating the blood pressure within the estimation period. By doing so, the blood pressure estimated within the estimation period is corrected collectively afterwards.
- the blood pressure estimation unit performs weighting according to the reflection coefficient every time the blood pressure is estimated (for example, weighting that affects the estimation of the current blood pressure or the future blood pressure) or instead, weighting according to the reflection coefficient (that is, weighting that affects blood pressure that has been estimated in the past) can be collectively performed after the series of blood pressures estimated within the estimation period. .
- the blood pressure estimation unit is configured to measure the blood pressure at the reference time measured by the blood pressure measurement unit and the blood pressure. The reflection coefficient is corrected so that a difference between the blood pressure at the reference time estimated by the estimation unit is small.
- the blood pressure estimation unit can correct the weighting reflection coefficient so as to absorb or compensate for an error between the blood pressure measured by the blood pressure measurement unit and the blood pressure estimated by the blood pressure estimation unit. it can.
- the blood pressure estimation unit may correct the reflection coefficient so that the difference (particularly the absolute value of the difference) between the blood pressure measured by the blood pressure measurement unit and the blood pressure estimated by the blood pressure estimation unit becomes zero. .
- the blood pressure estimation unit can more appropriately estimate the blood pressure.
- the blood pressure estimation unit further includes a storage unit that stores the reflection coefficient corrected by the blood pressure estimation unit, and the blood pressure estimation unit stores the reflection stored in the storage unit.
- the blood pressure at the desired time is estimated based on the ratio or the rate of change that is weighted according to a coefficient.
- the reflection coefficient which is an example of information that can identify the individual difference, is a history (for example, a database indicating a history) for each blood pressure measurement by the blood pressure measurement unit or for each blood pressure estimation by the blood pressure estimation unit. ) May be stored, for example, by a recording unit or the like. In this case, when estimating the blood pressure next time, the blood pressure estimation unit can use the reflection coefficient stored in the storage unit to improve blood pressure estimation accuracy.
- the reflection coefficient is information that can identify individual differences between living bodies
- the reflection coefficient is in a form that can be distinguished for each living body (for example, in a form associated with a living body). It may be stored.
- the reflection coefficient is highly likely to be corrected as time passes. Therefore, the reflection coefficient may be stored in a manner distinguishable for each time (in other words, in a manner associated with time).
- the blood pressure estimation unit further includes a storage unit that stores the blood pressure estimated by the blood pressure estimation unit within a predetermined estimation period, and the blood pressure estimation unit includes the blood pressure within the estimation period. After the estimation, the blood pressure estimated within the estimation period is compiled afterwards by performing a predetermined filtering process on the blood pressure estimated within the estimation period stored in the storage unit. To correct.
- the blood pressure estimation unit collectively performs a predetermined filtering process (that is, a filtering process that affects blood pressure that has been estimated in the past) for a series of blood pressures estimated within the estimation period. Or it can be done after the fact.
- a predetermined filtering process that is, a filtering process that affects blood pressure that has been estimated in the past
- the blood pressure estimation unit is a predetermined time in the past from the blood pressure at the reference time measured by the blood pressure measurement unit and the reference time measured by the blood flow measurement unit.
- the blood pressure at the desired time is estimated based on the ratio or rate of change of the blood flow at a time that is traced back in the past by a predetermined time from the desired time measured by the blood flow measurement unit with respect to the blood flow at the retroactive time.
- the blood pressure estimation unit is predetermined from the desired time with respect to the blood flow volume at a time that is traced back by a predetermined time from the reference time.
- the blood pressure at the desired time can be estimated based on the ratio or change rate of the blood flow at the time that goes back in time by the time. This is because the correlation between the temporal change in blood pressure and the temporal change in blood flow may vary depending on individual differences in the living body. More specifically, the time until a change in blood flow appears as a change in blood pressure may vary depending on individual differences in the living body. Therefore, according to this aspect, the blood pressure estimation unit can appropriately estimate the blood pressure while taking into account individual differences in the living body (for example, while absorbing or compensating for such individual differences).
- the blood pressure estimation unit is determined based on the blood pressure at the reference time measured by the blood pressure measurement unit and the reference time measured by the blood flow measurement unit.
- the blood pressure at the desired time based on the ratio or rate of change of the average value of the blood flow within a predetermined period determined with reference to the desired time measured by the blood flow measurement unit with respect to the average value of the blood flow within the period Is estimated.
- the blood pressure estimation unit calculates the desired time with respect to the average value of the blood flow rate within a predetermined period determined based on the reference time instead of the ratio or change rate of the blood flow rate at the desired time with respect to the blood flow rate at the reference time.
- the blood pressure at a desired time can be estimated based on the ratio or change rate of the average value of the blood flow rate within a predetermined period determined by the reference.
- the blood pressure estimation part can estimate blood pressure suitably, eliminating the influence of the pulsation contained in a blood flow rate, or a biological dependence fluctuation.
- the blood pressure estimation unit (i) estimates the blood pressure when the blood pressure measured by the blood pressure measurement unit satisfies a predetermined condition, and (ii) the blood pressure When the blood pressure measured by the measurement unit does not satisfy the predetermined condition, the blood pressure is not estimated.
- the blood pressure estimation unit can estimate the blood pressure when the blood pressure satisfies the predetermined condition (for example, when it is preferable to estimate the blood pressure). In other words, the blood pressure estimation unit does not have to estimate the blood pressure when the blood pressure does not satisfy the predetermined condition (for example, when it is not necessary to estimate the blood pressure). Therefore, the power consumption of the blood pressure estimation device is reduced as compared with the blood pressure estimation device of the comparative example in which the blood pressure estimation unit constantly estimates the blood pressure.
- the blood flow measurement unit measures (i) the blood flow when the blood pressure measured by the blood pressure measurement unit satisfies the predetermined condition, and (ii ) When the blood pressure measured by the blood pressure measurement unit does not satisfy the predetermined condition, the blood flow rate is not measured.
- the blood flow measurement unit can measure the blood flow when the blood pressure satisfies the predetermined condition (for example, when it is preferable to estimate the blood pressure).
- the blood flow measurement unit does not have to measure the blood flow when the blood pressure does not satisfy the predetermined condition (for example, when the blood pressure does not have to be estimated). Therefore, the power consumption of the blood pressure estimation device is reduced as compared with the blood pressure estimation device of the comparative example in which the blood flow measurement unit constantly measures the blood flow.
- the blood pressure measurement unit further includes timer means for setting a timing at which the blood pressure measurement unit measures the blood pressure, and the blood pressure measurement unit has the blood pressure at the timing set by the timer means. Is automatically measured for each of the first periods.
- the blood pressure measurement unit can automatically measure the blood pressure for each first cycle.
- the blood pressure measurement unit includes, for example, a sphygmomanometer (for example, a non-invasive sphygmomanometer that wraps a cuff around an arm and pressurizes the arm through the cuff) with a manual operation of a measurer.
- a sphygmomanometer for example, a non-invasive sphygmomanometer that wraps a cuff around an arm and pressurizes the arm through the cuff
- the blood pressure measurement unit automatically performs measurement without timing by the measurer (or without manual operation by the measurer) according to the timing set by the timer unit.
- the blood pressure for each first cycle can be measured.
- the timer unit is configured such that (i) the frequency of the timing at which the blood pressure is measured when the blood pressure measured by the blood pressure measurement unit satisfies a predetermined condition is ( ii) The timing is set so that the blood pressure measured by the blood pressure measurement unit is higher than the frequency of the timing at which the blood pressure is measured when the predetermined condition is not satisfied.
- the timer means can suitably set the timing at which the blood pressure measurement unit measures blood pressure according to the blood pressure measured by the blood pressure measurement unit. For example, when the blood pressure satisfies a predetermined condition (for example, when it is preferable to estimate the blood pressure), the timer means measures the blood pressure so that the blood pressure measurement unit measures the blood pressure relatively frequently. Can be set. On the other hand, for example, when the blood pressure does not satisfy a predetermined condition (for example, when it is not necessary to estimate the blood pressure), the timer means causes the blood pressure measurement unit to measure the blood pressure relatively lowly. Can be set.
- a predetermined condition for example, when it is preferable to estimate the blood pressure
- the timer means causes the blood pressure measurement unit to measure the blood pressure relatively lowly. Can be set.
- the blood pressure estimation method of the present embodiment includes a blood pressure measurement step for measuring the blood pressure of the living body for each first cycle, and a blood flow for measuring the blood flow volume of the living body for each second cycle shorter than the first cycle. Blood pressure estimation for estimating the blood pressure for each third cycle shorter than the first cycle based on the measurement step, the blood pressure measured by the blood pressure measurement step, and the blood flow measured by the blood flow measurement step A process.
- the blood pressure estimation method of the present embodiment may adopt various aspects.
- the blood pressure estimation device of the present embodiment includes blood pressure measurement means, blood flow measurement means, and blood pressure estimation means.

In particular, the laser element 121 preferably irradiates a blood vessel of the earlobe with laser light. However, the laser element 121 may irradiate the result of the other part with laser light.
- the light receiving element 122 receives the beat signal light generated by the mutual interference between the reflected light of the laser light from the living body and the scattered light of the laser light LB from the living body.
- the light receiving element 12 generates a detection current obtained by converting the received beat signal light into an electrical signal.
- the amplifier 123 amplifies the detection current output from the light receiving element 122 after converting it into a voltage signal.
- the A / D converter 124 performs A / D conversion processing (that is, quantization processing) on the output of the amplifier 123 (that is, the voltage signal corresponding to the beat signal light received by the light receiving element 122). As a result, the A / D converter 124 outputs the sample value of the voltage signal (that is, the quantized voltage signal) corresponding to the beat signal light received by the light receiving element 122 to the arithmetic circuit 125.
- a / D conversion processing that is, quantization processing
- the arithmetic circuit 125 performs frequency analysis using FFT (Fast Fourier Transform) on the output of the A / D converter 124 (that is, the sample value of the voltage signal corresponding to the beat signal light received by the light receiving element 122). . - the reference blood pressure BP (s) is, for example, the blood pressure BPm (s) measured at the latest time (reference time s) among the blood pressure BPm (n) measured by the blood pressure measurement unit 11. Also good.
- the blood flow storage unit 132 is a memory that stores the blood flow BF (n) measured by the blood flow measurement unit 12.
