Description

카테터 및 이의 제조방법
본 발명은 카테터 및 이의 제조방법에 관한 것으로서, 보다 구체적으로 온도 변화 및 pH 변화에 대한 안정성 및 높은 광물리적 특성 등 제반 물성이 우수하고, 광에너지 조사에 따른 광감응제의 여기에 의해 생성되는 활성산소를 통해 자가멸균을 할 수 있으며, 세균에 의한 바이오 필름 형성을 방지할 수 있는 광기능성 자가멸균 카테터 및 이의 제조방법에 관한 것이다.
일반적으로 카테터(catheter)는 의료용으로 사용되며 직접 신체에 삽입되는 모든 수단을 포괄적으로 지칭하는 물품으로써, 혈관 주사용, 요도 삽입용, 기도 삽입용, 복강경 수술용 등 다양한 용도로 사용된다.
이러한 카테터는 시술 시 직접 신체에 삽입됨에 따라, 카테터에 의한 신체 감염을 방지하는 것이 중요하다.
예를 들어, 통상 혈관 주사용 카테타의 경우에는 튜브 형의 본체 일부가 신체의 혈관으로 삽입됨으로써 발생될 수 있는 신체 감염을 방지하고자 각종 화학약품을 사용하여 상기 본체의 외표면 및 내부를 세척하고 재사용하고 있다.
하지만, 이러한 세척액은 인체에 무해하다고 하여도 세척액을 통해 상기 본체의 내부를 세척하는 것이 매우 어렵다는 문제점이 있으며, 세척에 소요되는 비용이 크다는 문제점이 있다.
또한, 카테터를 1회 사용한 후 폐기하는 방법이 있으나, 본체의 내구성이 우수함에도 불구하고 감염의 우려 때문에 폐기하는 것은 경제적으로 매우 불리하고, 결국 수술 단가를 높이는 결과를 초래하는 문제점이 있다.
또한, 요도 카테터(urinary catheter)는 요도로 삽입된 카테터에 집락화(colonization)된 병원균이 상행성 감염을 유발시킨다.
이러한 요도 카테터의 삽입은 병원성 요로감염(nosocomial UTI)의 원인이 되며, 구체적으로 단기간의 요도 카테터의 삽입은 발열 및 급성 신우신염(acute pyelonephritis)와 연관되고, 장기간 삽입은 요로 결석(urinary stone), 만성 신염(chronic renal inflammation) 등의 합병증을 유발하는 것으로 알려져 있다.
이러한 요로 감염의 원인균으로는, Escherichia coli가 가장 흔한 원인균이고, 포도당 비발효 그람음성 간균 중에는 Pseudomonas aeruginosa가 가장 흔한 원인균이며, 그람양성 구균 중에는 Enterococcus spp.가 흔하며 Staphylococcus aureus도 중요한 원인균이다.
최근 분리되는 요로감염 원인균 중 penicillin, 협범위 및 광범위 cephalosporin, carbapenem, fluoroquinolone, aminoglycoside 등 여러 항균제에 내성을 지닌 경우가 흔하고, E. coli, P. aeruginosa 등 그람음성 간균은 extended-spectrum β-lactamase (ESBL), metallo-β-lactamase 등 다양한 기전으로 광범위 cephlosporin, carbapenem 등에 내성인 경우가 흔하다.
이에 따라 자가멸균 기능을 갖는 카테터에 대한 개발이 필요하며, 세균의 바이오 필름 형성을 방지하는 기술에 대한 개발이 요구되고 있다.
본 발명은 상기와 같은 문제점을 해결하기 위한 것으로, 본 발명의 목적은 온도 변화 및 pH 변화에 대한 안정성 및 높은 광물리적 특성 등 제반 물성이 우수하고, 광에너지 조사에 따른 광감응제의 여기에 의해 생성되는 활성산소를 통해 자가멸균을 할 수 있으며, 세균에 의한 바이오 필름 형성을 방지할 수 있는 광기능성 자가멸균 카테터 및 이의 제조방법을 제공하는 것이다.
본 발명의 상기한 목적을 달성하기 위하여,
본 발명의 일 측면에 따르면,

이하, 본 발명의 일 실시예에 따른 카테터를 첨부된 도면을 참고하여 상세히 설명한다. 첨부된 도면은 본 발명의 예시적인 형태를 도시한 것으로, 이는 본 발명을 보다 상세히 설명하기 위해 제공되는 것일 뿐, 이에 의해 본 발명의 기술적인 범위가 한정되는 것은 아니다.
본 발명은 튜브 형 본체 및 상기 본체의 내부 원주면에 담지된 광감응제를 포함하는 카테터에 관한 것이다.
이때, 상기 본체는 실리콘, 라텍스 및 이들의 혼합체로 형성될 수 있으며, 상기 광감응제는 포르피린(H2TTP), 프탈로시아닌(phthalocyanine), 염료(dye), 포르피린 치환체 및 프탈로시아닌 치환체로 이루어진 그룹으로부터 선택된 하나 이상일 수 있다.
본 발명의 일 실시예에 따른 카테터는 혈관 주사용, 요도 삽입용, 기도 삽입용, 복강경 수술용 등 다양한 용도로 사용될 수 있으나, 이하 설명의 편의를 위하여 본 발명의 일 실시예에 따른 카테터는 요도 카테터인 경우(즉, 요도 카테터의 사용 과정에서 발생되는 오염원인 경우), 본 발명의 일 실시예에 따른 카테터를 구성하는 광감응제는 포르피린인 경우를 예로 들어 설명한다.
도 1은 본 발명의 일 실시예에 따른 카테터의 제조과정을 나타내는 개념도이고, 도 2는 본 발명의 일 실시예에 따른 카테터의 자가멸균 과정을 나타내는 개념도이다.
본 발명의 일 실시예에 따른 요도 카테터는 튜브 형의 본체 및 상기 본체의 내부 원주면에 담지된 포르피린(H2TPP)을 포함한다.
여기서, 상기 본체는, 이에 제한되지 않으나, 예를 들어 실리콘, 실리콘-라텍스 혼합체 또는 라텍스 등으로 형성될 수 있고, 내부면과 외부면을 가지며 그 내부로 약물 등을 주입할 수 있는 유체의 유동공간이 형성된다. 상기 포르피린(H2TPP)은 광감응성 물질로서, 광에너지가 조사되면 산소로의 전자 전이 또는 에너지 전이 과정을 통하여 활성산소를 발생시키게 된다.
이러한 활성산소는 전자 전이에 의한 라디칼 산소와 에너지 전이에 의한 단일항 산소로 구분될 수 있으며, 각기 다른 활성효율과 에너지 특성을 갖고 있다.
한편, 활성산소의 의한 바이러스, 박테리아 및 암세포 제거는 다양한 논문 등을 통하여 알려져 있으며, 본 발명의 일 실시예에 따른 요도 카테터는 광에너지를 포르피린에 조사하여 라디칼 산소와 단일항 산소의 다른 활성효율에 따라 다양한 박테리아를 멸균할 수 있다.
또한, 포르피린은 유기화합물로서 카터테 본체 내벽에 부착성이 양호하다.
카테터 본체의 내부 원주면에 상기 포르피린를 0.5mm이하의 두께로 도입시킴으로써, 생성된 활성산소가 카테터의 외부 원주면을 통하여 밖으로 확산되는 것을 방지할 수 있다.
도 3은 본 발명의 일 실기 예에 따른 카테터의 공초점현미경을 통한 사진이다.(왼편부터 블라이트 필드(bright field), 형광, 병합(merged) 사진)
본 발명의 일 실시예에 따른 카테터의 공초점현미경 사진을 살펴보면, 광감응제가 카테터 내벽에 균일하게 도입되어 있음을 확인 하였다. 이를 통해 카테터 내벽에 광감응제가 균일하게 도입되어 내벽 전체에서 활성산소 생성을 유도할 수 있다.
도 4는 본 발명의 일 실시예에 따른 카테터의 흡수(좌) / 형광(우) 스펙트럼을 나타내는 그래프이다.
본 발명의 일 실시예에 따른 카테터의 흡수 스펙트럼 및 형광 스펙트럼을 살펴보면, 광감응제(H2TPP)가 도입된 광기능성 카테터를 각각 Hitach사의 U-2800 UV/Vis spectrophotometer와 Jasco사의 V550 Reflectance UV/Vis spectrophotometer를 이용하여 정류상태 흡수 스펙트럼(steady-state absorption spectrum)을 측정한 결과, 포르피린의 고유의 흡수 파장 피크인 B-band(400nm)와 Q-band(510, 530, 580, 630nm)가 포르피린이 도입된 카테터의 흡수스펙트럼에서도 동일하게 나타남을 확인하였다.
따라서, 상기 포르피린의 정류상태 흡수 스펙트럼의 흡수 피크는 350nm 내지 700nm의 파장, 바람직하게는 400nm 내지 650nm에 존재하는 것이 바람직하며, 상기 포르피린의 넓은 정류상태 흡수 파장에 따라, 본 발명의 일 실시예에 따른 카테터는 넓은 가시광 영역의 광에너지를 활용할 수 있으며, 이러한 광에너지 조사에 따른 광감응제의 여기에 의해 생성되는 활성산소를 통해 자가멸균을 할 수 있으며, 세균에 의한 바이오 필름 형성을 방지할 수 있다.
또한, 광감응제(H2TPP)가 도입된 광기능성 카테터를 Hitachi 사의 F-4500 spectrophotometer를 이용하여 정류상태 형광스펙트럼(steady-state Fluorescence spectrum)을 측정(여기파장(excitation wavelength): 510nm)한 결과, 650, 720nm에서 고유의 형광 피크가 나타나는 것을 확인할 수 있었다.
이를 통해 상기 포르피린의 발광(photoluminescence)은 인광(phosphorescence) 과정을 통하여 이루어지며, 이는 상대적으로 긴 수명시간을 가짐으로써 높은 활성산소 생성 효율을 유도할 수 있다.
이에 제한되지 않으나, 예를 들어 상기 포르피린의 형광 스펙트럼의 형광피크는 600nm 내지 750nm의 파장에 존재할 수 있으며, 인광과정을 통해 활성산소의 생성이 이루어짐으로써 긴 수명시간에도 광물리적 특성이 유지될 수 있다.
이를 통해 광감응제(포르피린, H2TPP)가 실리콘 카테터 소재에 도입된 후에도 광감응제의 정류상태의 광물리적 특성이 유지됨을 확인할 수 있었다.
도 5는 본 발명의 일 실시예에 따른 카테터를 구성하는 포르피린의 농도에 따른 유기물의 분해 효율을 나타내는 그래프이다.
도 5에 나타난 바와 같이, 도입되는 포르피린(H2TPP)의 농도가 제어된 다양한 농도의 광기능성 자가멸균 카테터를 이용하여 유기물(Rhodamine B) 분해실험을 수행한 후, 정류상태 형광분광스펙트럼의 세기변화를 통해 도입된 포르피린(H2TPP) 농도와 유기물 분해효율 관계를 알 수 있으며, 본 발명의 일 실시예에 따른 카테터를 구성하는 상기 포르피린의 농도는 6.5*10-8mol/mL 내지 2.4*10-6mol/mL 인 것이 바람직하다.
위와 같은 구조 및 기능을 갖는 카테터를 제조하는 방법을 도면을 참조하여 구체적으로 설명한다.
본 발명은 본체 및 상기 본체 내에 담지된 광감응제를 포함하는 카테터의 제조방법으로서, 용매에 광감응제를 용해시켜 광감응제 용액을 제조하는 단계; 및 상기 광감응제 용액을 카테터 본체의 내벽에 담지시키는 단계를 포함한다.
일 실시태양에 따르면, 본 제조방법은 1mL 디클로메탄(용매)에 0.4mg 포르피린(광감응제)을 용해시켜 포르피린 용액을 제조하는 단계 및 본체의 팽윤현상을 이용하여 포르피린 용액을 주사기 등으로 본체의 내벽에 도입하는 단계를 포함한다.
여기서, 상기 포르피린 용액의 농도는 6.5*10-8mol/mL 내지 2.4*10-6mol/mL 일 수 있으며, 이후, 25℃ 내지 135℃의 온도로 가열한 후, 용매제거 및 디스웰링(deswelling) 공정을 수행하는 단계 및 카테터를 초음파 세척기로 세척하는 단계를 거쳐 카테터를 제조할 수 있다.
이하, 본 발명의 일 실시예에 따른 카테터의 온도 변화 또는 pH변화 등에 의한 안정성을 구체적으로 살펴보면,
도 6는 본 발명의 일 실시예에 따른 카테터의 pH변화에 따른 광촉매 효과를 나타내는 그래프이고, 도 6은 본 발명의 일 실시예에 따른 카테터의 온도 변화에 따른 광촉매 효과를 나타내는 그래프이며, 도 7은 본 발명의 일 실시예에 따른 카테터의 안정성도이고, 도 8은 본 발명의 일 실시예에 따른 카테터를 생체 유사조건에서 실험한 경우의 안정성을 나타내는 그래프이다.
도 6를 참조하면, 본 발명의 일 실시예에 따른 카테터는 다양한 pH조건에서도 활성산소에 의한 광촉매 효과가 있음을 확인할 수 있으며, 도 7을 참조하면, 카테터의 멸균 온도 조건(약 135℃) 처리 후에도 광촉매 효율을 유지함을 확인할 수 있으며, 이와 같이 온도 변화에 따른 안정성을 유지하고 있음을 확인할 수 있었다
도 8은 생체내 유사조건(pH 6.8, 37℃, 27시간)에서 광감응제가 도입된 카테터로부터 광감응제의 용출 여부를 검증한 그래프로서, 본 발명의 일 실시예에 따른 카테터는 생체내 유사조건에서 안정성을 갖는 것을 확인할 수 있었다.
도 9는 광감응제가 도입된 카테터를 이용하여 유기물을 분해하기 전, 후의 광물리적 특성을 실험한 그래프로서, 본 발명의 일 실시예에 따른 카테터는 유기물 분해 전, 후에 걸쳐 동일한 광물리적 특성을 가지는 것으로 확인할 수 있었다.
도 10 내지 도 17은 본 발명의 일 실시예 따른 카테터의 항 박테리아 효과에 대한 시험 결과를 도시한 것이다.
구체적으로 실리콘 도뇨관의 바이오필름 형성의 주오염균인 에세리키아 콜리(Escherichia coli ATCC 25922 - 도 10 및 도 14) 외 3종의 박테리아 [스타필로코코스 아우레우스(Staphylococcus aureus ATCC 25923 - 도 11 및 도 15), 슈도모나스 아에루기노사 (Pseudomonas . Aeruginosa ATCC 27853 - 도 12 및 도 16), 칸디다 알비칸스(Candida albicans ATCC 90228 - 도 13 및 도 17)를 사용하여 제조된 광기능성 자가멸균 카테터의 항박테리아 효과 (antibacterial effect)에 대해 실험하였다.
박테리아는 BHI 브로스 (Brain Heart Infusion broth (Oxoid))에서 37℃ 및 24시간 이상의 호기성 조건에서 배양하였다. 밤새 배양한 박테리아 4종의 현탁액은 PBS에서 희석시켜 2.0 x 105cfu 가 되도록 하였다.
PBS(pH=7.0) 10ml를 완충액으로 사용하였고 빛을 조사하기 전 30분간 암실 조건에서 자유 교반(free stirring)을 실행하였다. 그 후 광원으로는 10ns Nd-YAG laser (510nm, 8mW)를 이용하여 1시간 조사를 시켰다.
빛 조사 후 현탁액에서 10 마이크로리터와 비교형으로 20마이크로리터를 떠내어 맥 콘키 아가(Mac Conkey agar: MAC)에 플레이팅(plating)하였다.
모든 플레이트(plates)는 37℃에서 16시간 이상 배양하였고 그 뒤 생존 균주는 바이어블 카운팅(viable counting)을 실행하였다.
실험결과 E. coli, S. aureus, P. Aeruginosa의 경우 5분 이내에 본 실험조건에서 100% 괴사하였으며, C. Albicans의 경우 60% 정도 괴사하는 것을 확인하였다.
이와 같이 제조된 카테터의 자가멸균 방법을 구체적으로 설명하면,
본 발명의 일 실시예에 따른 요도 카테터를 시술 환자에게 삽입하고, 카테터의 체외 말단(외부로 노출된 부분) 부위에 초소형 레이저(레이저 포인트 등)를 밀착 시킨 후, 주기적인 레이저(광에너지) 조사를 수행하여 활성산소의 발생을 유도한다.
이때 카테터 내로 조사된 레이저는 카테터 내벽을 따라 가이드 되어 카테터 채내 말단부위까지 도달하게 된다.
이처럼 광에너지 공급 시술이 매우 간단하며, 추가적인 장비 또는 장치를 요하지 않는다.
위에서 설명된 본 발명의 바람직한 실시예는 예시의 목적을 위해 개시된 것이고, 본 발명에 대한 통상의 지식을 가지는 당업자라면 본 발명의 사상과 범위 안에서 다양한 수정, 변경, 부가가 가능할 것이며, 이러한 수정, 변경 및 부가는 하기의 특허청구범위에 속하는 것으로 보아야 할 것이다.

Claims (14)

  1. 튜브 형 본체; 및
    상기 본체의 내부 원주면에 담지된 광감응제를 포함하는 카테터.
  2. 제 1 항에 있어서,
    상기 본체는 실리콘, 라텍스 및 이들의 혼합체로 형성된 것을 특징으로 하는 카테터.
  3. 제 1 항에 있어서,
    상기 광감응제는 포르피린(H2TTP), 프탈로시아닌(phthalocyanine), 염료(dye), 포르피린 치환체 및 프탈로시아닌 치환체로 이루어진 그룹으로부터 선택된 하나 이상인 것을 특징으로 하는 카테터.
  4. 제 1 항에 있어서,
    상기 광감응제의 정류상태 흡수 스펙트럼의 흡수 피크는 350nm 내지 700nm의 파장에 존재하는 것을 특징으로 하는 카테터.
  5. 제 1 항에 있어서,
    상기 광감응제의 정류상태 형광 스펙트럼의 형광 피크는 600nm 내지 750nm의 파장에 존재하는 것을 특징으로 하는 카테터.
  6. 제 1 항에 있어서,
    상기 본체 내부 원주면에 담지된 광감응제는 두께가 0.5mm이하인 것을 특징으로 하는 카테터.
  7. 용매에 광감응제를 용해시켜 광감응제 용액을 제조하는 단계; 및
    상기 광감응제 용액을 카테터 본체의 내벽에 담지시키는 단계를 포함하는 광감응제를 포함하는 카테터의 제조방법.
  8. 제 8 항에 있어서,
    상기 카테터의 본체는 실리콘, 라텍스 및 이들의 혼합체로 형성된 것을 특징으로 하는 카테터의 제조방법.
  9. 제 7 항에 있어서,
    상기 광감응제는 포르피린(H2TTP), 프탈로시아닌(phthalocyanine), 염료(dye), 포르피린 치환체 및 프탈로시아닌 치환체로 이루어진 그룹으로부터 선택된 하나 이상인 것을 특징으로 하는 카테터의 제조방법.
  10. 제 7 항에 있어서,
    상기 광감응제 용액의 농도는 6.5*10-8mol/mL 내지 2.4*10-6mol/mL 인 것을 특징으로 하는 카테터의 제조방법.
  11. 제 7 항에 있어서,
    상기 광감응제는 상기 본체의 내부 원주면에 0.5mm이하의 두께로 담지되는 것을 특징으로 하는 카테터의 제조방법.
  12. 제 7 항에 있어서,
    25℃ 내지 135℃의 온도로 가열한 후, 용매제거 및 디스웰링(deswelling) 공정을 수행하는 단계; 및
    본체를 초음파 세척기로 세척하는 단계를 더 포함하는 카테터의 제조방법.
  13. 제 7 항에 있어서,
    광감응제 용액을 카테터 본체의 내벽에 담지시키는 단계는 본체의 팽윤현상을 통하여 광감응제 용액을 본체의 내벽에 도입하는 것을 특징으로 하는 카테터의 제조방법.
  14. 제 7 항에 있어서,
    상기 광감응제는 상기 본체의 내부 원주면에 0.5mm이하의 두께로 담지되는 것을 특징으로 하는 카테터의 제조방법.
