Definitions
- the present application relates to a system for removing solid material in the oral cavity of a patient.
- Laser radiation is used, for example, to remove gums from the jawbone before drilling holes in the jawbone.
- Such holes are, for example, usual in the introduction of implants.
- Such holes are to be introduced, it is known to previously produce a template, which is designed as a kind of dental splint and to indicate the position for the previously planned on the computer using X-ray or CT recordings holes.
- a template which is designed as a kind of dental splint and to indicate the position for the previously planned on the computer using X-ray or CT recordings holes.
- toothed rails usually made of plastic, usually metal-reinforced openings through which the drill can be introduced.
- Object of the present invention is to provide a system with which in a few steps and in a particularly precise and reliable manner erosion of solid material in the oral cavity can be made.
- a system for removing solid material in the oral cavity of a human or animal comprises a fixing structure and a delivery device.
- the introduction device has at least one means for removing solid material.
- jawbone, tooth material, filling material or gum introduced into the teeth can be a solid material to be ablated.
- ablations for the subsequent introduction of implants, fillings or inlays, for the attachment of crowns, for a tooth movement for example, in the context of a Rapid Orthodontic Tooth Movement Therapy be made.
- non-contact ablation As a means for non-contact ablation come here in particular laser beams or lasers that can generate such in question. In principle, however, other methods are conceivable with which such material can be removed. It is conceivable here, for example, the use of ultrasound, strong radio waves or others. As a non-contact method, for example, classical drilling should be mentioned.
- the fixing structure according to the invention is designed so that it can be fixed to a human or animal head, in particular to a jaw, for example, on the jawbone and / or teeth.
- a jaw for example, on the jawbone and / or teeth.
- teeth come here, for example, tooth rails or the like.
- the fixing device has a first connecting means, the insertion device a corresponding second connecting means, wherein the connecting means are arranged so that the second connecting means is rigidly connectable to the fixing device. Furthermore, the fixing device and the insertion device are designed so that the second connecting means has a fixed relative to a jawbone or a tooth of the human or animal after rigidly connected to the fixed to the head fixing structure. This means that the connecting means or the fixing structure and the introduction structure are to be designed so that the fixing structure can be brought into a fixed position relative to a jawbone or a tooth.
- first connecting means is rigidly and rigidly connected to the securing device and the second connecting means is rigidly connected to the introduction device and the connecting means are arranged such that they can be rigidly connected to one another when connected.
- locking means clamps, sliding latch and / or magnetic alignment conceivable.
- a rigid connection does not necessarily have to be firm.
- Sufficient is a connection if, for example, when pressing from bottom to top with respect to the orientation in the direction perpendicular to the printing direction level is fixed. This can be realized for example by grooves and corresponding projections.
- locking and / or fixing means are provided in such a way that the connection is not only rigid but also so strong that it can not be changed without actuation of solvents to be provided, such as the retraction of a bolt.
- the introduction device can apply the at least one removal means at a specific location.
- Such a system can be used in such a way that first the securing device is fixed to the head of the patient, in particular to the jaw of the patient, and then the delivery device is fixed thereto by means of the connecting means. Subsequently, the removal can be carried out.
- the introduction device is set up such that the introduction device or an end section of the insertion device directly adjoining the second connection means is connected to the second connection means such that the introduction device or the end section has a non-variable distance to the structure in the direction of removal in which the removal is carried out.
- the direction of the ablation is the mean direction of incidence of the contactless ablation means.
- the insertion device or the end portion is only rotatable about the direction of the ablation, in particular the longitudinal axis of the hole, that is, about the direction of insertion of the hole, and otherwise rigidly connected to the second connecting means. Then position determining means are advantageously provided for determining the exact rotational position.
- the end portion may be completely rigidly connected to the second connection means. Then no position determining means are provided.
- the end portion has at least one deflection means for deflecting the means for contactless removal.
- the deflection means is advantageously positioned on an axis by the direction of the ablation.
- a delivery device By such a deflection means, a delivery device can be created, which is particularly easy to position in the oral cavity with its limited space, especially perpendicular to the chewing surfaces.
- the fixed layer according to claim 2 is a predetermined position by the fixing device and the connecting means.
- a CT or other pictorial representation of the situation in the oral cavity or a part thereof can be obtained (eg by interoral scanners or the like) and, for example, a planning of the ablation can be carried out on the computer. Then, for example, by "rapid prototyping" a Festlegeregal be prepared. This may be adapted to the shapes encountered in the oral cavity and having a first connecting means at a predetermined position and oriented in a predetermined direction so that the delivery device can be attached thereto via the connecting means to the securing device and by means of the ablation means now An ablation can be performed that corresponds to the previously planned ablation as far as possible.
- a corresponding computer program that represents images taken from the oral cavity and permits planning of the removal. It could furthermore be set up to produce by corresponding means a fixing device which is set up so that, following this, and fixing the fixing device and connecting the introduction device to the fixing device, an automatic removal can be carried out which corresponds to the previously planned largely exactly.
- the fixing device and / or the introduction device have at least one position determining means for determining the fixed position.
- a position determination is advantageously carried out with respect to the fixing device.
- the fixing device then has a predefined position relative to the jaw or teeth, then the position of the introduction device relative to the jaw structure or the teeth can be determined via the position determining means.
- the connecting means provide a definition that allows rotation about an axis. This is advantageous, for example, in order to be able to place a delivery device as freely as possible in the oral cavity.
- a tooth and / or jaw rail is used for the positive clamping on a jaw portion and / or teeth and / or implants as a fixing device. This provides a particularly simple and reliable way of fixing.
- a pulsed laser or a light guide connected to a pulsed laser is used as the means for ablation.
- Pulsed lasers are particularly suitable for gentle ablation.
- the introduction device is set up so that it uses a laser beam for removing solid material in the oral cavity.
- the use of pulsed lasers is particularly advantageous because it can prevent carbonification of a jawbone structure and the heat input can be kept relatively low.
- the connecting means are chosen so that the connection is waterproof. So no germs can penetrate from the oral cavity via the saliva.
- the connecting means are chosen so that no particles or liquids can enter the introduction device.
- the introduction device can be used in a simple way for different patients. To do this, it would then only have to be disinfected from the outside or provided with an exchangeable cover.
- the contactless ablation agent for example a laser
- the contactless ablation agent does not necessarily have to be positioned in the oral cavity. Rather, it can be positioned remotely and be guided via a means of transport, such as a light guide, into the mouth.
- the introduction device in particular when using a pulsed laser as ablation means, a scanner.
- a scanner is generally advantageous for controlling or deflecting and, in particular, for moving the ablation agent or the point of impact and / or impact of the ablation means in a sweeping manner.
- such a scanner includes at least one movement device to which, in particular, a scanner mirror is fastened.
- a scanner mirror is fastened.
- targeted control of the movement device for example, a laser beam can be deflected controllable.
- the movement device is advantageously rigidly connected to the second connecting means.
- the scanner may be arranged without such a rigid connection.
- at least one sensor for detecting the change and / or position and / or orientation may be provided to preserve the information.
- means may be provided which ensure an identical place of impact or place of action even with changes in position and / or orientation. This is possible for example by special, coupled with the moving and / or stationary parts optics. This can also be ensured by using optical fibers at the moving parts and / or joints and / or adjusting locations, for example if the direction of the coupling into the optical waveguide is changed by the scanner.
- the laser beam can be deflected in different planes.
- a heat input can be reduced and a particularly planning-oriented removal can be performed.
- the laser beam can then be guided in various patterns over the area at which a hole is to be formed.
- the points at which the laser is not irradiated, in the meantime cool down again. It can also be generated by a particularly vertical hole shape.
- the hole floor plan in contrast to conventional methods with drills in almost any geometry, such as hexagonal, running.
- a corresponding method would thus include the method steps of switching on the pulsed laser and directing the laser beam via the control of the movement devices in such a way that ablation is carried out at the desired positions so that the planned ablation is realized at the end.
- the introduction device can have an end section connected to the second connection piece, in particular stiff.
- deflecting mirror In such an end portion of a fixedly connected thereto deflecting mirror is arranged.
- the end section in turn is rigidly connected to the scanner.
- Stiffly connected to the scanner is a coupling-in section for coupling laser pulses, which in particular is movably connected to the scanner.
- a laser pulse can be coupled into the scanner, deflected accordingly by the scanner mirrors and then guided via the fixed deflection mirror to the predetermined location.
- Such a system is particularly preferably designed or used to introduce at least one hole, in particular an angular cross-section, into a jaw.
- the ablation means for example the laser energy or pulse shape, the type of scanner or the connection means.
- Angular holes are particularly advantageous, since a particularly good hold, eg. Of implants can be achieved.
- a system is designed with particular advantage to introduce a mounting structure for an implant in a jawbone. An appropriate use highlights the advantages of this system particularly clearly.
- such a system can be advantageously used to insert a hole or a recess in a tooth.
- a design can be characterized by a corresponding computer system for planning and implementation, the ablation means, for example the laser energy or pulse shape, the type of scanner or the connection means.
- the ablation means for example the laser energy or pulse shape, the type of scanner or the connection means.
- such a system can also advantageously be designed for the removal of jawbones in the immediate vicinity of a tooth for influencing the movement of a tooth in the jaw in a tooth misalignment correction or be used for this purpose.
- This can be done, for example, in the context of a Rapid Orthodontic Tooth Movement Therapy.
- the ablation means for example the laser energy or pulse shape, the type of scanner or the connection means.
- the scanner and / or the connection means must be arranged so that the ablation means directly at the adjacent to the tooth jaw area, so often in the interdental space can conduct.
- the systems are connected to the corresponding control software and hardware, which ideally also make it possible to plan the intervention or have corresponding interfaces.
- the insertion device includes a distance measuring device.
- a distance measuring device for example, the distance to the material to be removed can be determined.
- the success of the ablation or the depth or shape of the ablation can be determined. This allows an even more accurate execution of the removal, since a corresponding feedback is possible.
- Such a distance measurement can be formed, for example, by triangulation means for carrying out a distance measurement by means of laser triangulation.
- a steerable laser beam which is also used for ablation, but may be used with possibly lower intensity and the performed ablation or the shape of the material located at the Abtragungsortes be determined.
- such a distance measuring device may comprise means for detecting an ablation bang.
- an ablation bang This is, for example, a microphone. If, for example, material is removed by means of a laser beam, a plasma and an ablation bang are generated. This ablation bang can be detected. This is possible in different ways.
- Such a distance measurement has the advantage that they are performed simultaneously with the ablation. This also makes it possible to measure the hole geometry or the shape of the removal or of the material that is located at the point of removal. It offers, for example, the advantage over laser triangulation that it delivers comparatively accurate results even in the case of a comparatively large amount of steam or particles in the ablation region.
- Such a measurement can be carried out by determining the duration of the sound waves of the ablation bang, which can then be converted on the basis of the speed of sound in between ablation site and microphone in a distance between place of origin (Abtragungsort) and receiving microphone.
- the generation of the ablation means for example the laser pulse, or the passage of the ablation means, for example the laser pulse
- the introduction device can be used by the introduction device as the starting point of the time measurement since, for example, the transit time of a laser beam can generally be neglected.
- a change in depth caused by an ablation carried out between two measurements can be measured.
- a distance determination can be made without knowledge of the generation time.
- a distance can be determined in a known and simple manner by trilateration.
- a control of the control of the laser beam can also be carried out by calculating from the transit times to the at least two or three microphones. Because by a simple calculation based on the maturity of the place of origin can be calculated.
- the detection of the ablation bang can be used to deduce further properties of the ablating material or of the material in the vicinity of the ablation.
- the remaining amount of material can be roughly determined or strength of the removed material can be determined. As a result, a particularly good control of the removal can be realized.
- the detection of the ablation bang can determine the required laser or pulse energy when using a laser as a means of ablation. This can vary depending on the jawbone or substance to be ablated. Thus, for example, the laser energy can be slowly increased until an ablation bang is detected. As a result, the required laser energy can be set very precisely and adapted to the individual points of erosion, thereby achieving a particularly gentle removal or the heat input can be minimized.
- the delivery device when comprising, as a means for ablation, a pulsed laser or optical fiber connected to a pulsed laser according to claim 4 and means for detecting the ablation noise, also comprises means for detecting the time delay between generation or passage of a laser pulse through the delivery device and the detection of the ablation bang.
- the duration of the laser beam can be neglected in the rule.
- a focusing device in particular in, provided on or adjacent to the introduction device.
- the laser beam in particular at the location of the ablation to be carried out, can be focused.
- the focusing device is set up so that the focus is displaceable and can be tracked in particular in the course of removal. This is particularly advantageous for deep ablation, so that the full intensity of the ablation agent is always focused on the ablation site available.
- the tracking may be based on a planned, calculated, and / or measured already carried out ablation and / or on the depth of the planned, calculated and / or measured Abtragungsortes.
- a coolant supply in particular with a nozzle for the targeted introduction of the coolant may be provided.
- a coolant for example, cooled water or iced water in question.
- coolant is selected that allows a laser beam to pass as uninfluenced as possible.
- a suction channel can be provided.
- existing suction means can be connected to the dentist.
- a corresponding method would thus comprise the steps: a. Providing or producing a fastening device, in particular due to image material of the oral cavity, b. Determining the fixing device on the head, in particular on the jaw or teeth, c. Connecting the introduction device with the fixing device via the connecting means, d. Apply the ablation agent to ablate the material.
- a planning of the removal is carried out beforehand.
- the removal is achieved by a scanning process, in which the removal is always carried out at small points and so gradually the planned or desired removal is achieved.
- distance measurements are made to optimize the removal, for example, to achieve a more precise control of the ablation with respect to the energy or the focus and / or for example.
- further data are collected based on the evaluation of the ablation, which are also used to optimize the ablation.

- a hole 4 is shown, on the ground of which a load beam 8 is directed. It is introduced using a handpiece 5.
- the handpiece 5 can be held by a doctor or user. Keeping only means that its weight is held by the doctor.
- the exact orientation of the handpiece 5, or at least that of an end portion, is determined by a toothed rail 3 and the connecting means.
- the hole 4 is generated by ablation of jawbone.
- a toothed rail 3 is placed on these two teeth 2 and serves for fastening or alignment of the handpiece 5.
- a hole 4 can be selectively generated.
- FIG. 2 shows a schematic representation of a system according to the invention, wherein the toothed rail 3 is placed on a not belonging to the system jaw 1.
- a first connecting means 6, which is firmly and rigidly connected to the toothed rail 3.

- the scanner 11 is movably connected to a coupling-in section 16 which opens into a laser 9.
- the laser 9 is arranged in a large mobile housing and couples its laser pulses into the multiply-movable coupling-in section 16 which guides the laser beam. If the laser beam exits the coupling-in section 16 and enters the scanner 11, it can be deflected there. The laser beam then enters an end section 14 in which it is deflected in the direction of the jaw 1. There he can remove jaw material.
- FIG. 3 shows a cross section through a system according to the invention. Further shown is a jaw 1. Put on the jaw 1 is a toothed rail 3, which is fastened there via a positive fit.
- the toothed rail 3 has a first connecting means 6, which cooperates with a second connecting means 7.
- Fixedly connected to the second connecting means 7 are an end section 14 and a scanner 11.
- the scanner 11 has an optical waveguide 10 into which a pulsed laser pulse generated in a laser is coupled as a laser beam 8 into the scanner 11.
- the scanner 11 are two scanner mirrors 12, each mounted on a moving device 13. This moving device 13 is computer controlled and makes it possible to deflect the laser beam 8 in different directions and planes. This is indicated by the multiple laser beams 8 shown in different gray levels.
- the thus deflected laser beams 8 then meet in the end portion 14 on the fixed deflecting mirror 15 and are thereby deflected in the direction of pine 2. So they wear jaw material and create the hole 4.

- a laser beam 8 is advantageously used, which does not lead to an ablation.
- the scanner mirror 12 the laser beam 8 can be moved in the hole 4. As a result, the depth or the change in the depth of the hole 4 at different locations can be determined.
