Abstract
Gegenstand der Anmeldung ist ein Wirbel säulenimplantat zur Anbringung zwischen den Lamina und/oder den Dornfortsätzen benachbarter Wirbelkörper (12, 12' ) - Ein Wirbelsäulenimplantat der eingangs genannten Art zu schaffen, wird dadurch erreicht, dass ein Unterteil (11) zur Anbringung an einem ersten Wirbelkörper (12' ) und durch ein Oberteil (10) zur Anbringung an einem zweiten Wirbelkörper (12), wobei sowohl am Unterteil (11), als auch am Oberteil (10) je eine Kontaktfläche (13, 14) ausgebildet ist, mit der das Unterteil (11 ) und das Oberteil (10) aneinander anliegen.
Description
WIRBELSÄULENIMPLANTAT
Die vorliegende Erfindung betrifft ein Wirbelsäulenimplantat zur vertrebalen Anbringung zwischen den Dornfortsätzen benachbarter Wirbelkörper. Zu den Wirbelkörpern der Wirbelsäule zählen neben den Hals-, Brust- und Lendenwirbel auch das Sakrum.
Aus dem CA 2,320,821 und dem EP 1 330 987 B1 sind interspinose Wirbelsäulenimplantate bekannt, die vertrebal zwischen benachbarte Dornfortsätze der benachbarten Wirbelkörper eingesetzt werden. Diese Wirbelsäulenimplantate umfassen ein Federelement, welches zwischen den Dornfortsätzen angeordnet ist und vier Haltstege, die seitlich an den Dornfortsätzen anliegen, um das Wirbelsäulenimplantat in der jeweiligen Position zu halten. Eine Befestigung dieses Wirbelsäulenimplantates am Wirbelkörper ist nicht erforderlich, da das Federelement aufgrund seiner Spannkraft das gesamte Wirbelsäulenimplantat in Position hält. Die vier Haltestege dienen lediglich zur Sicherung des Wirbelsäulenimplantates gegen seitliches Verrutschen.
Bei einer Kyphosierung der Wirbelsäule kommt es immer wieder vor, dass Teile des Wirbelknochens aufgrund der großen vorherrschenden Drücke im Laufe der Zeit beschädigt werden. Auch kann es passieren, dass hierbei Nerven zwischen den Wirbelknochen eingeklemmt werden und große Schmerzen verursachen.
Zur Vermeidung einer solchen Kyphosierung ist es erforderlich, benachbarte Wirbelkörper zumindest vertrebal voneinander zu distanzieren. Das aus der CA 2,320,821 und der EP 1 330 987 B1 bekannte Wirbelsäulenimplan-
tat ist hierzu nur bedingt geeignet, da das jeweilige Federelement bei großen, auftretenden Kräften sehr weit zusammengedrückt wird und somit die gewünschte Distraktion verloren geht. Auch wirken die Federelemente derart auf die Dornfortsätze der benachbarten Wirbelknochen ein, dass deren Bewegungsfreiheit behindert wird.
Davon ausgehend, liegt der vorliegenden Erfindung die Aufgabe zu Grunde, ein Wirbelsäulenimplantat der eingangs genannten Art zu schaffen, mit dem eine Kyphosierung der Wirbelsäule zuverlässig vermieden wird und welches eine große Bewegungsfreiheit der Wirbelknochen ermöglicht.
Als technische Lösung dieser Aufgabe wird erfindungsgemäß ein Wirbelsäulenimplantat gemäß den Merkmalen des Anspruches 1 vorgeschlagen. Vorteilhafte Weiterbildungen dieses Wirbelsäulenimplantates sind den Unteransprüchen zu entnehmen.
Ein nach dieser technischen Lehre ausgebildetes Wirbelsäulenimplantat hat den Vorteil, dass das an einem ersten Wirbelkörper angebrachte Unter- teil und das an einem zweiten Wirbelkörper angebrachte Oberteil einen definierten Abstand zwischen den Wirbelkörpern herstellt, so dass eine definierte Beabstandung der benachbarten Wirbelkörper erreicht wird. Dies ist besonders vorteilhaft bei beschädigten Knochenabschnitten, insbesondere bei den Gelenkfortsätzen, da diese hierdurch vollständig entlastet werden können. Es versteht sich, dass dem behandelnden Arzt eine Vielzahl von Unter- und Oberteilen jeweils unterschiedlicher Dicke zur Verfügung stehen, so dass der Arzt je nach Befund zueinander passende Unter- und Oberteile aufwählt, um die gewünschte Beabstandung der Wirbelknochen zu erreichen.
In vielen Fällen ist es vorteilhaft, das Wirbelsäulenimplantat statt interspinös am Dornfortsatz wie im Stand der Technik, an einer Lamina des
Wirbelbogens anzubringen, insbesondere wenn die Domfortsätze geschädigt sind, im Falle einer Kyphosierung oder dergleichen. Eine solche interlaminäre Anbringung des Wirbelsäulenimplantates an der Lamina hat auch den Vorteil, dass hier keine so großen Hebelkräfte auftreten und somit der Wirbelknochen entlastet wird. Es versteht sich, dass auch bei einer Anordnung des Wirbelsäulenimplantats im Bereich der Lamina eine dauerhafte und zuverlässige Beabstandung der Wirbelknochen erfolgt.
Ein weiterer Vorteil besteht darin, dass mit dem erfindungsgemäßen Wirbelsäulenimplantat bei einer Platzierung in der Lamina ein Hypomochlion möglich ist. Dabei werden benachbarte Wirbelkörper durch das Wirbelkörperimplantat distrahiert und gleichzeitig über eine vertikal am benachbarten Wirbelkörper angebrachte Klammer komprimiert, so dass eine Relordosierung erfolgt.
In einer besonders bevorzugten Ausführungsform sind die Kontaktflächen des Wirbelsäulenimplantates im Wesentlichen waagerecht angeordnet. Dies hat den Vorteil, dass die auf das Implantat einwirkenden Kräfte quasi senkrecht zur Kontaktfläche eingeleitet werden und somit gut aufgenommen werden können. Ein anderer Vorteil besteht darin, dass hierdurch eine laterale Beweglichkeit der einzelnen Wirbelkörper erhalten bleibt, da durch die waagerechte Anordnung der Kontaktfläche dieser Bewegungsabsicht keinerlei Hindernisse entgegenstehen.
In einer bevorzugten Weiterbildung sind beide Kontaktflächen plan ausgebildet, so dass einer seitlichen Bewegung der Wirbelknochen nichts entgegensteht.
In einer anderen, bevorzugten Ausführungsform ist eine Kontaktfläche plan und die andere konvex ausgebildet. Hierdurch wird die seitliche Beweglichkeit der Wirbelknochen erhalten und gleichzeitig erlaubt das Wirbelsäulen-
implantat ein Verdrehen der Wirbelknochen zueinander, so dass die benachbarten Wirbelknochen in sämtliche Richtungen bewegt werden können.
In noch einer anderen, bevorzugten Ausführungsform ist eine Kontaktfläche konvex und die andere Kontaktfläche konkav ausgebildet. Hierdurch liegen die beiden Kontaktfläche wieder großflächig aneinander an, wobei auch hier eine große Beweglichkeit der Wirbelknochen und somit der Wirbelsäule erhalten bleibt. Ein weiterer Vorteil besteht darin, dass hierdurch auch saggitale Verschiebungen vermieden werden.
Alternativ hierzu kann eine Kontaktfläche anstatt konkav auch rinnenartig ausgebildet sein.
In einer besonders bevorzugten Ausführungsform sind am Unter- und/oder Oberteil Stege angeformt, mit denen das Wirbelsäulenimplantat am Wirbelknochen befestigt werden kann. Dabei sind diese Stege derart ausgebildet, dass sie seitlich am Dornfortsatz oder seitlich an der Lamina zur Anlage kommen. In einer bevorzugten Weiterbildung weist jedes Unter- und Ober- teil zwei solche Stege auf, wobei das Wirbelsäulenimplantat in diesem Fall im Querschnitt im Wesentlichen U-förmig aussieht. Dabei hat es sich als vorteilhaft erwiesen, einen Steg kürzer als den anderen auszubilden, um ein Einsetzen des Wirbelsäulenimplantates zwischen die Wirbelkörper zu erleichtern.
Weitere Vorteile des erfindungsgemäßen Wirbelsäulenimplantates ergeben sich aus der beigefügten Zeichnung und den nachstehend beschriebenen Ausführungsformen. Ebenso können die vorstehend genannten und die noch weiter ausgeführten Merkmale erfindungsgemäß jeweils einzeln oder in beliebigen Kombinationen miteinander verwendet werden. Die erwähnten Ausführungsformen sind nicht als abschließende Aufzählung zu verstehen, sondern haben vielmehr beispielhaften Charakter. Es zeigen:

In den Figuren 1 bis 4 ist eine erste Ausführungsform eines erfindungsgemäßen interlaminären Wirbelsäulenimplantates dargestellt, welches ein
Oberteil 10 und ein Unterteil 11 umfasst. Diese sind vertrebal zwischen einem ersten Wirbelkörper 12 und einen dazu benachbarten zweiten Wirbelkörper 12' im Bereich der jeweiligen Wirbelbogen angebracht.
Das Oberteil 10 besitzt eine konvex ausgebildete Kontaktfläche 13 und das Unterteil 11 besitzt korrespondierend hierzu eine konkav ausgebildete Kontaktfläche 14. Das Oberteil 10 und das Unterteil 11 sind derart an dem jeweiligen Wirbelkörper 12, 12' angebracht, dass im endgültigen Zustand die Kontaktfläche 13 des Oberteils 10 großflächig in die Kontaktfläche 14 des Unterteils 11 eingreift. Dabei hat es sich als vorteilhaft erwiesen, die konvexe Kontaktfläche 13 und die konkave Kontaktfläche 14 im Wesentlichen waagerecht anzuordnen, um die hier auftretenden Kräfte am besten aufnehmen zu können.
Abstehend von der Kontaktfläche 13 sind am Oberteil 10 ein kurzer Steg 15 und ein langer Steg 16 angeordnet, welche an der Lamina des Wirbel- bogens des Wirbelkörpers 12 zur Anlage kommen, um das Oberteil 10 zu fixieren. Analoges gilt für das Unterteil 11. Auch hier sind abstehend je ein kurzer Steg 17 und ein langer Steg 18 vorgesehen, die ebenfalls auf der Lamina des Wirbelbogens des Wirbelkörpers 12' zur Anlage kommen, um das Unterteil 11 dort zu fixieren. In den langen Stegen 16, 18 ist eine Aussparung 19 vorgesehen, durch die ein Nagel oder eine Schraube zur Fixierung des Oberteils 10 und des Unterteils 11 am Wirbelkörper 12, 12' hindurchgeführt werden kann.
Durch die relative Nähe der Lamina zum Wirbelkörpermittelpunkt treten hier geringere Hebelkräfte auf, so dass die Belastung des Wirbelknochens geringer ist. Außerdem ist der Wirbelkörper 12' im Bereich des Wirbelbogens sehr viel stabiler, so dass eine Verletzung des Wirbelkörpers 12' vermieden wird. Ein weiterer Vorteil besteht darin, dass hier eine sehr viel größere Fläche des Knochens zur Verfügung steht, so dass sich die auftretenden
Kräfte auf eine größere Fläche verteilen können, was wiederum zu einer Reduzierung der Belastung führt.
Durch die konvexe / konkave Ausbildung der Kontaktflächen 13, 14 wird eine größtmögliche Beweglichkeit der Wirbelkörper 12, 12' zueinander erhalten, so dass die natürliche Beweglichkeit des Bewegungsapparates durch das Wirbelsäulenimplantat nicht beeinflusst wird. Aufgrund der Tatsache, dass die beiden Kontaktflächen 13, 14 aufeinander reiben, wird eine Resorption des Knochens über herkömmlichen starren Distanzhaltern (interspinosen Implantaten) vermieden. Noch ein Vorteil besteht darin, dass durch die gegeneinander verschieblichen Kontaktflächen 13, 14 eine sagitale Verschiebung vermieden wird. Dabei darf nicht vergessen werden, dass durch das Oberteil 10 und das Unterteil 11 die Wirbelkörper 12, 12' in diesem Teil dauerhaft voneinander beabstandet werden, wodurch eine Kyphose vermieden wird, und wodurch etwaige eingeklemmte Nerven dauerhaft freigesetzt werden.
In den Figuren 5 und 6 ist eine zweite Ausführungsform eines erfindungsgemäßen Wirbelsäulenimplantates dargestellt, welches ebenfalls ein Oberteil 20 und ein Unterteil 21 umfasst. Im Gegensatz zu dem ersten interlaminären Wirbelsäulenimplantat wird diese zweite Ausführungsform interspinös eingesetzt. Auch hier sind die Kontaktflächen 23 und 24 konkav bzw. konvex ausgebildet. Im Unterschied zu der ersten Ausführungsform gemäß den Fig. 1 bis 4 sind die Stege 25, 26, 27, 28 des Oberteils 20 und des Unterteils 21 sehr viel länger ausgebildet und weisen je zwei
Aussparungen 29 zur Aufnahme eines Nagels oder einer Schraube auf. Hierdurch ist das Wirbelsäulenimplantat zur Anbringung an den Dornfortsätzen benachbarter Wirbelkörper 22, 22' geeignet, während das Wirbelsäulenimplantat gemäß den Fig. 1 bis 3 zur Anbringung an den Lamina der jeweiligen Wirbelkörper 12, 12' ausgelegt ist.
Die in Figuren 7 und 8 dargestellte dritte Ausführungsform unterscheidet sich von der in den Figuren 5 und 6 dargestellten zweiten Ausfϋhrungsform lediglich dadurch, dass hier die beiden Kontaktflächen 33 und 34 des Oberteils 30 und des Unterteils 31 jeweils plan ausgeführt sind.
In den Figuren 9 bis 11 ist eine vierte Ausführungsform eines erfindungsgemäßen Wirbelsäulenimplantates dargestellt, welches zwischen dem fünften Lendenwirbel und dem Sakrum (Kreuzbein) angeordnet ist. Dabei ruht dieses Wirbelsäulenimplantat auf der Lamina der beiden Wirbelkörper. Auch dieses Wirbelsäulenimplantat gemäß der vierten Ausführungsform umfasst ein Oberteil 40, welches am Lendenwirbel L 5 befestigt ist und ein Unterteil 41 , welches am Sakrum angebracht ist. Auch hier weist das Oberteil 40 eine konvexe Kontaktfläche 43 auf, während das Unterteil 41 eine kontave Kontaktfläche 44 besitzt. Das Oberteil 40 besitzt zwei abstehende Stege 45, 46 und das Unterteil 41 besitzt zwei abstehende Stege 47, 48, wobei der Steg 48 etwas länger ausgebildet ist, als der Steg 47. Mittels dieser Stege 45, 46, 47, 48 können sowohl das Oberteil 40, als auch das Unterteil 41 am Wirbelkörper L 5 oder am Sakrum mittels Nägel oder Schrauben befestigt werden, welche durch eine entsprechende Aussparung 49 hindurchgeführt werden können.
In einer anderen, hier nicht dargestellten Ausführungsform ist eine Kontaktfläche anstatt konkav, rinnenartig ausgebildet, wobei die konvexe andere Kontaktfläche in der rinnenartigen Kontaktfläche zur Anlage kommt.

Claims
1. Wirbelsäulenimplantat zur vertrebalen Anbringung zwischen den Lamina und/oder den Dornfortsätzen benachbarter Wirbelkörper (12, 12', 22, 22', 32, 32' L5, Sakrum), gekennzeichnet durch ein Unterteil (11 , 21 , 31 , 41) zur Anbringung an einem ersten Wirbelkörper (12', 22', 32', L5, Sakrum) und durch ein Oberteil (10, 20, 30, 40) zur Anbringung an einem zweiten Wirbelkörper (12, 22, 32, L5, Sakrum), wobei sowohl am Unterteil (11,21,31,41 ), als auch am Oberteil (10, 20, 30, 40) je eine Kontaktfläche (13, 14, 23, 24, 33, 34, 43, 44) ausgebildet ist, mit der das Unterteil (11,21,31,41) und das Oberteil (10, 20, 30, 40) aneinander anliegen.
2. Wirbelsäulenimplantat nach Anspruch 1 , dadurch gekennzeichnet, dass die Kontaktflächen (13, 14, 23, 24, 33, 34, 43, 44) im Wesentlichen waagerecht angeordnet sind.
3. Wirbelsäulenimplantat nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass mindestens eine Kontaktfläche (33, 34) plan ausgebildet ist.
4. Wirbelsäulenimplantat nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass mindestens eine Kontaktfläche (13, 23, 43) konvex ausgebildet ist.
5. Wirbelsäulenimplantat nach Anspruch 4, dadurch gekennzeichnet, dass eine Kontaktfläche (13, 23, 43) konvex ausgebildet ist und dass die 5 andere Kontaktfläche (14, 2, 44) korrespondierend hierzu konkav ausgebildet ist.
6. Wirbelsäulenimplantat nach Anspruch 4, dadurch gekennzeichnet, o dass eine Kontaktfläche konvex ausgebildet ist und dass die andere Kontaktfläche korrespondierend hierzu rinnenartig ausgebildet ist.
7. Wirbelsäulenimplantat nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, 5 dass am Unterteil (11,21,31,41) und/oder am Oberteil (10, 20, 30, 40) jeweils mindestens ein Steg (15, 16, 17, 18, 25, 26, 27, 28, 35, 36,, 37, 38, 45, 46, 47, 48) ausgebildet ist, welcher seitlich am Dornfortsatz oder an der Lamina zur Anlage kommt.
o 8. Wirbelsäulenimplantat nach Anspruch 7, dadurch gekennzeichnet, dass im Steg (15, 16, 17, 18, 25, 26, 27, 28, 35, 36, 37, 38, 45, 46, 47, 48) eine Aussparung (19, 29, 39, 49) zur Aufnahme einer Schraube oder eines Nagels vorgesehen ist. 5

Wirbelsäulenimplantat nach einem Ansprüche 7 oder 8, dadurch gekennzeichnet, dass am Unterteil (21) und/oder am Oberteil (20) je zwei Stege (25, 26, 27, 28) derart gegenüberliegend angeordnet sind, dass die Stege (25, 0 26, 27, 28) in Verbindung mit der jeweiligen Kontaktfläche (13, 14, 23, 24) den Dornfortsatz oder die Lamina teilweise, vorzugsweise U-förmig, umgreifen.
10. Wirbelsäulenimplantat nach Anspruch 9, dadurch gekennzeichnet, dass ein Steg (15, 17, 45, 47) kürzer als der andere Steg (16, 18, 46, 48) ausgebildet ist.
