Abstract

The humeral insert (3), which can be engaged on a humeral rod (2) and can receive a glenosphere (4), comprises means on the posterior surface thereof for fixing to the humeral rod (2), and an articular concave surface (9) on the anterior surface (8) thereof. The posterior surface (6) and the anterior surface (8) are joined by means of an enclosing wall (10). According to the invention, the enclosing wall (10) contains a recess (12).

Description

INSERT humeral PROSTHESIS FOR SHOULDER REVERSE The present invention relates to a humeral insert a reverse shoulder prosthesis. As part of a reconstruction of the humeral joint, it is known to put a total shoulder prosthesis. When the cap is damaged, you can ask a so-called reverse prosthesis. This type of prosthesis includes a humeral subset and a subset shoulder. Under the humeral assembly consists of a humeral stem which is engaged in the medullary canal of the humerus and an insert which is fixed on the head of the humeral stem. This insert has a concave outer surface and is generally made of polyethylene. The scapular sub-assembly consists of a glenoid sphere attached to the shoulder part of the joint to be reconstructed. Fixing the glenoid sphere is via a metal base anchored in the bone scapular. Clinical experience has revealed a major drawback of this type of prosthesis. Practitioners have noted that it appears a notch in the pillar of the scapula, that is to say the part below the glenoid cavity of the scapula. This bony notch is due to a conflict between the humeral implant and abutment of the shoulder blade. Indeed, in an arm adduction movement, especially at the end of this movement, there may be a collision between the humeral insert and the pillar of the scapula, which can then create this bone notch. This phenomenon is even more marked in patients in whom the pillar of the scapula is vertical. This notch is generally stable, but in some cases can progress to more extensive necrosis, due in particular to polyethylene debris resulting from the friction of the glenoid sphere on the insert. Although these cases are not controlling, it is very desirable to allow to form a notch in a bone, since the latter, although stabilized remains a weakening of the bone area. An object of the invention is therefore to provide a humeral insert minimizes the formation of a bone recess. To this end, the present invention relates to a humeral insert engageable on a humeral rod and able to receive a glenoid sphere, comprising fixing means to the humeral rod on its rear face and a concave articular surface on its front wall, the face posterior and the anterior side being connected by an enclosing wall, characterized in that it has a recess in the enclosing wall. Thus, the presence of a recess in the insert enclosing wall allows orienting the recess facing the pillar of the scapula, to avoid a conflict between the insert and the pillar of the scapula. According to an advantageous embodiment, the recess intersects with the concave articular surface, which keeps a smaller footprint of the insert, without affecting the holding of the glenoid sphere in the concave articular surface. According to one advantageous possibility, the enclosing wall is substantially trapezoidal, the recess being formed in its large base. Preferably, the recess has a cylindrical surface, allowing to realize the recess by a milling operation. In addition, the recess surface forms, with the belt surface, an acute angle. In one embodiment, the recess extends over a portion of the belt face adjacent to the front side. To avoid a conflict between a base fixed in the bone receiving the glenoid sphere scapular and humeral insert, a chamfer is formed in each of connecting regions between the recess and the anterior face. A variant may provide that the insert has a conical peg projecting from its rear side by its attachment to a humeral stem. For a good understanding, the invention is described with reference to the drawing attached showing by way of nonlimiting example one embodiment of a humeral insert according thereto. Figures 1 to 3 are respectively a side view and in perspective in two orientations a humeral insert according to the invention cooperating with a glenoid sphere to a total shoulder prosthesis. 4 shows a total shoulder prosthesis incorporating the humeral insert according to the invention. 4 shows the usual components of a shoulder prosthesis, namely a humeral shaft 2 intended to be anchored in the medullary canal of a humerus, a humeral insert 3 which engages in the metaphyseal portion of the humerus rod 2, a glenoid sphere 4 and a base 5 fixed to the shoulder part of the joint to be reconstructed. Referring to Figures 1 to 3, one can see that the humeral insert 3 has a rear face 6 which exceeds a conical pin 7. This conical pin 7 can be fitted to the humeral stem. Note also that the rear face 6 has a circumferential rib. The front face 8 of the insert has a concave articular surface 9, which in the example shown, has a perfect matching with the glenoid sphere 4. The belt surface 10 connecting the anterior and posterior faces has a trapezoidal shape, which prevents rotation of the humeral insert 3 when mounted on the humeral stem 2. the insert may, in the usual manner, be made of polyethylene. As shown in figures, a recess 12 is formed in the belt face of the insert. The recess 12 is formed in the large base of the belt and cuts the concave articular surface 9. The recess 12 has a cylindrical surface which is inclined relative to the belt surface of an inclination angle of up until to about 50 °. It is also noted that the recess 12 is not performed over the entire height of the belt surface 10, but is widened as the part of the enclosing wall 10 on the side of the front face 8. It is also be noted that, at each of the connection areas of the recess and the front surface, two chamfers 14 are formed. Thus, when a total shoulder prosthesis comprising an insert

3 is placed, it can be seen with reference to Figure 3 that the end of the supply movement, the humeral insert 3 form an angle with the glenoid sphere which result in conflict between the pillar of the scapula and insert humeral, if it was not formed therein, I recess 12. the humeral insert 3 is of course oriented in such a way that the recess 12 is located in the inlet plane. It is noted that chamfers 14 are also involved in the proper functioning of the prosthesis by avoiding a conflict between the humeral insert 3 and the base 5 metal (also called metal back glenoid) during an internal rotation of the arms. The invention thus provides a humeral insert that avoids the formation of a bone recess in the pillar of the scapula. Of course, the invention is not limited to the embodiment described above by way of example, but on the contrary encompasses all embodiments. Thus, the humeral insert could be circular and not generally trapezoidal shape.

Claims

CLAIMS 1. Humeral insert (3) being engaged on a humeral stem
(2) and able to receive a glenoid sphere (4), comprising fixing means to the humeral rod (2) on its rear face (6), and a concave articular surface (9) on its front face (8), the face rear (6) and the anterior face (8) being connected by a belt wall (10), characterized in that it has a recess (12) in its belt wall (10). 2. Insert humeral according to claim 1, characterized in that the recess (12) intersects the concave articular surface (9). 3. Insert humeral according to claim 1 or claim 2, characterized in that the enclosing wall (10) is substantially trapezoidal, the recess (12) being formed in its large base. 4. Insert humeral according to one of claims 1 to 3, characterized in that the recess (12) has a cylindrical surface. 5. Insert humeral according to claim 4, characterized in that the surface of the recess (12) forms with the enclosing wall (10), an acute angle. 6. Insert humeral according to claim 4 or claim 5, characterized in that the recess (12) extends over a portion of the belt surface (10) adjacent the front face (8). 7. Insert humeral according to one of claims 2 to 6, characterized in that a chamfer (14) is formed in each of connecting regions between the recess (12) and the anterior face (8). 8. Insert humeral according to one of claims 1 to 7, characterized in that it has a conical piece (7) projecting from its rear face (6).
