- the solution suggested according to the present invention is based on the fact that the wound to be subjected to treatment is generally comprised of a cicatricial tissue that replaced, during the reparatory phase following the wound, the pre-existing epithelial tissue, so that the operations suggested according to the invention are, in succession, the removal of the cicatricial tissue and the stimulation of creation of tissue comprised of epithelial cells, thus having the same features of those before the wound.
- a very slightly bleeding surface wound similar to an abrasion is obtained, involving only the corneous layer.
- this new wound is immediately and in the following days subjected to treatment by substances helping in the formation of epithelial tissue, activated by the use of ultrasounds and radiation at the same time of laser and radiofrequency.
- the use of ultrasounds aims to reduce the inflammation and at the same time help the penetration and the intra-tissue spreading of the substances used, the laser radiation stimulates the reconstruction processes and the radiofrequency induction is used for the known capability of stimulating the production of elastin and collagen, that are the basic bricks of the cutaneous tissue structure.
- treatments according to the invention can also be carried out according to a sucked phase, or to a pulsed pressure, alternating sucking moments and intervals at room pressure.
- the apparatus suggested according to the present invention allows to widen the medical - surgical instrument range relevant to the plastic surgery and aesthetic medicine fields.
- the apparatus suggested is suitable for the ambulatory treatment of cicatrix, stretch marks, cheloids or other surgical or traumatic, pathological wounds and is based on the use of a infrared laser source, a negative ion flow generator, an ultrasound transducer and a radiofrequency emitter.
- Treatments by the apparatus according to the invention are carried out in two phases, a preparatory phase, aiming to ablate the surface cicatricial tissue and a following phase that, by the combined action of the laser, of the ultrasounds and of the radiofrequency stimulate the reconstruction of the wound by new tissue of the epithelial kind, equal to that already existing before the original wound.

It is therefore specific object of the present invention an apparatus for treating cutaneous injuries comprising a first structure for exfoliation and sucking of cells and a second structure for reconstruction of epithelial tissue, said first structure providing a laser source, a box having a body with a central longitudinal hole for the passage of the laser beam, and electric coupler, provided within said longitudinal central hole, a pneumatic insulator, a cannula, an ionising device electrode exiting from the cannula, within said cannula, and sucking means for the ablated cells; said second structure providing a laser source, a box having a body with a longitudinal central hole for the passage of the laser beam, a pneumatic insulator, a disposable cannula, means for sucking the ablated cells, an antenna, a piezoelectric element and an ultrasound coupler; the apparatus further providing a sucking and filtering block for the sucked particles, a low voltage and low intensity current feeder,
- said sucking block provides a sucking pump able to produce vacuum with values included in the range of 0 - 760 mmHg.
- said high tension generator produces a current adjustable up to 1 0 kV, with a current intensity not higher than 0.05 mA.
- said radiofrequency generator produces radiofrequency at 3 kHz, with the power that can be adjusted.
- said second structure is comprised by a handle, with a mechanical coupling for the coupling with interchangeable boxes.
- the laser source of the two structures can be unified, the remaining components of the structures being coupable to the other parts of the apparatus.

- the apparatus substantially provides two blocks, namely a first block providing inside a sucking pump that can produce an atmospheric vacuum between 0 and 760 mmHg, a Laser source feeder, a high tension generator, adjustable up to 1 ,0 kV, with a current intensity not higher than 0.05 mA and a radiofrequency generator at 3 kHz, with adjustable power. Said frequency is not critical, but it has been chosen to obtain the emission of ultrasounds having a surface action on the tissues subjected to treatment by a piezoelectric ceramic.
- the second block is comprised of a handeable movable laser source, with mechanical coupling for the connection with interchangeable boxes.
- first figure 1 it is shown the structure of the box comprising the handle to carry out the first phase of the treatment, in order to obtain an exfoliation of the surface of the wound.
- body 1 of the box is holed along its axis, to allow the fixing of the glass disposable or sterilizable cannula 2, comprising the interface between the system and the part of the body to be subjected to treatment.
- Box 1 further provides a pneumatic insulator 9, that is comprised of a plane lens transparent to the laser radiation.
- the function of the insulator 9 is that of realising a pneumatic insulation, in such a way that the laser source 7 can work at atmospheric pressure, thus not being subjected to pressure rushes that could damage the same.
- FIG. 2 it is schematically shown the structure of the apparatus according to the invention for carrying out the second phase or reconstruction phase.
- the body 1 comprised of a suitable plastic material, there are provided, along the longitudinal hole 3, glass or plastic cannula 2, providing inside a spiral antenna 9, for the induction of the radiofrequency signal to the part subjected to the treatment.
- the fact that the end portion of the antenna 9 is a spiral antenna is not a basic feature to obtain the working frequency, but it only has the function of widening the diffusion ray of the signal induced by capacitive effect.
- Said antenna 9 is removably coupled with a reversed metallic cone 10, coupled with the radiofrequency generator (not shown).
- Antenna 9 has the object of contacting the radiofrequency signal with the toroidal piezometric ceramic element 1 1 for the generation of the ultrasonic vibration, transmitted by the same cone to the cannula 2.
- Signal transmitted from the antenna 9 has the same frequency of the ultrasound signal, since both the antenna and the piezoelectric ceramic element 1 1 are supplied by the same generator.
- Sleeve 8 allows the coupling of the box with the sucking system and is angled with respect to the body axis 1 , to prevent that possible sucked particles can soil the optical parts of the laser.
- Body face 1 opposed to that from which the cannula 2 exits is closed by the lens 9, that let the laser ray 6 passing and insulates the block comprised of the radiofrequency generator for the excitation of the piezoelectric ceramic element 1 1 and for supplying the antenna.
- box A for the first phase treatment (shown in figure 1 ) is coupled with blocks B, C and D.
- Generator shown in figures 3 and 4 is comprised of four blocks:
- - block B comprised of a sucking pump and relevant particle interception filter
- - block C comprised of the feeder of the Laser source
- - block D comprised of the high tension and low intensity current feeder for producing the negative ion flow
- block E comprised of radiofrequency generator for excitation of the piezoelectric ceramic element 1 1 and for supplying the antenna 9.
- the functional part of the solution suggested according to the invention is focused on two handles, respectively carrying out the functions of exfoliation and suction of the cells, so that they must be realised very carefully.
- the treatment that can be carried out by the apparatus according to the invention provides two phases, respectively an exfoliation and a reconstruction phase, both providing the use of a laser, the latter becomes a common element.
- the handle for treatments substantially provides the LASER source fixedly coupled with the generator, and two separated boxes provided with a suitable optical, mechanical coupling system between the boxes and the same source.
- the treatment is mainly carried out under sucking conditions, and the laser source is provided with delicate alignment systems of the optical cavity, is unavoidable that it is not subjected to stresses due to the pressure, that could damage the same.
- the treatment boxes are pneumatically insulated with respect to the laser source in such a way that the latter can work exclusively under atmospheric room pressure.
