  • the anterior cruciate ligament is an important ligament in the centre of the knee that provides the knee with stability. It can rupture after certain types of trauma and in some cases requires ACL reconstruction.
  • grafts such as a portion of the patella tendon or hamstrings to surgically replace the deficient ACL.
  • these grafts are free grafts ie detached at both ends and then reattached at both ends by a fixation device- be it "interference screws" or "endobutton suspension”.
  • My invention will incorporate parts of 2 preexisting separate inventions ie my invention is a concept of assimilation.
  • Drawing 3 will illustrate how I intend to use my invention as a rachet mechanism to tension the ACL graft.

  • the KTl is a new way of fixing and tensioning the ACL graft in ACL reconstruction surgery of the knee (whether or not the graft is left attached to the tibial tuberosity or detached from the tuberosity and fixed there with a screw instead).
  • the ACL graft will be left attached to the tibial tuberosity as shown in drawing 3.
  • I will detach the top end of the graft from the knee cap and then stitch that bit to the 3 drill holes on the cut tail of the cable tie as shown.
  • the cable tie has a very high pull out strength, approximately 25 kg, is more than the interference screws. The cable tie remains in the bony tunnel of the femur.

Abstract

L'invention concerne un dispositif de mise en tension de ligament, comprenant 1) un endobouton destiné à venir en butée sur le fémur, 2) une première bande pourvue de dents et aux extrémités de laquelle sont ménagés des trous permettant de fixer ladite bande à un ligament et à un fil de suture et 3) une seconde bande présentant une tête de verrouillage à une de ses extrémités et plusieurs trous ménagés à l'extrémité pour fixer cette bande à l'endobouton, la tête de verrouillage permettant le passage de la première bande dans une seule direction. Ce dispositif, appelé dispositif de mise en tension Khashaba 1 (KT1), est un concept d'assimilation procurant une nouvelle méthode pour fixer et mettre simultanément sous tension une greffe de LCA dans une reconstruction du LCA du genou. Selon l'invention, la greffe reste fixée à la tubérosité tibiale antérieure afin de maintenir la circulation du sang. Néanmoins, même si la greffe n'est plus fixée, elle doit être à nouveau fixée à l'aide d'une vis, ce qui fait que le KT1 constitue dans ce cas également un dispositif de mise en tension pour une reconstruction du LCA.
