Abstract

The invention discloses an extensible filtering sheath for surgical use for vena cava or large blood vessels. It consists of a sheath (2) formed by two radially extensible hollow tubular elements (5, 6), with a malleable, semirigid and self-extensible latticed wall, mutually linked by two peripheral longitudinal bars arranged to open up into a cone shape across the treated vessel, to form a filtering grid when one of these two elements, the supporting element (5) expands radially, the second expandable element remains compressed, but is shaped to be expanded subsequently so as to stop the filtering by completely clearing the passage of the treated vessel. It is generally useful for filtering or decreasing the blood flow to avoid certain necrotic lesions in tissues due to circulatory disorders caused by excessively high blood pressure, by thrombosis or clotting, and in particular for limiting the risks of infarction caused by the obstruction of the myocardium vessel by a clot or a foreign body carried by the blood circulating in the vena cava.

Description

The present invention relates to an expandable sheath filter cellar for surgical use or large vein blood vessels.

It is intended generally to filter or reduce blood flow to avoid some necrotic tissue due to circulatory disorders caused by too high blood pressure, thrombosis or embolism, and in particular to reduce the risk of infarction caused by occlusion of a vessel infarction by a clot or foreign material from blood flowing into the vena cava.

The devices used so far are generally made of self-expanding filters formed of fibers 1 (figure 1) connected by one of their ends and s' opening umbrella when the filter is released once brought to the chosen location. These prostheses are previously compressed to be introduced into a tube mounted at the end of a catheter, and then driven out of the latter when in place, the elasticity of the constituent material ensuring their radial extension.

This type of filter has a number of disadvantages. Indeed, it is very difficult to accurately position and not only it tends to migrate after installation, but it often gets in at the time of installation. Moreover, in most cases, its action must be deleted after a certain time, which requires extraction. Often this requires major surgery to have direct access to the ship concerned.

The device of the present invention eliminates all these disadvantages. Indeed, it provides filters for blood vessels with no risk of migration while facilitating accurate positioning and an easy retirement free service operation. These filters can under no circumstances get in the vessel and also simultaneously ensure the expansion of the latter facilitating the flow of blood flow. Finally the implementation runs through a hole of diameter substantially less than that required for existing systems.

It consists of a sheath formed of two tubular members radially expandable hollow, semi-rigid wall self-expanding or malleable mesh, interconnected by longitudinal peripheral bars arranged to diverge conically through the vessel blood to be treated, to form a filtering screen when one of these two elements, the holding element undergoes a radial expansion, the second expandable member residing in the compressed state, but is configured to allow its subsequent expansion to remove filtering completely freeing the passage in the treated vessel.

In the accompanying schematic drawings, given as non-limiting examples of embodiments of the object of the invention: Figure 1 (already mentioned) is a vessel filter the type currently used. 2 shows before expansion, side view, a filter sheath according to the invention mounted on an inflatable balloon used to cause the expansion of the retaining element, Figure 3 shows in the same way the sleeve after expansion of the retaining element, Figure 4 shows the expansion of the second element to remove the filtering effect and figures 5 and 6 illustrate two variant embodiments of the filter jacket.

The device, Figures 2 to 6, comprises a tubular sheath 2 formed of a hollow cylindrical portion of circular section and whose wall consists of a lattice or grid made of malleable semirigid material arranged to allow said sheath to expand radially under the effect of an internal pressure and to keep the shape thus obtained. The internal pressure is caused for example by means of an inflatable balloon 3 mounted at the end of a catheter 4 in accordance with a known technique.

The sheath 2 is subdivided into three elements. one of the ends, the holding element 5, which is the longest in order to prevent any risk of migration, has an expansible lattice structure and has the role of ensuring good adhesion of the device to the desired location within the vessel, as well as the expansion of the latter. When installing, only that element is disposed on the balloon 3 to be expanded. At the other end, a second member 6, normally shorter, but the same structure as the previous remain compressed during the establishment. Therebetween, the filter element 7 itself consists of longitudinal bars 8 devices. When the holding member is expanded by inflation of the balloon 3, that the second member 6 does not undergo expansion, these bars open conically across the blood vessel to form a grid providing the desired filtration ( Figure 3).

To remove the effect of the device and completely free passage in the treated vessel, simply subjecting the second element 6 an expansion in the same way as to ensure the implementation of the sheath 2 in the vessel is -to say using inflatable balloon 3 'mounted at the end of a catheter 4, and inserted inside said second element, which is configured to allow, eliminating the need for surgery ( Figure 4).

The sheath 2 is preferably in one piece and produced by EDM laser from a block of metal such as stainless steel or tantalum, or by laser etching or machining of a thin-walled metal tube . The mesh constituting the two expandable elements 5, 6 is for example constituted by longitudinal elongate elements interconnected by the annular elements arranged in staggered rows, but may be of any configuration that allows an adjustable extension according to the applied pressure.

The filter rods 8 preferably have a width close to 20/10 mm, the width of the elongated members 9 forming the mesh being of the order of 13/10 mm.

The sheath 2 is preferably manufactured in expanded form, then "crushed" on the inflatable balloon 3. According to an alternative embodiment, the sheath 2 is comprised of a central five holding element connected by two 8 filtration busbars two expandable elements terminals 6 (Figure 6).

According to another alternative embodiment, the sheath 2 is self-expanding and in expanded form made of a flexible material or shape memory. For its implementation, it is radially compressed and inserted into a hollow tube of small diameter mounted at the end of a catheter and said tube driven by a suitable means when it is in the designated location.

The expansion or terminal elements (6, 6 ') is temporarily prevented through one or more son résorbableε Long-resorption, allowing this or these end members to resume their original shape after a time or long, depending on the number of son, without any intervention.

In order to facilitate movement of the sheath 2 when it is in place, the free end of the second extendable member 6 (and, if applicable, of one other terminal element 6 ') may be cut at an angle ( Figure 5).

For some applications, the sheath 2 may comprise integrated fastening means, radialementε directed outwardly so as to dig into the inner wall of the vessel under the effect of the pressure of the inflatable member 3, thereby preventing all the mobility of the device.

The particular structure of the casing 2 enables the use as an anchor member of a tubular prosthesis, particularly in the case of abdominal aortic aneurysm in the vicinity of the departure of the renal arteries. In this case, the entire sheath is expanded

The particular structure of the casing 2 enables the use as an anchor member of a tubular vascular prosthesis in the case of aneurysm in the vicinity of venous or arterial branches, especially for abdominal aortic aneurysm located near departures of the renal arteries.

In this case, the whole of the sleeve 2 is expanded once brought to the correct location, so that the rods 8 are longitudinal peripheral to the location of the lead or to allow easy passage of the blood, the holding member 5 being applied against the walls of a healthy portion of the vessel to ensure the attachment, and the short member 6 facing the aneurysm for supporting the vascular prosthesis.

The junction of the sheath 2 and the prosthesis may be effected by means of an inner cylindrical annular member or outer or fastening elements such as hooks, provided for this purpose on the short member 6.

To prevent one endothelium of the vessels to penetrate inside the holding member 5 through the meshes of the mesh, it will be advantageously covered with a flexible film in synthetic material such as polyurethane, silicone or polyester.

To facilitate operations for setting up and annihilation of the filter, the sheaths 2 preferably will include radiopaque markers consisting of X-rays and consist of layers of a heavy metal such as tantalum, titanium or gold deposited by electrolytic means. These layers may have a thickness of 0.1 mm.

The positioning of the various components gives the object of the invention a maximum of useful effects which had not been, to date, been obtained by similar devices.

Claims

1. Sheath expandable surgical filter vena cava or large blood vessels, intended to filter or decrease the blood flow of the veins and major arteries and especially the vena cava, in order to limit circulatory disorders caused by a blood pressure too high or the risk of infarction caused by occlusion of a vessel infarction by a clot or foreign body from blood flowing in said sheath (2) being self-expanding or of the type intended to be placed in place by means of an inflatable balloon (3) mounted on a catheter, and consisting of a cylindrical tube portion may extend radially and keep the resulting shape, characterized in that it is formed by two elements (5 , 6) of different lengths having a wall radially expandable lattice structure interconnected by bars (8) longitudinal peripheral arranged to departs r conically across the blood vessel to form a filtering screen, when the longest of these two elements, the holding element (5), undergoes a radial expansion, the short element (6) not subjected to expansion.
2 °. Sheath according to claim 1, being characterized in that it is made of malleable semirigid material, the short element (6) being configured to allow to insert an inflatable balloon (3 ') mounted at the end a catheter (4) to cause its expansion to remove the filtration fully releasing the passage in the treated vessel. 3 °. Sheath according to claim 2, being characterized in that it is fabricated in expanded form, then "crushed" on the inflatable balloon (3).
4 °. Sheath according to Claim 1, characterized in that it is self-expanding and in expanded form made of a flexible material or shape memory so as to perform its introduction in the radially compressing and introducing in a hollow tube of small diameter mounted at the end of a catheter, then driving the outside of said tube when it is in the designated location, the expansion of the short member (6) is temporarily prevented through one or more son resorbable long resorption, so that it resumes its original shape after a more or less time depending on the number of son, without any intervention.
5 °. Sheath according to any one of the preceding claims characterized in that the holding element (5) is sized to simultaneously ensure fastening of the device to the desired location within the vessel prohibiting migration and expansion of said vessel.
6 °. Sheath according to any one of the preceding claims characterized in that it is in one piece and produced by electroerosion from a metal block, such as stainless steel. 7 °. Sheath according to any one of the preceding claims characterized in that it consists of a holding element (5) connected by central two busbars (8) filtration two expandable terminals short members (6, 6 ').
8 °. Sheath according to any one of the preceding claims characterized in that the rods (8) filter have a width close to 20/10 mm, the width of the elongate members (9) forming the mesh being of the order of mM 13/10.
9th. Sheath according to any one of the preceding claims characterized in that the free ends of or short members (6, 6 ') are radially expandable cut on the bias so as to facilitate movement of the sleeve (2) when its implementation.
10 °. Sheath according to any one of the preceding claims characterized in that it comprises integrated latching means directed radialements outwardly so as to dig into the inner wall of the vessel under the effect of the pressure of the inflatable balloon (3), thus preventing any possibility of displacement of the device.
11 °. Sheath according to any one of the preceding claims characterized in that the holding element (5) is covered with a flexible plastic film so as to prevent one endothelium of the vessels to penetrate inside through the lattice mesh. 12 °. Sheath according to any one of the preceding claims characterized in that it comprises radiopaque markers consisting of dots or longitudinal or annular strips visible to X-rays and consist of layers of a heavy metal such as tantalum , titanium or gold deposited electrolytically, said layers having a thickness of 0.1 m.
13 °. expandable sheath for surgical use louse blood vessels, intended to serve as an anchoring element of a tubular vascular prosthesis in the case of aneurysm in the vicinity of venous or arterial branches, especially for abdominal aortic aneurysm located close to departures of the renal arteries, said sheath (2) being self-expanding or of the type intended to be put in place by means of an inflatable balloon (3) mounted on a catheter, and consisting of a cylindrical tube portion radially expandable and keep the resulting shape, characterized in that it is formed by two elements (5, 6) of different lengths having a wall radially expandable lattice structure interconnected by bars (8) longitudinal peripheral allowing easy passage of blood to the arterial branches or from venous bypasses, the longest of these two elements, the element may ntien (5) being intended to be applied against the walls of a healthy portion of the vessel to ensure the attachment, the short element (6) being oriented towards one aneurysm and serving as a support for said vascular prosthesis.
14 °. expandable sheath of claim 13, characterized in that the short element (6) has securing elements for the junction of the sheath (2) and the vascular prosthesis.
