Abstract

Un procédé d'élimination d'héparine de l'organisme ou de dérivés d'héparine de faible poids moléculaire dans lequel un filtre de sang contenant une enzyme immobilisé, se composant d'enzymes telles que l'héparinase, glucuronate-reductase, aldose-reductase, beta-glucuronidase, O-sulfatase, et N-sulfatase, ou d'autres composés de dégradation ou de neutralisation d'héparine, est conçu pour une utilisation au niveau de l'effluent d'un dispositif extracorporel. Le dispositif satisfait aux régles rigoureuses spécifiques à une utilisation clinique. La biocompatibilité du système dans son ensemble est la première condition. Deuxièmement, il doit être conçu pour éliminer les niveaux cliniques d'héparine à des débits variables allant jusqu'à 4 litres de sang/min pour un adulte et jusqu'à 50 ml de sang/min pour un enfant, avec des durées de perfusion variables allant de plusieurs heures jusqu'à entre deux et dix jours en cas de troubles respiratoires neo-natals. Des modes de réalisation spécifiques employant de l'héparinase liée par chlorure de trésyle ou par des techniques CNBr, à des dispositifs de fibre creuse de cellulose reconstituée ou du type utilisé pour la dialyse du foie, sont divulgués. Ces modes de réalisation incluent des dispositifs à étage unique ou multiple destinés à l'élimination d'héparine seule ou conjointement avec dialyse ou oxygénation. Des dispositifs analogues à feuilles de membranes multiples sont également divulgués.
