Description

DISPOSITIF DE COMMANDE DU DEBIT D'UN FLUIDE, ORGANE PROTHETIQUE EQUIPE DE CE DISPOSITIF ET PROCEDE DE FABRICATION D'UN TEL DISPOSITIF.DEVICE FOR CONTROLLING THE FLOW OF A FLUID, PROSTHETIC MEMBER EQUIPPED WITH THIS DEVICE AND METHOD FOR MANUFACTURING SUCH A DEVICE.
The present invention relates to a device for controlling the flow of a fluid, in particular a biological fluid.
A device for controlling the flow of a fluid, which comprises a valve of the type sensitive only to the pressure obtained by manual deformation of at least one flexible activating balloon containing a fluid for controlling the valve, is described in the Application of French patent n ° 83 14607.
The valve that is the subject of this French Patent Application is produced by cooperation between a rod and a sheath in which the rod is tightly enclosed by any suitable clamping means, this clamping sheath being made of a flexible plastic material having a shrinkage upon polymerization and being intended to deviate from the rod which it envelops under the action of the pressure exerted by said control fluid during the deformation of the activating balloon which contains it. The rod is advantageously provided with means intended to prevent its axial displacement under the action of the pressure of the control flui¬ authorizing the expansion of the sheath and therefore the opening of the valve. The use of this valve in interchangeable type urethral prostheses, for male and female use, which are intended to be installed without surgery and which also constitute another object of the French Patent Application No. 83 14607 aforesaid has however shown that sometimes, the opening of the valve by expansion of said tightening sheath of said rod under the action of the control pressure does not take place or takes place with difficulty, in particular because of the adhesion of the silicone elastomer (of which the tightening sheath is composed) against the latter, adhesion which is due to the natural aging process of the silicone elastomers and which is accentuated when these elastomers remain inactive for a long time.
The object of the present invention is to remedy this drawback so that the good performance of the valve is always ensured in response to each control action, even when its opening frequency is particularly low.

The present invention relates to a device for controlling the flow of a fluid, in particular a biological fluid, of the type comprising at least two flexible net balloons connected by a connection tube made of a flexible plastic material having a withdrawal from the polymerization, a valve system of the type sensitive only to the pressure resulting from the manual deformation of one of the above-mentioned balloons, which valve system comprises a rod tightly clamped, in the condition of rest, by any suitable tightening means, inside the above-mentioned connecting tube, which control device further comprises means capable of preventing the axial displacement of the valve stem along the connecting tube under the effect of said pressure, said control device being characterized in that the rod of said valve system comprises means for initiating the loosening, namely expansion or removal, the adite tight tightening sheath of this tig under the effect of the pressure of a fluid whose flow is controlled by said valve stem.

It should be understood, however, that these drawings and the corresponding descriptive parts are given solely by way of illustration of the subject of the invention, of which they do not in any way constitute a limitation.
La figure 1a se réfère à un mode de réalisation du dispositif de commande de l'écoulement d'un fluide, notamment d'un fluide biologique,et en particulier de l'écoulement uri- naire, du type comprenant un système fermé rempli d'un fluide et comportant deux ballonnets activateurs A et B souples reliés par un tube D dont la paroi serre directe¬ ment et de façon êtanche une tige E insérée à l'intérieur de ce tube, étant entendu que le serrage de ladite tige E peut être réalisé par tous moyens assurant également la séparation êtanche entre les ballonnets activateurs souples, en condition de repos.FIG. 1a refers to an embodiment of the device for controlling the flow of a fluid, in particular a biological fluid, and in particular of the urine flow, of the type comprising a closed system filled with a fluid and comprising two flexible activating balloons A and B connected by a tube D, the wall of which directly and tightly squeezes a rod E inserted inside this tube, it being understood that the tightening of said rod E can be carried out by any means also ensuring the watertight separation between the flexible activating balloons, in the rest condition.
The axial displacement of the rod E in the tube D is prevented by equipping each of the ends of this rod E with a sphere s_, for example, as indicated in FIG. 1a, thus giving the rod a dumbbell configuration.
This variant shows that the same result, namely maintaining the rod E in position, can also be obtained by connecting the tube D to each of two activating balloons A and B by a tube 1 ^ whose diameter is less than the diamè¬ be from tube D.
A.E it contains, thus ensuring the watertight separation of the two balloons A and B, the volume of which is maximum in the rest position. The cooperation between the rod E and the tight tightening sheath D (in rest condition) therefore defines a valve system in accordance with the provisions object of French Patent Application 83 14607. The operating principle of the control device as set out in said French Patent Application is based on 1 tightness between the two activating balloons A and B, as well as on the elasticity of the wall of the tube D and on the tendency of the balloons A and B to return to their original shape as soon as they are not requested. More specifically, an increase in pressure, for example in the balloon A, obtained by manual or digital support, causes the wall of the tube D to separate from the surface of the rod E and, consequently, the passage from the compressed fluid contained in balloon A to balloon B, where the fluid is stored. When the pressure on the balloon A is released, the fluid stored in B does not return spontaneously to A, owing to the fact that, if the latter tends to return to its initial shape, the tube D nevertheless opposes this movement in tightening tightly on the rod E, which causes a depression in the balloon A and therefore a suction of fluid from the balloon A to the balloon B, namely the deflation of the first balloon A. It goes without saying that an increase pressure in balloon B causes the reverse phenomenon and the transfer of fluid from B to A, and therefore deflation of balloon B and inflation of balloon A.
On the basis of the above, it is very well understood that if, for example, the activator balloon A is inserted in a duct, it is possible to open and shut off, respectively, the lumen of this duct in pro ¬ yielding in the manner described above, namely by alternately compressing the two activating balloons A and B: it goes without saying that the shape of one of the two net balloons (for example that of balloon B) can be a priori any, in particular β spherical, annular or other, but in any case such that it can adapt to the different applications which the control device according to the invention is capable of, while the shape of the other balloon , (therefore that of the balloon A) must be able to adapt, in the state of rest, to the shape of the conduit which it must close, in order to be able to fulfill the role of balloon both activator and obturator of the conduit whose flow must be ordered. In accordance with the present invention, the valve stem E comprises means for initiating the loosening, namely expansion or spacing, of its tight clamping sheath D, capable of ensuring the operation of said valve system whatever that is the frequency of control of this valve, namely in spite of the natural process of aging of the elastomer of which the sheath is made up D.
To this end, and in a nonlimiting manner, the valve stem E has, in each of the two embodiments illustrated in FIGS. 1a and 1b, and at each of its ends, an axial blind channel which is in communication with each other. tion with the control fluid contained in each of the two balloons A and B as well as with at least one channel opening onto the external surface of the rod E at the corresponding end of the internal surface of the tight tightening sheath D .
With regard to the possible, but not limiting, arrangements of said channel opening onto the surface of the rod, it is possible to provide for this purpose in each of the ends of this rod:
- un canal radial unique, et ce du même côté ou de part et d'autre par rapport à l'axe longitudinal de la tige de soupape,- a single radial channel, on the same side or on either side with respect to the longitudinal axis of the valve stem,
- deux canaux radiaux situés sur la même perpendiculaire à l'axe longitudinal de la tige de soupape, ou encore - trois canaux radiaux ou une pluralité de ces canaux disposés autour dudit canal axial comme les "rayons" d'une étoile.- two radial channels located on the same perpendicular to the longitudinal axis of the valve stem, or - three radial channels or a plurality of these channels arranged around said axial channel like the "spokes" of a star.
In addition, the channel emerging at the external surface of the rod E can have a path a priori arbitrary, for example spiral or other.
En particulier, on peut ménager au moins une gorgeIt goes without saying that the means for initiating the loosening of the tightening sheath of the valve stem are not limited to the embodiments of FIGS. 1 to 6. In particular, at least one groove can be provided
Although the arrangement of these grooves can be of any wall, Figures 9 (and 10) and 11 show two advantageous arrangements constituted by 4 longitudinal grooves g_ arranged at 90 ° from each other on the valve stem E. and by a helical groove g, formed on the valve stem E 5 , respecti¬ vement. Of course the number of longitudinal grooves g_ and the angular spacing between two of these consecutive grooves are not limited to 4 and 90 °, respectively.
La figure 13 représente une autre variante des moyens d'amorce du desserrage du cathéter J qui sont constitués par un canal axial a communiquant avec un canal radial r_ qui à son tour communique avec une gorge annulaire g_~ ménagée à la surface de la tige E_ .FIG. 13 represents another variant of the means for initiating the loosening of the catheter J which are constituted by an axial channel a communicating with a radial channel r_ which in turn communicates with an annular groove g_ ~ formed on the surface of the rod E_ .
In each case, the pressure of the control fluid contained in each of the two balloons A and B is sufficient for the jet of fluid, which follows the axial channel a and the channel or channels (possibly radial or radial) opening to the external surface of the rod E, allows the localized spacing of the tight clamping sheath D by relative to this rod, thus allowing the passage of the control fluid between the external surface of said valve stem and the internal surface of its tightening sheath.
Les orifices o mettent en communication les canaux axiaux bor¬ gnes a, (qui sont destinés à communiquer à leur tour avec le fluide de commande dont l'écoulement est contrôlé par la soupape) avec la surface externe de la tige de soupape. lesdits canaux a. et les orifices o constituant les moyens d'amorce du desserrage d'une gaine (non représentée à la figure 2) enveloppant la soupape E1 .It goes without saying that, although the description of the application of the means for initiating the expansion of the tight tightening sheath D relative to the valve stem, has been limited, in the foregoing, to only embodiments illustrated in Figures la_ and lb and which correspond to Figures la and lb of French Patent Application 83 14607 cited above, it will be readily understood that the application of the above loosening initiation means extends also to the other embodiments described in the aforementioned French Patent Application and, in particular to those shown in Figures 2, 3 and 4 thereof. It is also clear that the application of the means for initiating the expansion of said tight tightening sheath relative to the valve stem according to the present invention extends to other systems than those described in the French Patent Application. n ° 83 14607 above. To this end, FIG. 2 represents a mode of realization of the valve stem E, which - in accordance with the variant illustrated in the appended FIG. 1b as well as in FIG. 1b of French Patent Application n ° 83 14607 - has no bulges at its ends. However, in accordance with the present invention, the valve stem is constituted by a portion of tube E, having an intermediate partition £ delimiting, in cooperation with the wall of this portion of tube, two blind axial channels a, located on both sides with respect to said partition c. The wall of the tube portion E. is provided with at least one orifice o formed on either side of the transverse partition c. The orifices put in communication the axial channels bor¬ a, (which are intended in their turn to communicate with the control fluid, the flow of which is controlled by the valve) with the external surface of the valve stem. said channels a. and the orifices o constituting the means for initiating the loosening of a sheath (not shown in FIG. 2) enveloping the valve E 1 .
This valve - which has been shown so that one half, symmetrical with respect to the partition c, is dotted - applies to the case where the flow of the control fluid is bilateral, that is to say that it takes place in both directions, in relation to the application to Figures la and lb. It goes without saying that the bilateral flow of the control fluid is not limiting, just as this fluid is not necessarily limited to exerting, by the action of its pressure, a control function of the rod of valve according to the invention: it can also exert a therapeutic action via the valve for controlling its flow, for example in one direction only. In this order of ideas, the valve of figure 2, and more precisely one of the two halves E_ of the valve E, described above - which is the only functional part usable in the case of flow unitaleral of said fluid with therapeutic properties - is susceptible of an original and immediate application to devices for repeated injection of medication, in unit doses and ^ a frequency which depends on the needs of the therapeutic treatment of a given patient.
These are devices intended to be implanted in the body of a patient in a subcutaneous position and of the type comprising (cf. FIG. 3 appended to this description): - a site S for injecting the medicament, intended to be implanted below the patient's skin,
En fait, étant donné que l'extrémité distale du cathéter J est bouchée par la soupape E2 et que ce cathéter se referme rapi¬ dement sur la soupape après chaque utilisation, chassant tout résidu de sang éventuellement présent, aucun risque de coagu- lation de sang ne se pose, et donc de thrombose par introduc¬ tion de caillots dans le circuit sanguin du malade.an external means of supplying this site with said drug, comprising in particular a needle g_ * of pi¬ q re of said site connected to a syringe (not shown) or to a pump (also not shown), such as a pornp *; with rollers or the like, and - An injection catheter J of each unit dose of drug, intended to be implanted in the area of therapeutic action, in particular consisting of a vein or artery V of the blood circuit of a patient. Application to the device of FIG. 3, a detail of which has been shown on a larger scale in FIG. 4, makes it possible to eliminate the problem of blockage of the distal end of catheter J which takes place after each injection of mediate by blood coagulation, which causes the formation of clots which in turn pose the problem of thrombosis. In fact, given that the distal end of the catheter J is blocked by the valve E 2 and that this catheter closes quickly on the valve after each use, expelling any residue of blood which may be present, no risk of coagulation no blood arises, and therefore thrombosis by introduction of clots into the patient's blood circuit.
Celle-ci montre des lèvres L de blocage de la tige de soupape Eg , pourvue de moyens d'amorce du desserrage du cathéter J constitués par un canal axial borgne a communiquan avec deux canaux radiaux r. et r- .Figures 5 and 6 essentially show alternative embodiments of the device shown in Figures 3 and 4 and show a thinned area j. 1 & extré- wall of the distal catheter mite J. This thinning is advantageous because the drug pressure is not to be high (such as, for example, in the case where the injection device is connected to a pump with rollers), so as to reduce the resistance to loosening of the catheter wall. With regard to FIGS. 7 and 8, these show means for holding in position the stem of the pope which are alternative to the bulges at the end as shown in FIG. 1a (see also French patent application n ° 83 14607). These alternative means are constituted by a tube por¬, which abuts against a ren¬ trant shoulder formed in the catheter J, and by a wire f_ connecting the tube portion t to the valve stem E, and ∑ 2 . This wire f_ and this portion of tube t are preferably made of stainless steel or plastic. Means for holding the valve stem in position (also alternative to those shown in FIGS. 1a, 1b, 7 and 8) are shown in FIG. 12. This shows lips L of blocking of the valve stem E g , provided with means for initiating the loosening of the catheter J constituted by a blind axial channel a communicating with two radial channels r. and r-.
Said locking lips L protrude on either side of the valve rod E g from the internal wall of the catheter J, thus delimiting for this rod E g a housing disposed at the distal end of the catheter.
As is apparent from the above, the invention is in no way limited to those of its embodiments and of application which have just been described in a more explicit manner; on the contrary, it embraces all the variants which may come to the mind of the technician in the matter without departing from the framework, or the scope, of the present invention.
