To all whom it may concern:

Be it known that I, CHARLES D. SoHIrr- LER, a citizen of the United States, residing.

at Columbus, in the county of Franklin and State of Ohio, have invented a certain new and useful Improvement in ManifoldRecording and Mailing Devices, of which the following is a specification.

The object of this inventionis to provide an improved manifold recording and mailing device whereby when a record card is filled out with desired data a plurality of mailing cards may by'the use of a duplicating paper, be contemporaneously filled out and addressed by one or two passings of the card through a type-writer. The invention will be particularly useful in the Workmens compensation department of state industrial commissions where correspondence is to be separately conducted with claimants for in'-. jury and with their employers. As now carried on in such commissions a large force of employs is necessary in the making of records in books, in the handling of papers and the mailing and receiving of reports.

In the accompanying drawings therefore, I illustrate my invention as the same may be prepared, in part, for use in the business of such commission but obviously the invention can be used with advantage in other businesses.

Fig. 2 is a similar view of the opposite side Without inversion verticallyto better show what printing occurs on the opposite side of the cards shown in Fig. 1.

Fig. 3 shows the upper portion folded over forward fortyping with a carbon or duplicating paper inserted.

Fig. 4 shows the strip of Fig. 2 inverted and the then upper card folded over rearward for typing with an inserted carbon or duplicating paper.

Fig. 5 is a perspective View, partly broken out, of the card portion opened from the position shown in Fig. 3. and the duplicating or carbon paper removed to better illustrate the function of the registering section or card hereinafter referred to.

In the views 5 designates the record card, 6 the card to be addressed to the employ, and 7 the card to be addressed to the. employer. The card 8 is left attached as hereinafter described to-the card 7 when mailed 1923. Serial No. 643,060.

,Thestrip of cards is of a width about equal to the width of an ordinary United States postal card and the strip is marked off or perforated on transverse parallel lines shown as broken lines dividing the cards 5, 6, 7 and 8 to make them of a depth when separatedabout equal to the depth of such a postal card.

The card 5 is ruled near its top at the rear to provide spaces or aguide for typing thereon the employs claim number, name, address and date of injury, and it is also ruled, on the same side near the bottom to provide spaces or a guide for the employers risk number and his or its name and address.

The card 6 is ruled to'provide space. for the serial number given by the commissioner to the claim and the date of the injury; and between the cards 5 and 6 is provided what may be termed a registering card or member 9 the function. of which is to cause the registering or coinciding of the ruling of the record card 5'with that of card 6 when folded forward on the line marked Fold here? as shown in Figs. 2, 3 and When the parts have been so folded a duplicating or carbon sheet 10 is inserted under theforward folded part to imprint the cards beneath and the card 5 typed with the data indicated. When so typed the cards 6 and 7 are addressed to the employ and employer respectively together with the employs claim, serial number and the employers risk number respectively. After this operation the strip is removed from the typing machine and turned over vertically and the then top card folded rearward and withan interposed carbon sheet 12 put into the machine to receive a date for example of the date of notice to employer or the date of injury as may be elected. The card 8 is printed with the address of the commission on one side and on the other with matter suitably instructing the employer to make report to the commission as to when the employ resumes work. The cards 6, 7 and 8 are also provided with spaces for printing or applying a postage stamp.

The strip 11 at the top of Fig. l is provided to insure the holding of the card 5 and carbon against the roller platen of the typewriting machine so that it shall not spring away and may be properly typed near the bottom.

After the typing has been completed the strip 11 and registering card or section 9 are removed and thrown away, the card 5, constituting the record of the claim, being retained by the commission in its ofiice for alphabetical or other arrangement with the other like cards. The card 6 is removed and mailed to the employ-claimant and the two cards 7 and 8 are mailed as one piece to the employer. When the workman returnsto work the employer indites that fact and its date on card 8, detaches it and mails it to the commission.

It-will thusbe observed thatrby two passings of the strip through the type writing machine with the appropriate typing there is provided a record card for the commission, a notification card addressed to .each the employ and employer with a date otthe return card to the employer showing the date of claimed injury or notification thereof thereby avoiding-the multiplicity of operations heretoitore incident to such business.

In some uses of the invention a card corresp nding to card 8 can be dispensed with or the printing and manipulation of card 8 as well as other matters and details can be changed or modified without departing from the gist of the invention as claimed.

\Vhat I claim is:

l. A manifold recording and mailing device consisting of a strip divided by transverse-lines to form a record card and a plurality of mailing cards of substantially equal depth and a registering card of less depth than said record and. mailing cards, said registering card occurring between the record card and the plurality of mailing cardsand adapted toflproject the record card to lie over some of the mailing cards when the strip is folded at the junction of the registering card with the mailing cards whereby datamay, with the aid of a dupli cating paper interposed between the record and said mailing cards, be imprinted upon said record cardand one side of said mailing cards in one passage of the strip through a type writing machine, the terminal mailing card of said .strip being reversely toldable to imprint, with the aid of a duplicating sheet, data upon it and the opposite side of one of said mailing cards.

A manifold recording and mailing device consisting of a strip divided by trans? verse lines to form a record card and two mailing cards of substantially equal depth and a registering card of less depth than said record and mailing cards, said registering card occurring between the record card and the two mailing cards and adapted to project the record card to lie over the two mailing cards when said strip is folded at the junction of the registering card with the mailing cards whereby data may be simultaneously imprinted with the aid of aduplicating paper interposed between the record and mailing cards upon all three cards in one passage of the strip through a typewriting machine.
