Be it known that I, JosnPH Pinares, a citizen of the United States, residing at Pocatello, in the county of Bannock and State of Idaho, have invented certain new and useful Improvements in Commercial Forms; and I do hereby declare the following to be a full, clear, and exact description of the invention, such as will enable others skilled in the art to which it appertains to make and use the same.
My invention relates to blank forms and consists in certain novel and useful improvements in the same, whereby the conducting of business may be simplified.
It frequently happens, that an individual or a banking institution or other concern, is called upon to forward to a State, county or city official, a number of documents, such as deeds, releases, mortgages, satisfaction of mortgages, etc., for recording. In such cases, under the present system, it is necessary to forward the papers, with a special letter of advice, of which a carbon copy must be kept, with a request that the recording oliicial notify the sender of the amount of the fees charged for recording, and withthe promise of a, remittance to cover such charges, and in the mean time it may happen thatthe papers are not recorded, until the fees are received. The object of my present invention is to simplify this procedure, by producing a form in which there are combined a letter of advice, a certificate of an official that the papers mentioned in the letter of advice we re received onl a certain date and led for record, and a draft on the sender for the amount of feescharged for recording the instruments. A copy sheet is provided, properly delineated in order that certain desirable data may be made and kept of record. In order that my invention may be clearly understood, I have illustrated it in the accompanying drawings, in which- Figure l is a view of my improved form filled out with fictitious entries.
Fig. 2 is a view of the copy sheet used in connection with the form.

1 represents my improved form vas employed in banks, filled out with fictitious entries showing how it is used in practice. In this instance the form is 'provided with a number, 2 and the name of the bank is printed upon it as at 3, giving its location.
A delineated date space 4 is also provided and the name or title of the proper official to whom the form is addressed, is inserted as at 5,' immediately following which there are spaces 6, 7 and 8 provided for the-name of the county, city 4and State in which the official functions.
Below the address is a space 9, for `the letl ter portion, which contains a general statement regarding the instruments inclosed for record, and directing the official to fill. in the amount of fees required for each instrument and to use the attached draft, hereinafter described, in drawing uponthe sender for the total amount of fees required. This letter is designed tobe signed in the name of the sending bank by one of its oflicers, in the` present instance theY president.
A request is also appended as at 10, asking that the receipt be acknowledged and that the form be returned for -the senders files.
Below the letter portion, there are provided designated columns and delineated spaces, to be filled in bythe sender.
The column 11 is designated for'the insertion of the kind Vof instrument inclosed, and in the present instance, it will benoted, for example, that there are three insertions, mortgage, satisfaction of mortgage and deed. Of course there could ybe any number of different kinds of instruments.-
Column 12 is designated as Date of inst. and is divided into sub-columns a, b, c to receive opposite the insertion of each instrument, the month, day and year of execution.
Column 13 is designated Executed by and is intended to contain the lnames of the parties, executing the various instruments.
Column 14 is designated In favor of and in this column lthere are to be inserted the names of the parties in whose favor the instruments are drawn. Y'
Column 15 is designated Fees and in this column the .recording official, is to enter the amount of vfees chargedv for recording each instrument, a space being left at the bottom ofthe column for inserting the total charge for recording all of the instruments.
Below the columns a space 16 is provided for a blank certificate to be filled out by the receiving official, in this instance the county auditor certifying thata certain number of instruments were filed for record, giving the hour, day, `month and year when received, and stating that he had drawn on the sender by draft detached from the form, as `hereinafter described, said draft bearing the same number as the form.
The draft 17, above alluded to, is of the usual style, and extends transversely across one end of the form, with which it is connected b y a weakened line of perforations 18, whereby the draft may be readily detached from the form. lt is to be noted that the draft has the same number as the form as shown at 2.
The forms are intended to kbe bound in book form, and alternate leaves 19 are provided as shown in Fig. 2, which are designed to be carbon copies of the data entered on the forms in the columns 11, 12, 13, 14 and 15. To this end, columns identical with those of the forms are provided, which normally register with the columns on the form, so that when a carbon sheet is inserted between the leaves and data is entered in the columns on the form, the same matter will be made simultaneously to appear on the intervening leaf as a carbon copy, which is permanently bound in the book or pad, while the form being provided with a weakened line at 2O may be torn off for transmitting purposes.
The copy leaf is likewise provided with a number as at 2", identical with the number appearing upon the form and draft. for purposes of identification.
In addition to the columns like those appearing on the form, the copy leaf 19 is provided with additional columns (l, e, f. These columns are designated as Amount chargcd;7 Date of charge and Charged to respectively, and are to be filled in b v the sender after the form is received back from the recording ofiicial.
In column Z is entered whatever sum the owner or maker of the instrument recorded,
n has paid in cash to the bank, abstract company, or other concern, which owns the book of forms and has forwarded the instrument for recording.
Ifthe ling fee is not paid but is to be charged to an account, the date when the charge is made is inserted in column e, and in column 7, the name of the party against whose account the charge is made.
It is obvious that all matter inserted by the sender on the form may be impressed Leonesa upon the alternating leaf through the me-V dium of a carbon sheet, thus vmaking' it possible to retain a permanent record of such mattei'.
it may also be noted, that for convenience in recognizing' the alternating leaves from the forms, the former may be given a different color, as yellow, for example.
The forms ,and alternating leaves when bound together, will appear as a book 21, as shown in Fig. 4.
The manner of using .my invention will be apparent from the foregoing.
A sender fills out the form, in the manner indicated in Fig. 1, .first 'placing a carbon sheet between it and the leaf 19, tears `the form from its stub 22, and incloses it, -together with the instruments noted, and addresses it to a proper official, for recording.
yThe ofhcial, upon receipt, will enter in column 15, opposite each instrument, the fee to be collected for recording same, total the columns, fill out the certificate `16 and re turn it to the sender, having first vmade out the draft 17, drawing upon the sender for the total amount of fees charged, and detached the said draft from the form. The draft is deposited in the offcials bank as cash or for collection.
Upon receipt of the returned form from the recording official to the sender, the latter makes the proper entries on the permanent leaf 19, in columns d, e and f and the record is complete. V
It is obvious that I may vary the details of the form from that described, without departing .from the spirit of my invention.
l/Vhat I claim and desire to secure by Let- Athe oliicial to whom sent, delineated and appropriately designated spaces for entering the kind of instruments transmitted, .their date, by whom executed, in favor of whom executed and fees charged, a certificate to be filled in with data certifying as to the number of instruments filed, the time `when filed for record andthe date of filing and a draft to be filled out by the receiving oilicial upon the sender.
2. A blank form as claimed in claim V1, in which the form proper and draft are provided with tearing lines.
3. The combination with a blank formV as claimed in claim 1, of means for making and retaining a copy of matter inscribed on form, said means comprising a delineated copy sheet, provided with lines and columns which register with sinfilar lines and columns on the form propfef.
