Be it known that l, vJosera Pennes, a citizen oi the United States, residingat Poca-telle, in the county oi Bannock and @tate of `ldaho, have invented certain new a d ur nl Improvements in Bankingltorms: and l do hereby declare the iollow ing, to bea full, clear, and exact description oit the linvention, such as will enable others skilled in the art to which it appertains to make and use the same.
My invention relates to means Yfor the simplification of banking` methods and consisi's oit an improved key numbered blank torre. to be used in connection with dishonored checks, or checks which are returned by a bank unpaid tor one re*L son or another.
ln order that my invention may be clearly understood, l have described the same kin the iol-,loivil speciiication, reference being had the eecompanyinp,- drawing, and .pointed out more specifically in the appended claims.
My improved form is provided with a delineated space in which there is printed a number oli possible reasons why a check, which has been presented to a bank or de positoiy .tor payment, has been dishonored, and each reason is given a key number.
There is also a space provided preferably at the top of the form, for the insertion oi the naine oic the bank using; the form, and a dating space, while above the space in which are contained the reasons ior thereturn of the check unhonored, there is preferably printed the legend YWe charge your account and return herewith unpaid and immediately below the same the words Key to reason tor return. Y
Below the list of the keyed reasons for return, is a delineated space 'for the insertion of the name oi the person or concern to whom the return is directed.

In the drawing, A represents a space in which appear the possible reasons for the return of a check, each reason having opposite it a number, the first reason being numbored 1, .for example, the second reason, 2,.
the third reason 3, and so on, the space to contain as many reasons for non-payment as a bank may have. ln the present instance l have given vreason No. l, as Account closed; reason No. 2, Garnisheed, reason No, 3, Dated ahead etc.
ln the present instance and orvpurposes illustration l have set up twenty-one reasons and have left a blank space for a turther possible reason which may be inserted and numbered 22.
B represents a rectangular space provided with horizontal lines and being sub-divided by vertical lines into smaller spaces C, D, E, F. Above the smaller spaces C, D,.E, F respectively appear the words Drawn on, Drawn by neason, .dniount.
rlhespace C is to receive the name or naines oi' the bank or banks upon which a dishonored check or dishonored checks are drawn. 'lhe space D is for the purpose of receiving the naine or names oa person or persons by whom the dishonored checks are drawn.' The space E is for the insertion of a key number which refers back to the nurnopposite the reason for the non-payment oi the check appearing in the space A.
The space F is for the insertion of the amounts for which the `v "agious dishonored checks may have been drawn. f
G is a space for the naine ol the place where the bank usine,` the form, is located and the date. ifi is a space for receiving the naine of a bank using the form, and l a space for inserting` the name oit a person or concern to whom the form is addressed.
From the foregoing, the manner of using the form will be apparent. Let us assume that Richard Doe t Sons deposits with the Doe National Bank of Pocatello, a number of checks drawn on New York banks by various customers, for example, bv John Doe, Richard Roe, John Roe, etc. rThe Doe National Bank lefives the depositor Richard Doe & Sons, credit for the amount and mails ythe checks to the Roe National Bank or other banks in New York, which are charged with the amounts of the checks. Y
The checks are received by the Roe National Bank or other banks and are dishonored for different. reasons. Each New York bank holding` one or more of the dishonored checks, for example, the Roe National Bank, will lill out one of the forms, giving the reason 'for the non-payment of the checks and return, by inserting the key number in its proper space, E, the name of the bank on which the check is drawn in its proper space C and the name of the drawer and the amount in their proper spaces D, F. This slip, together with the dishonored check is sent to the Doe National Bank at Pocatelle.y charging to its account the returned checks.
The form is directed to the proper bank by placing' the address in the space I.
In practice, I contemplate using my improved torrn in. triplicate and preferably in three colors1 let ns say white, pink and yellow, and bind them into book 'Forni with a weakened line K provided between stubs L and the body of the white and pink forms, so that the latter may be readily detached from the book, while the yellow forni, Fig. 2 for example, has no weakened line and is adapted to remain permanently in the book.
The pink and yellow iorms are designed to be carbon copies oi' the original or white form, which has been forwarded with the dishonored returned checks, the first carbon copy or pink form to take the place ot the returned check in. the New York banks tiles to show the proper charge, and the secu ond carbon copy or yellow form to remain in the book for permanent reference.
The Doe National Bank receives the returned checks and return forms and at once fills out a torni A, Fig. 2, which is substantially identical with the torni just described, but in this case is directed and sent to Richard Doe & Sons, the dishonored checks beingr listed as heretofore set forth, and the key numbers inserted showingA the reasons for non-payment.
At the end of a month or other stated period, the pink form or first carbon copy oi A is sent to Richard Doe t:- Sons With the statement rendered by the bank, showing that Richard Doe & Sons account was debited in the amount of the returned dishonored checks. The second carbon copy or yellow torni, remains in the book as a permanent reterence.

It is also true that my invention makes it possible to use one i'orin tot' severai retur il checks, thus effecting; a savingr of labor` time and stationery.
The use oi this 'form in triplicate as described, tends to exclude the f-onnnission ot errors, and 'provides a, complete pernianent record of the transaction.
What I claim and desire to secure by Letters Patent is l. A banking' form, cinnprisingjT a bod;7 portion provided. with a space containing reasons for the non-payment of' a. check presented to a bank tor payment, each ot said reasons being provided with a k y number' and a space .tor inserting the key number which refers back to the speciic reason for the non-payment of lsaid check.
