Beit known that 1, ERNEST L. SMITH, of SaltLalre City, in the county, of Salt Lake and State of Utah,'have invented certain new and useful Improvements in Combined Passes or Franks and Record-Writing Forms, of which the following is a` specifi cation.
This Y,invention relates to improvements in forms and` devices employed` by railroads `and other carriers 1n issuing and writing separate annual or term railroad, Steamship, sleeping car'or other passes and express or telegraph franlrs, andwhich are employed in making a permanent record of such passes orfranks as issued in duplicate on card forms prescribed by the Interstate CommerceCommission to be usedfor such purposes. e i
One object of the invention is to provide in combination on a single sheet of suitable material a form of this character and a record Writing form in such a manner and so arranged that separate passesor franks may be written and duplicate record cards therefor made in earbons in a single writing operation on a typewriter or other writing device. e

1n the embodiment illustrated, a Hat rectangular sheet 1 is shown on which a rectangular-shaped pass or frank and a U- shaped record Writing form are printed and combined. The pass 2 which is an integral part of sheet 1 has its boundary indicated at 3 and is printed in the form and arrangement now generally used by carriers for passes and franks, one side edge of thepass 2 coinciding withv one side edge of the sheet `IElRNES'l Ii. ,These record cards 7 correspond in size and shapeto the site and shape of the combination sheet 1 and are numberedin duplicate according to the series numbers of the passes or franks and are "printed in a form thatwill permit carbon counterparts of the matter Written in the several spaces on the sheet 1 to -be transmitted'by the use of carbon sheet 8 directly to the record cards 7 in the conforming spaces provided thereon.
Y ,The combined pass orfrank andwriting form sheet 1 herein described is preferably made of fiber or other tough, durable, and pliable material suitable for suoli purposes, and the size and structure of this sheet `1 may be varied toeither increase or "decrease the dimensions thereof to adapt it for any desiredpurpose.
TheV edges of the pass or frank portion 2 indicated by the boundary line 3 are preferably wholly or partially perforated, or are partially cut and partially perforated at the time they are printed, for convenience in separating the pass portion 2 from the remainder of the sheet, but they need not `of Ysheet 1 outside the boundary line .Bof the i e vpass'in the form of printed words shown be necessarily so formed but may be merely indicated by lines when printed to facilitate Lit sired, as shown in Fig, 2.

The pass vor frank portion 2 is then filled out in the usual manner after 'which the additional data to be shownv on the reco-rd cards is writtenV on the face of the sheet I inthe spaces opposite the printed data guides 6 outside the boundary of the pass' 2 which, by the use of the carbon paper 8, is 't1ansmitted and impressed upon the record cards 7 in the conformingspaces provided therekon'. It willbe thus observed that the writing of the pass or frankr and the recording thereof on the prescribed record ycards 7 is performedin a Vsingle operation..v
After eachV sheet l, including the pass portion Q,` has been filledy out in connection with the record cards, the pass or frank portion is detached therefrom by severing it4 along the boundary line'B, ora plurality'of-sheets l maybe first filled out in the manner'above described and then assembled in superposed relation, and the pass or Vfrank portions 2 ythereof cut out along their boundaries by a suitable cutting machine in the usual manner. The pass or frankportion Q is preserved and used for the purpose intended land theremainder of sheet l having served its purpose of providing means for placing the pass or frank'portion of the combined sheet and' form in position for filling out and recording the matter written thereon in carbon onthe record cards, is discarded and destroyed.' l
The record cards 7, any desired number of "ponies of this patent maybe obtained for which may be employed to meet the requirements of the Interstate Connnerce Commission, which are completely filled out in the same operation with the printing or fill ing out of sheet l in accordance with the requirements of the Interstate Commerce Commission are preserved intact for record purposes.
I claim A combined pass blank and record writing form vto be used in making a permanent record ofthe pass as issued in duplicate on prescribed card forms and in filling in the other data on the card forms by a single writing operation, lcomprising n flat rectangular sheet equal in' size to the size of the prescribed card forms and having` three division lines thereon, two of said lines being parallel to each other and extending transversely of the sheet from one side edge thereof to points adjacent the middle of the sheet, and being spaced equal distances from the end edges of the sheet,y the remaining division line extending longitudinally of the sheet between the transverse lines, said division lines dividing said sheets into a rectangular shaped pass blank of convenient size to fit into a pocket book and a U-shaped record writing form, both of which are free of division lines, the pass blank being bounded at itsy two ends and one side edge vby the`division lines, the remaining side edge' being a portion of the herein mentioned side edge of'said sheet, and printed data guides on the pass blank and on the record writing form, said guides being arranged to correspond to those on the aforesaid prescribed car'd forms so 'that when the former are filled in by a single writing operation the card forms will be completely filled in in duplicate.
