Abstract

Спосіб виконання дистальної резекції підшлункової залози із збереженням селезінки в дітей, при якому після мобілізації великого сальника проводять мобілізацію тіла та хвоста підшлункової залози по нижній поверхні із візуалізацією передньої поверхні верхньої брижової артерії. Далі проводять виділення гирла селезінкової артерії та селезінкової вени, останні беруть на гумові трималки відповідного кольору, проводять інтраопераційне УЗ дослідження передньої поверхні підшлункової залози із визначенням ступеня поширення пухлини на селезінкові судини при відсутності поширення пухлини на судини селезінки. Проводять подальшу мобілізацію тіла/хвоста підшлункової залози із ретельною візуалізацію та обробкою коротких артеріальних та венозних гілок, які відходять від селезінкової артерії або впадають в селезінкову вену. Всі виявлені вищезазначені короткі судинні гілки ретельно обробляють шляхом перев'язки або прошивання, при виникненні кровотечі із системи селезінкової артерії або вени проводять тимчасове перетискання селезінкової артерії або вени біля гирла до ліквідації кровотечі. Після мобілізації тіла/хвоста підшлункової залози проводять перетинання залози в зоні перешийка скальпелем, в подальшому куксу підшлункової залози зашивають по типу "рот риби" із застосуванням атравматичної монофіламентної нитки розміром 4-0. Перед зашиванням операційної рани необхідно додатково підтвердити задовільне кровопостачання селезінки шляхом відсутності зон демаркації відповідно темного кольору.Method for performing distal resection of the pancreas with preservation of the spleen in children, in which, after mobilization of the large omentum, mobilization of the body and tail of the pancreas on the lower surface with visualization of the anterior surface of the superior mesenteric artery.
