Abstract

A method for treating clotted hemothorax comprises the puncture of pleural cavity, its draining and intrapleural administration of the drugs. The puncture is preceded by local anesthesia of the thoracic tissues. The upper drainage is inserted into the pleural cavity. The puncture is performed under sonographic control following the administration of 0.9 % solution of sodium chloride in a volume of 1-1.5 mL until the end if the needle contacts the blood clot. The elastic catheter is passed according to Seldinger technique. Then the body of the patients is placed in supine position. The rarefying warm solution with 5 % of sodium bicarbonate and 1.5 % of hydrogen peroxide is administered to the clouded area. In 10-15 minutes, the body of the patient is placed vertically. The utmost lower point of the pleural cavity is assessed and the end of the tube of the lower drainage is passed to this point. The fluid content is removed from the pleural cavity. X-ray examination is repeated. The procedures stated above are performed again with puncture and injections of the rarefying solution into the remaining clouded areas until their complete removal. Finally, the pleural cavity is washed with warm 0.9 % solution of sodium chloride until clear lavage.
