Description of the invention
The invention relates to medicine, namely, to radiodiagnostics, and can be used to assess the 2nd degree of kidney parenchyma function disorders in children with urinary system pathology.
Diseases of the urinary system in the structure of diseases in children take second place after diseases of the respiratory organs. In the diagnosis of such diseases, clinical laboratory and radionuclide methods of research, which make it possible to assess the functional capacity of the kidneys and the degree of its disorders, occupy a leading place.
Відомій спосіб оцінки функціонального стану нирок (1), що включає застосування динамічної 70 реносцинтиграфії з загальновідомим радіофармпрепаратом УУп"То-ДТПО (9пТо- диетилентриамінопентаоцтом).There is a known method of assessing the functional state of the kidneys (1), which includes the use of dynamic 70 renoscintigraphy with the well-known radiopharmaceutical UUp"To-DTPO (9pTo-diethylenetriaminopentaacetate).
The disadvantages of the method are that the radiopharmaceutical used in the method allows determining the amount of functioning parenchyma based on the parameters of the speed of filtration processes, which is not always possible in the presence of obstruction and significant impairment of kidney function. A disadvantage is obtaining a planar image of the kidneys in one projection, which makes it impossible to assess the functional activity of different areas of the kidney parenchyma. There is also a well-known method of assessing the amount of functioning parenchyma (2), which we took as a prototype, which includes nephroscintigraphy with the use of a radiopharmaceutical for long-term fixation in the kidneys with 9tto-DMSO (ZtTeo-dimethylene succinatocet) in a static mode.
The disadvantages of the method are the impossibility of assessing the functional capacity of the kidney parenchyma on the basis of only a static study and the duration of the patient's stay for four hours in the diagnostic department, which significantly complicates the study in children.
The invention is based on the task of improving the method of assessing the degree of kidney parenchymal function disorders in children with urinary system pathology by using the radiopharmaceutical p"To-DMSO of long-term fixation in the kidneys for dynamic and static nephroscintigraphy. Dynamic nephroscintigraphy with 25 is performed during the first 30 minutes from the moment administration: radiopharmaceutical, static - after 180 minutes. Static nephroscintigraphy after 180 minutes is performed in four projections: front, back, right and left lateral. The state of kidney parenchyma disorders is assessed by dynamic (blood half-life time, percentage of accumulation of radiopharmaceutical in the bladder, coefficient of differential absorption: by its liver at the 30th minute of research) and static renoscentigrams (percentage of accumulation of a radiopharmaceutical in the kidneys after 180 minutes and the coefficient of its differential absorption by the liver at the 180th minute). The application of the method makes it possible to obtain a complex parameters for assessing the condition of the kidney parenchyma, which significantly increases the effectiveness of the study and the reliability of the obtained results (Ga") compared to static scintigraphy in 84.09 cases.
The task is solved by the fact that the method of assessing kidney parenchymal function disorders in children with pathology of the urinary system, which includes nephroscintigraphy with the use of: radiopharmaceutical 97 To-DMSO, according to the invention, nephroscintigraphy is performed dynamically during the first 30 minutes of administration of the radiopharmaceutical and in in a static mode after 180 minutes and with time indicators of semi-purification of blood of 40-50 minutes, accumulation of radiopharmaceutical in the urinary bladder from 0.8 to 1.296, coefficient of differential absorption of the liver at the 30th minute from 0.14 to O0.18 µm. unit and on the 180th - from 0.08 to 0.12 µm. unit and 40 accumulation of the radiopharmaceutical in the kidney after 180 minutes from 5 to 895 - is considered a moderate violation - with the function of the kidney parenchyma, and with time indicators of half-purification of the blood more than 50 minutes, the accumulation of a radiopharmaceutical in the urinary bladder is more than 1.595, the coefficient of differential absorption of the liver by 30- and "» minute more than 0.20 um. units and on the 180th - more than 0.14 um. units and the accumulation of radiopharmaceutical in the kidney after 180 minutes is less than 595 - is considered a significant impairment of the function of the kidney parenchyma. 45 Method of assessing the degree of impairment of function renal parenchyma in children with urinary system pathology is carried out as follows: after determining the activity of the radiopharmaceutical 97 To-DIMSO at the rate of 1.85 MB k per kg of the patient's body weight and clarifying its activity in the syringe, perform nephro-scintigraphy in dynamics in the patient's position on the back for 30 minutes with recording of information on a planar gamma camera immediately after the intravenous injection of 997 To-DMSO with a frequency of 1 frame per 30 seconds (6 0 frames). The information is recorded on the g 20 computer of the gamma camera. Next, on the combined frame of the dynamic renoscintigram, "zones of interest" are selected: the heart, right and left kidneys, bladder, liver, and background activity. During computer processing of the results of the study: six curves are obtained that characterize the dynamics of the distribution of the radiopharmaceutical in the selected zones. These are the heart curve, renograms of the right and left kidneys, the curve of the radiopharmaceutical entering the bladder, the curve over the liver area, and the background curve. 55 Based on the heart curve, the time of half-purification of the blood from the radiopharmaceutical is calculated according to the formula:
Ф! F! T1/2 blood - 1p(0.5)s, where
Т1/2 крові - час напівочищення крові від радіофармпрепарату, о 11(0,5) - натуральний логарифм 0,5, який дорівнює 0,693, 2 Тп(Ав - Афон) / (Азо - Афон) / (30 - 18), 60 Арон - фонова активність (імп/хв..),T1/2 of blood - the time of half-purification of the blood from the radiopharmaceutical, at 11(0.5) - the natural logarithm of 0.5, which is equal to 0.693, 2 Tp(Av - Afon) / (Azo - Afon) / (30 - 18), 60 Aron - background activity (imp/min..),
Av - activity over the area of the heart at the 18th minute of the study (bpm/min..),
Azo - activity over the area of the heart for 30 minutes of the study (bpm/min..).
According to the curve of activity over the urinary bladder, the percentage of radiopharmaceutical accumulation in 30 minutes of the study is determined. because the following parameters are calculated according to renograms:

The assessment of the kidney parenchyma function is carried out taking into account the inclusion of the radiopharmaceutical in the liver, which also allows for a more qualitative assessment of the half-life of the blood. Based on the pharmacokinetics of UptTo-DMSO, the blood is slowly cleared of it due to significant binding to plasma proteins (9195). After intravenous administration, UptTo-DMSO is evenly distributed in the body, but most of it is retained in the liver.
сThat is, the liver plays the role of a radiopharmaceutical depot from which it is gradually distributed to the kidneys. From the third to the fifth minute of the study, this distribution has a directly proportional dependence with a correlation coefficient of 0.7, that is, how much of the drug is removed from the liver, how much is captured by the kidneys. Ago. determination of the coefficient of differential absorption of the liver is more important than the coefficient of differential absorption of the left and right kidneys and this parameter must be evaluated when determining the function of the kidney parenchyma. with

She has been sick since she was 4 months old, when an increase in T2 of the body was noted up to 392, tubular proteinuria "sl 20 0.099g/l, leukocytes - the entire field of vision. Acute pyelonephritis, pyeloectasia was diagnosed. In the future, frequent relapses of the disease were observed up to 2-3 times a year. Chronic secondary pyelonephritis was established. To assess the condition of the kidney parenchyma and its function in the patient on December 8, 2004. dynamic and static scintigraphy with 89tTo-DMSO according to the proposed method is prescribed. Results: blood half-clearing time 38 minutes, accumulation of radiopharmaceutical in the urinary bladder 790, KDP of boys at 3 minute 0.15 units, KDP of girls At 180 minutes O, Tum. units, accumulation of zatto-DMSO after 180 minutes in the right kidney 6.595, in the left -1195, which indicates (Ф) moderate impairment of the parenchyma function of the right kidney and preservation of the parenchyma function of the left kidney at a satisfactory level. Three hours later, a static scintigraphy was performed, which indicated a decrease in the amount of functioning parenchyma in the right kidney by 4595, the distribution of the radiopharmaceutical in the right kidney is diffusely uneven, in the left kidney it is uniform, which confirms the data of the previous dynamic nephroscintigraphy in the first 30 minutes after the administration of the radiopharmaceutical. Thus, the patient was diagnosed with a moderate impairment of the functional capacity of the parenchyma of the right kidney.
І-х. . Dynamic and static scintigraphy with U97To-DMSO was carried out on November 3, 2004. according to the proposed method.
The results of the study: blood half-clearing time 52 minutes, accumulation of radiopharmaceutical in the urinary bladder 1.595, KDPdechinki at 30 minutes O0.19um.od., KDPdechinki at 180 minutes 0.15um. units, the percentage of accumulation of Z97To-DMSO after 180 minutes in the right kidney is 4.595, in the left - 12.895, which indicates a significant impairment of the parenchyma function of the right kidney and preservation of the parenchyma function of the left kidney at a satisfactory level.
Static scintigraphy showed a decrease in the number of functioning parenchyma in the right kidney by 6595, the distribution of the radiopharmaceutical in the right kidney is diffusely uneven, reduced, and uniform in the left. Thus, in 70 patients, a significant violation of the functional capacity of the parenchyma of the right kidney was determined.
Thus, the use of the proposed method allows to significantly increase the effectiveness of the research and the reliability of the results obtained in 84.0956 cases, to assess the functional state of the kidney parenchyma and the presence of disorders of the kidney parenchyma function in children in a shorter time, which reduces the patient's stay in the diagnostic department and the psycho-emotional burden on him.
