Одним з методів шунтування є спосіб Полуектова Л.В. та співавт. (2001), який пропонує при каротидній ендартеректомії встановлення пункційним способом шунту, який являє собою систему дренажних трубок. |1)One of the shunting methods is the method of Poluektov L.V. et al. Найбільш близьким аналогом способу, що заявляється, вибраним як прототип, є спосіб внутрішньопросвітнього шунтування описаний Золлингер Р.М. (1997), який пропонує, проведення шунту через 70 розріз біфуркації сонної артерії дистальним кінцем у загальну сонну артерію (ЗСА) та проксимальним кінцем у внутрішню сонну артерію (ВСА) після накладання на них затискувачів типу "бульдог" |21.The closest analogue of the proposed method, chosen as a prototype, is the method of intraluminal shunting described by Zollinger R.M. Спосіб застосовують таким чином: хворому під час ендартеректомії оголюють біфуркацію загальної сонної артерії і зовнішньої сонної артерії, виділяють та лігують верхню щитоподібну артерію (ВЩА). Після цього на -- відстані 2 см від кукси ВЩА зовнішню сонну артерію перев'язують, проксимально до біфуркації ЗСА на ЗвСсСА Га накладають резиновий джут та пересікають ЗвСА між лігатурою та джутом. Куксу ЗвСА відводять медіально і вниз, таким чином утворюють між ЗвСА та ГСА максимально можливий тупий кут. На дистальний кінець ее, стерильного поліхлорвінілового шунта діаметром 6 мм і довжиною 10 см накладають затискувач типу "москіт", джут на куксі ЗВСА послабляють, через куксу ЗвСА у просвіт ВСА проводять проксимальний кінець шунта, після цього джут знов затягають на шунті. Затискувач з дистального кінця шунту на короткий час знімають, шунт « промивають кров'ю для запобігання повітряної емболії. На відстані З см нижче біфуркації ЗСА на її латеральну З 50 поверхню накладають кисетний судинний шов (шовний матеріал Ргоіїеп 6.0), розсікають ЗСА, проводять с дистальний кінець шунта в просвіт ЗСА, кисет зав'язують, навкруги шунта проксимально на ВСА та дистально на з» ЗСА накладають резинові джути, таким чином запобігають зворотного кровотоку. Донецьк. ка Діагноз: Стеноз правої внутрішньої сонної артерії, ішемічні інсульти 1999 р., 2001 р. 18.11.02 Неврологічне обстеження: явища помірного геміпарезу ліворуч. - 18.11.02 Транскраніальне доплеровське сканування: (Те) 20 Хід загальної сонної артерії (ЗСА) відносно рівний. Стеноз обох ВСА: 7095 праворуч (0), 3595 ліворуч (5), бляшки гетерогенні з рівною поверховістю. Прискорений кровоток праворуч до 130 см/с. Знижений кровоток по їз обох задніх мозкових артеріях, пікова систолічна швидкість (Р5) 0-48 см/с, 5-56 см/с. По середній мозковій артерії (СМА) РОС 0-90 см/с, 5-87 см/с. Проба Матасса: при перетискуванні справа кровоток в СМА знижувався до 36 см/с, за відновленням підвищувався до 93,7 см/с. 29 26.11.02 операція: ендартеректомія з ВСА, біфуркації ЗСА.
