Вдосконалення методів мікрохірургічного лікування хворих із птеригіуумом є актуальною проблемою офтальмології у зв'язку з тим, що клінічний досвід свідчить про високий відсоток (до 5,295) рецидивів та неможливість повноцінної адаптації кон'юнктиви на місці видаленого птеригіума .Improvement of methods of microsurgical treatment of patients with pterygium is an urgent problem of ophthalmology due to the fact that clinical experience shows a high percentage (up to 5.295) of relapses and the impossibility of full adaptation of the conjunctiva at the site of the removed pterygium.
Птеригіум - достатньо поширене дистрофічне захворювання кон'юнктиви, яке відноситься до пухлин фіброзної тканини, що представляє собою як функціональний так і косметичний недолік. Основою розвитку птеригіума є вогнища еластоїдної дегенерації колагена в кон'юнктиві з вираженою ангіопатією судин. Вогнища еластоза - імунні васкуліти, локалізовані поверхнево. Імунні комплекси переносяться по системі анастомозів, палісад та термінальних аркад в глибокі шари; формуються на стінках венул, визиваючи пошкодження структурного та метаболічного гемостаза в інтактних тканинах, що сприяє розповсюдженню та поглибленню процесу.Pterygium is a fairly common dystrophic disease of the conjunctiva, which refers to tumors of fibrous tissue, which is both a functional and a cosmetic defect. Задача, яку вирішує спосіб полягає в розробці оптимального підходу до оперативного лікування птеригіума.The problem solved by the method is to develop an optimal approach to the operative treatment of pterygium.
Поставлена задача досягається тим, що у відомому способі хірургічного лікування птеригіуму, що включає гідродісекцію, резекцію птеригіуму, діатермокоагуляцію епісклеральних судин, шліфування рогівки наконечником з алмазним напилюванням, адаптацію кон'юнктиви згідно корисної моделі після шліфування рогівки викроюють трикутної форми клапоть кон'юнктиви з попереднім накладенням на його вершини трьох швів, проводять пластику кон'юнктиви шляхом імплантації клаптя кон'юнктиви в зоні видаленого птеригіума.The set task is achieved by the fact that in the known method of surgical treatment of pterygium, which includes hydrodissection, resection of pterygium, diathermocoagulation of episcleral vessels, grinding of the cornea with a tip with diamond filing, adaptation of the conjunctiva according to a useful model, after grinding of the cornea, a triangular-shaped flap of the conjunctiva is cut out with the previous applying three sutures on its tops, conjunctival plastic surgery is performed by implanting a conjunctival flap in the area of the removed pterygium.

Для порівняння двох методів було прооперовано 50 хворих (50 очей), що були розділені на дві групи. І група - хворих (25 очей) - прооперовані методом - прототипом (птеригіум ІЇ ступеня - 10 хворих (4095), І ступеня - 15ворих (60905). ІІ група - 25хворих(25очей) - прооперовані заявленим нами способом мікрохірургічного лікування птеригіуму (птеригіум І ступеня -Зхворих (32905), ПІ ступеня - 17 хворих (6895).Спостереження проводилося протягом трьох років кожні три місяці.To compare the two methods, 50 patients (50 eyes) were operated on, who were divided into two groups. В І групі повноцінна адаптація кон'юнктиви спостерігалась у 15 хворих, при цьому птеригіум ІІ ступеня був у 8 хворих (3290), ІІІ ступеня у 7 хворих (2895). У 10 хворих (4095) на 2-3 день спостерігалось розходження швів .In the 1st group, complete adaptation of the conjunctiva was observed in 15 patients, while pterygium of the 2nd degree was in 8 patients (3290), pterygium of the 3rd degree in 7 patients (2895). В ІІ групі повноцінна адаптація кон'юнктиви спостерігалась у всіх хворих. Рецидив птеригіума відмічався у 2 випадках (895) через 2,5 роки після операції.In the II group, full adaptation of the conjunctiva was observed in all patients. Recurrence of pterygium was noted in 2 cases (895) 2.5 years after surgery.
Таким чином, запропонований спосіб мікрохірургічного лікування птеригіума дозволяє досягти повноцінну адаптацію кон'юнктиви в більшій кількості випадків, та знизити кількість рецидивів.Thus, the proposed method of microsurgical treatment of pterygium allows to achieve full adaptation of the conjunctiva in more cases and to reduce the number of relapses.
Розроблений спосіб був апробований на базі Центру ендокринно-судинної патології органа зору м.Києва.The developed method was tested on the basis of the Center for Endocrine-Vascular Pathology of the Organ of Vision in Kyiv.
Отримані результати дозволяють рекомендувати його для широкого впровадження в практику.The obtained results allow us to recommend it for wide implementation in practice.
