Description of the invention
The application relates to the field of medicine, in particular to oncology and gynecology. 2 The problem of treatment of cervical intraepithelial neoplasia (CIN) remains relevant in clinical oncogynecology, since timely and adequate therapy of this pathology is a prevention of cervical cancer.
(8).In world practice, various methods of treating cervical intraepithelial neoplasia (CIN) have been developed, but not one of them has 10095 effectiveness. Most modern treatment regimens are based on the destruction of 70 affected tissue using electrosurgical excision, laser or cryodestruction, which does not ensure full recovery of patients. Half of the patients have relapses of the disease in the first six months to a year after treatment. There are no possibilities for repeated organ-preserving surgical manipulations. Recent studies have revealed that one of the etiological factors of CIN is infection with the human papilloma virus, especially types 16 and 18 of a high degree of oncogenic risk (2,3,5). The presence of human papillomavirus DNA was detected during 12 examinations of patients with relapses of CIN. This indicates that traditional methods of treatment do not ensure complete elimination of the virus from the body (8).
| ЗThere is no specific therapy for human papillomavirus infection. In recent years, much attention has been paid to the use of various immunomodulatory drugs, in particular interferons, in the treatment of CIN (1). The majority of scientific works and publications are devoted to the use of interferon preparations for the treatment of anogenital condylomas. 20 Sgovv S et al. (10) used interferon gel for the treatment of patients with acute condylomas (AC). After cryotherapy or electrosurgical excision of HC in 120 patients, 5 times a day for 4 weeks, a gel containing recombinant B-interferon 0.15 IU/g (Tgr.), 1.0 IU/g recombinant B-interferon (2 g.) was applied to the affected area. or placebo (Zgr.). After 24 months, the frequency of relapses in 1, 2, C was 19/35 (5496), 21/34 (62905), 27/36 (75905), respectively. 25 Mopzepedo.) and co-authors (11) used recombinant c, -interferon for the treatment of 25 patients with GC "intratumorally at a dose of tTmln IU/day, the course of treatment was 22 days. Agsope K et al. (13) evaluated the combined treatment of cervical condylomas with interleukin-2 and o-interferon in 25 women. The drugs were administered intravenously, the course of treatment lasted 6 weeks. Complete clinical recovery was achieved in 14 cases, partial in 9 cases, and in 2 cases the treatment was ineffective. | WITH
Many authors are developing different methods and modes of administration of the drug. Thus, Voppe MU and co-authors (7) conducted a comparative study of two modes of parenteral administration of interferon combined with cryotherapy for the treatment of anogenital condylomas. 49 patients were divided into two groups and received subcutaneous injections of g million IU/m2 (1g) or 4 million IU/m2 (2g) of interferon Z times a week for 35 weeks. No statistically significant differences were found between patients of groups 1 and 2 in the frequency of complete disappearance of HC, but meanwhile, the frequency of new lesions in patients of group 2 was significantly lower than in patients of group 1.
The prototype of the invention can be considered the clinical studies of Repp S. on the study of the effectiveness of intratumoral use of beta-interferon in the treatment of CIN associated with papillomavirus "20 infection (Repp S, Rayapi MO, Sogaidiapi K, ei ai. -v ipigaeriipeYia! peoriazia azvosiaFeuy my pitap rariShotamigiz iptesiop. /lctogi 1994 Arg 30,80 (2) p. 146-150.). Researchers conducted interferon therapy for 41 patients with CIN I-II associated with papillomavirus infection. At the same time, interferon was injected intratumorally in a dose of Zmln MO every day during the first week, 45 and three times a week during the 2nd and 3rd weeks of treatment. The course dose was ZZmln IU. sl The positive aspect of the prototype is that the use of the interferon drug contributes to the improvement of the immediate and long-term results of the treatment of patients with CIN associated with papillomavirus infection. eat) The disadvantage of the prototype is that interferon drugs are administered only intratumorally and are not used systemically. Patients with severe dysplasia of the cervical epithelium (CIN I) were not treated. 50 It should be emphasized that treatment with B-interferon drugs is much more expensive than the use of o-interferons. The invention is based on the task of creating a method of treating precancerous pathology of the cervix,
T" caused by human papilloma virus infection, by intratumoral and systemic administration of the drug o2B-interferon (If necessary, the treatment of patients with CIN PP-I is supplemented by performing diathermoconization or diathermocoagulation of the cervix), which will improve the immediate and distant results of treatment.
The task was solved as follows. Treatment was given in an outpatient setting
R" of the gynecological office. The cervix was removed in mirrors, a solution of the drug "2LB-interferon" was injected under the mucous membrane of the cervix with the help of an insulin syringe, with a needle thickness of 0.3-3 mm. This made it possible to reduce the trauma and pain of the manipulation. The o2B-interferon drug was administered in a regimen of 60 IOmln MO intracervical submucosal at Tmln every other day and ZOmln MO intramuscularly at Zmln daily.
Patients with mild and moderately expressed dysplasia (GRADES I-II) underwent 2 such cycles with an interval of weeks. Patients with severe dysplasia and intraepithelial cancer (HER PRICE) - 3 cycles with an interval of 5 weeks, under cytological and, if necessary, histological control, after which diathermoconization of the cervix was performed after 4 weeks using the ZN-57m device. 65 55 patients with CIN I-II were treated according to the developed method.
The criteria for the effectiveness of the used technique were: normalization of the colposcopic picture, cytological smears, negative polymerase chain reaction for papillomavirus. All studies were conducted before and after the end of treatment, after 1, 3, 6, 12 months. The observation period is from 1 to 3 years.
As a result of the treatment, cytological and colposcopic normalization was noted in 9490 (52/55)
