Abstract
Description
The invention relates to medicine, in particular to a method of treating burns and other skin damage.
The ultimate goal of burn treatment is their faster spontaneous healing in case of superficial lesions or faster surgical restoration of the lost skin in case of deep lesions.
One of the most frequent complications accompanying burns is general intoxication of the body, which is known as burn disease.
The wound surface is a kind of "window" through which pathogenic microflora penetrates the body. The greater the surface and depth of the burn lesion, the longer and more intensive the injection therapy, the purpose of which is to maintain the circulation of the fluid volume and the functioning of the microvascular blood flow at a normal level.
Injection therapy is combined with local treatment, the duration and overall end of the patient's treatment depends on their effectiveness.
The main goal of local treatment is to create conditions around and on the wound surface that actively suppress the pathogenicity of the wound substrate and exclude new portions of pathogenic microflora from the external environment from entering the wound, which ensures faster mummification of necrosis, reduction of the inflammatory process, and epithelization of the skin.
The success of the treatment largely depends on the effectiveness of antibacterial therapy, the purpose of which is to reduce the microbial flora in the wound surface as much as possible, in which case it is possible to count on protection against the generalization of the infection and engraftment of the autodermal graft.
- С. 74)There is a known method of local treatment of the wound surface, which includes treating the wound surface with an antiseptic and applying a wet-drying bandage (Seventh Scientific Conference on the Problem of "Burns", April 7-8, 1981, Leningrad, 1981. - p. 74)
The disadvantage of the known method is that for antibacterial treatment of the wound surface, it is necessary to periodically change the bandage. Each dressing inevitably injures the wound surface, which is very painful for the patient, and also opens access to the wound microflora from the external environment, which increases the healing time of the wound surface and the duration of the course of treatment.
The basis of the proposed invention is the task of creating such a method of treating the wound surface, the use of which allows you to shorten the healing time of the wound surface, and thereby reduce the duration of the treatment course.
The task is solved by the fact that in the method of local treatment of the wound surface, which includes treating the wound surface with an antiseptic and applying a wet-drying bandage, according to the invention, the bandage is periodically irrigated with a medicinal solution. This provides access to the wound of medicines without a bandage, which eliminates the possibility of injury and reduces the threat of new portions of microbes from the external environment entering the wound, which in turn allows to shorten the healing time of the wound surface and reduces the duration of the treatment course.
The use of a mixture of an antiseptic, an analgesic and an antibiotic diluted in a solvent for the preparation of a medicinal solution allows you to introduce the specified drugs into the body through the wound surface, excluding the formation of an infiltrate, the long-term use of which leads to a decrease in the effectiveness of therapy and increases the duration of the treatment course
In the following, the invention is explained by a detailed description of its implementation.
The wound surface is treated with an antiseptic, for example, a solution of iodinol, and a wet-drying bandage is applied. The bandage can consist of a layer of sterile gauze, an average absorbent layer of non-woven material and ordinary medical gauze. A layer of sterile gauze is impregnated with a medicinal solution.
The bandage is periodically irrigated with a medicinal solution, for the preparation of which a solvent is used, to which an antiseptic, analgesic and/or antibiotic diluted in a neutral solvent, which can be used pyrogenic water or physiological saline, are added.
The medicinal solution delivers to the wound surface a medicinal preparation that suppresses the pathogenicity of the wound substrate, while the injury to the healing tissue that occurs during dressing is excluded, and also reduces the threat of microflora entering the wound from the external environment. The proposed method of treatment allows creating conditions on the wound surface that are close to sterile, in which the number of microorganisms is reduced to a minimum and they do not show pathogenicity. Medicinal solution is prepared individually for each patient and its composition and concentration depend on the intensity of non-specific pathogenic secretions in the wound.
The frequency of irrigation is determined individually depending on the depth and plane of the lesion.
For PI-B degree lesions requiring staged operations, antiseptics and analgesics predominate in the composition of the medicinal solution
For lesions that do not require operative treatment, antiseptics and antibiotics predominate in the composition of the medicinal solution.
When avoiding pain syndrome, analgesics can be excluded from the composition of the medicinal solution.
