Description
Description of the invention
The useful model refers to medicine, namely, to orthopedics and traumatology and can be used 2 to restore movements in the elbow joint with persistent contractures or as an aid to maintain the limb in a certain position.
A well-known device for eliminating contractures in the elbow and knee joints, containing interconnected cuffs, a worm, two worm sectors connected by hinged sliding plates with cuffs (as. USSR Mo176040, Ab1E5/04, 1964). 70 The disadvantage of this device is the complexity of the design and the laboriousness of its manufacture, as well as unilateral fixation of the joint without taking into account the load on the medial and lateral sides of the patient's joint.
СРСРA well-known device for the development of contractures of the upper limb, which contains hinges, interconnected shoulder and forearm splints, which are rigidly connected to the corresponding sleeves, as well as screw fixators (assistant of the USSR
1/02, 1983). 12 The disadvantage of the known device is the impossibility of increasing the range of motion in the joint without the intervention of medical professionals, it is cumbersome and time-consuming to manufacture and is intended only for passive elimination of flexion contracture in the joint.
21435 Україна, АЄ18В17/56, 1994р.|.The closest in essence and achievable result to the proposed technical solution is a device for the independent active development of contractures in the knee and elbow joints, containing hinged shoulder and forearm splints connected to each other by means of a redress mechanism, which are rigidly connected to corresponding shoulder and forearm sleeves (Pat. 21435 Ukraine, АЕ18В17/56, 1994).
The redress mechanism here is made in the form of two parallel, spaced threaded rods, hinged using L-shaped plates with sleeves of the shoulder and forearm and connected to each other by a metal coupling, in which there are through holes for the specified rods. The latter are installed in the holes of the coupling with the possibility of moving relative to each other with the help of running nuts that interact with one or the other on the side surface of the coupling.
The disadvantage of this device is that its redress mechanism creates only those forces that act on the shoulder and forearm opposite to each other, and therefore this device is only suitable for eliminating extensor contractures of the elbow joint, while it cannot be used to eliminate flexion contractures , that is, for which forces acting in opposite directions are required. The presence of one "redressing" mechanism, located on one side of the limb, creates a rotational moment due to its mass and acting forces, which leads to the rotation of the sleeves relative to the segments of the limb and the emergence of local pressure on the limb. In turn, this causes a violation of blood circulation in the limb, pain and the likelihood of neurological disorders, which negatively affects the comfort and reliability of using the device. In addition, manipulation of nuts during redressing (unscrewing and tightening of nuts with a wrench) creates significant inconvenience when using this device.
The task of this useful model is to create an express orthosis for the active development of contractures of the elbow joint, which ensures the elimination of both flexion and extension contractures by creating forces "that act on the shoulder and forearm both opposite to each other and opposite, excludes the use of walking From the nuts, and therefore the manipulation with them, and prevents the rotation of the sleeves of the shoulder and forearm during redressing and, thus, increases its functionality, reliability and ease of use. c" The task is solved by the fact that the express orthosis for the active development of contractures of the elbow joint, which contains hinged joints connected to each other in the projection of the axis of rotation of the elbow joint, shoulder and forearm splints, which are rigidly connected to the corresponding sleeves of the shoulder and forearm, according to with a useful model, it has on the ulnar and radial sides additional splints of the shoulder and forearm and associated mechanisms - redressing, which are located parallel to the longitudinal axis of the limb, and each of the specified mechanisms is made in 4! in the form of two levers hinged to each other and connected by the corresponding sleeves of the shoulder and forearm, fixed on brackets, connected to each other by a screw tie, one end of which is freely installed in a hole made in one of the brackets and fixed on both sides of it with fasteners elements, and the second - in the -k 70 threaded hole of the second bracket and has a head, mostly ram type, while the screw fasteners of both mechanisms are located parallel to each other in the plane of the axis of rotation of the joint. c The comparative analysis of the proposed technical solution with the known closest analogue shows that the new features in it are the following: 1. The presence of additional shoulder and forearm splints and the related redress mechanisms and their location 29 on the ulnar and radial sides of the joint in parallel longitudinal axis of the limb. c 2. Execution of each of the redress mechanisms in the form of two, hingedly connected to each other, and corresponding sleeves of the shoulder and forearm, levers fixed on brackets, connected to each other by a screw tie, one end of which is freely installed in a hole that made in one of the brackets and fixed on both sides with fasteners, and the second - in the threaded hole of the second bracket, as well as, 60 placement of screw ties of both mechanisms parallel to each other in the plane of the axis of rotation of the joint. 3. The presence of a manual head redressing mechanism on each screw screed, preferably of the ram type.
The presence of additional splints of the shoulder and forearm and, associated with them, redress mechanisms and their location on the ulnar and radial sides of the joint parallel to the longitudinal axis of the limb creates a balance of forces acting on the limb on both sides of its longitudinal axis due to the mass of redress mechanisms. This prevents 62 the occurrence of a rotational moment and rotation of the sleeves of the shoulder and forearm, and, therefore, does not create reasons for the occurrence of local pressure on the patient's limb.
Execution of each of the redress mechanisms in the form of two, hingedly connected to each other, and corresponding sleeves of the shoulder and forearm, levers fixed on brackets, connected to each other by a screw tie, one end of which is freely installed in a hole made in to one of the brackets and fixed on both sides by fasteners, and the second - in the threaded hole of the second bracket, as well as placing the screw fasteners of both mechanisms parallel to each other in the plane of the axis of rotation of the joint contributes to the creation of forces acting on the shoulder and forearm in the process of redressing as opposite to each other, and vice versa, and therefore this orthosis is suitable for the development of both flexion and extension contractures. 70 The presence of a ram-type manual head redressing mechanism on the screw clamp of each makes it possible to perform redressing of the limb both in the direction of its bending and extension by hand by the patient himself without the use of a special tool, which increases the convenience of using the orthosis.
