The claimed useful model relates to medicine, in particular to cardiac surgery and can be used in the surgical treatment of acquired heart defects.
The main reason for the unsatisfactory result of operations under artificial blood circulation in most cases is related to inadequate protection of the myocardium, which was manifested by acute cardiovascular insufficiency. In connection with this, a fundamental element at the modern stage of surgery for acquired heart defects is the implementation of adequate retrograde pharmacocold cardioplegia. Presentation of cardioplegic with a retrograde route of presentation through the 70 coronary sinus, the pressure should be within 20 mm Hg. and less. Since most of the time during the operation, the retrograde route of introduction of the cardioplegic solution is used, then the fundamental importance is attached to the control of the pressure of the delivery of the solution itself, as well as the quality of the solution, especially in cases of a wide coronary sinus, when the solution enters to a significant degree in the reverse way. Since the catheter in the coronary sinus is placed in a greater number of cases on a working heart, that is, without visual 79 control, it is of fundamental importance to accurately measure the adequacy of presentation of cardioplegia due to retrograde return.
The closest analogue of the claimed method is the method of pharmacocold cardioplegia. It is based on the fact that the control is carried out by an indirect method, without visual control due to the nozzle, which is located in the cannula itself and gives pressure data only at the entrance of the cardioplegic solution (1). This pipe is sometimes bent, in some places it is necessary to supply a larger amount of solution, therefore one cannot be fully convinced of the data of the pressure indicator of the cardioplegic solution at the entrance, especially in cases of a wide coronary sinus, when there is a high probability of backflow of the cardioplegic solution. This well-known method, chosen by us as a prototype, has its drawbacks. It does not provide data on how efficiently the cardioplegic solution passes through all branches of the coronary arteries, since it provides pressure data only at the entrance to the network of coronary arteries, but does not control the very fact of the passage of the cardioplegic solution, which is its main negative drawback. In connection with this, the additional measurement in the system of coronary branches at the exit - the source of the left coronary artery is of fundamental importance to ensure the adequacy of myocardial protection.
«-The fundamental importance for the adequacy of the assessment of myocardial protection in a wide coronary sinus is the presence of visual control of the adequacy of the passage of the cardioplegic solution and sufficient pressure at the exit - the retrograde route of delivery, that is, in the throat of the coronary arteries, especially the left one, which is better visualized. "-
--Since with inadequate protection of the myocardium (with low or very high pressure of retrograde cardioplegic solution delivery) of the heart, there is a risk of rapid destruction of myocytes and the death of the patient due to the development of heart failure, surgical correction should be carried out with a clear consideration of "I indicators of pressure in the coronary artery network both at the inlet and outlet, and there should be visual control for adequate advancement of the solution in cases of wide coronary sinus. --
One of the most important principles of adequate protection of the myocardium in case of a wide coronary sinus is a set of measures aimed at the prevention of low pressure, the adequacy of the delivery of cardioplegic solution, as well as the absence of supercritical pressure of the delivery of retrograde cardioplegic solution and thus the development of further heart failure. To do this, surgeons strive to control the pressure of retrograde delivery of the cardioplegic solution in the range of 10-20 mm Hg. due to the line in the retrograde cannula, but sometimes it does not give full control in a wide coronary sinus, and therefore there is a need for adequate control, which
With" provides confidence in the absence of the possibility of heart failure in the early postoperative period. External cooling of the heart also additionally contributes to the full protection of the myocardium.
The choice of protection of the myocardium significantly affects the technique of performing the operation. Thus, with repeated retrograde administration of the cardioplegic solution, there is no need to spend extra time keeping the cannulas in the mouths of the coronary arteries, as is the case with the antegrade route of administration. "iz" The task to be solved consists in ensuring adequate protection of the heart myocardium by changing the parameters of pressure delivery in retrograde cardioplegia, as well as in the adequacy of measurement of pressure units due to the optimization of the method of delivery of cardioplegic solution, additional direct control of its - 20 leakage from the mouth left coronary artery.
Technical result: an increase in the arsenal of surgical possibilities with a significant improvement in the immediate and long-term results of operations in the correction of aortic and mitral-aortic acquired heart defects.
The set task is achieved by the fact that in the known method of controlling the adequacy of the pressure of retrograde cardioplegia with a wide coronary sinus, which includes the stage of direct monitoring of the leakage of the cardioplegic solution 29 from the coronary sinus, according to a useful model, additional visual control of the adequacy of the supply of the cardioplegic solution through the system of the wide coronary sinus is carried out sinus, narrowing its mouth with a temporary purse-string suture, and through the mouth of the left coronary artery, control of the rise of the column of cardioplegic fluid is carried out through the inserted measuring cannula to a height of about 500 mm H2O. within 7 seconds, which indicates the adequacy of myocardial protection by the method of retrograde cardioplegia in the absence of 60 reverse leakage from the coronary sinus into the right atrium.
The method is carried out as follows.
Operative treatment for the correction of acquired valvular heart defects is performed under conditions of moderate systemic hypothermic protection. After complete heparinization of the patient, under finger control, a cardioplegic cannula is inserted through the right atrium into the coronary sinus. If, during manual control, we have a wide coronary bo sinus where the cardioplegic cannula has entered freely, this means that the right atrium should be opened additionally and the mouth of the coronary sinus temporarily narrowed to avoid backflow during the delivery of cardioplegic solution from the coronary arteries. We start artificial blood circulation. As the ascending aorta begins to clamp and the heart stops working, we open the right atrium. With a 4-0 atraumatic needle, we apply a pocket suture around the inserted cardioplegic cannula, and we take it on a temporary tourniquet, which we tighten.
We begin the retrograde way of supplying the cardioplegic solution through the coronary sinus system. We tighten the bag in such a way that there is no return wave of the cardioplegic solution from the coronary arteries. At the same time, the pressure of the delivery of the cardioplegic solution should not exceed 20 mm Hg. The aorta is opened, and the throat of the left coronary artery is controlled by eye. We insert a cannula of the appropriate size into it (about 5 mm in diameter). In its 7/0 space, a cardioplegic solution appears due to retrograde supply, which enters through the cannula to a certain height (about 500 mm Hg within 7 seconds). With large heart sizes (cardiomegaly), the rate of rise of the column will be correspondingly reduced, but it will still rise to a level of at least 500 mm H2O, which indicates the adequacy of the passage of the entire cardioplegic solution through the system of coronary arteries, that is, adequate protection of the myocardium.
The method is illustrated by a specific example of its implementation.
Example. Patient L. 53 years old, medical history Mo1175. Entered the Institute of Cardiovascular Surgery on April 27, 2005. with complaints of shortness of breath and palpitations with light physical exertion, as well as pain in the heart, swelling of the lower extremities, periodic low-grade fever.
He considers himself a patient for more than 20 years. Throughout this period, periodically occurring, gradually increasing shortness of breath, periodic attacks of palpitations were noted. A pronounced deterioration of the condition has been noted since 1997. At the same time, during an echocardiographic examination, the patient was diagnosed with rheumatic mitral-dortal heart disease. At that time, the patient refused surgery. He underwent surgical treatment only in 2005.
тDuring the examination, an auscultatory pattern simulating a combined mitral defect, atrial fibrillation, and a combined aortic defect was noted. Transesophageal echocardiography confirmed the presence of a significant defect of both heart valves, as well as the absence of damage to the coronary arteries. t
Враховуючи великі розміри серця, особисто лівого шлуночка, його гіпертрофію стінки особисту увагу звернули на захист міокарду, який «--Operative treatment was carried out to correct both valves (their replacement). The operation was performed under advanced hypothermic protection (1-27), aortic compression was 117 minutes. Considering the large size of the heart, especially the left ventricle, its wall hypertrophy, personal attention was paid to the protection of the myocardium, which "--
Zo was performed as a retrograde method of delivering a cardioplegic solution. At the same time, under finger control, a retrograde cannula was inserted into the coronary sinus before the start of artificial blood circulation, but a wide coronary sinus was diagnosed by palpation. Therefore, after clamping the aorta, the right atrium of Ge was opened"! the mouth of the coronary sinus was lined with 4-0 prolene and taken for a temporary obturator. The return flow of the cardioplegic solution is minimal. The aorta is opened by a transverse approach. In the throat of the left coronary artery
A cannula was inserted into the artery and a column of solution rose in the cannula, respectively, up to 50 mm H2O. for 7 "-- seconds, which indicates the adequacy of the pressure of the cardioplegic solution supply. Both heart valves were subsequently replaced, after which the heart cavities were sealed and cardiac activity was restored.
The postoperative period went smoothly.
A total of 57 operations were performed on patients according to the proposed useful model. In all cases, there was no manifestation of significant heart failure, which indicates the adequacy of the method. There were no complications when carrying out similar operations.
