Abstract
Method for prevention of purulent-septic complications in gynecological patients during conditionally pure laparoscopic operations on pelvic organs, without the use of antibiotics, by preoperative preparation, intra- and postoperative measures. Additionally, the leukocyte index of intoxication is determined, after which a tampon impregnated with the antiseptic Decamethoxine is placed on the cervix 30-35 minutes before the operation until the end of the surgery.
Description
The useful model belongs to the field of medicine, namely operative gynecology and obstetrics, and can be used for the prevention of purulent-septic complications (SCU) during laparoscopic operations on the organs of the small pelvis.
особливо серед жінок молодого репродуктивного віку, які у 72- 77 У5 потребують оперативного (на сучасному етапі ендоскопічного) лікування (11.Currently, gynecological diseases (uterine fibroids, endometriosis, ovarian cysts, etc.) are constantly increasing, especially among women of young reproductive age, who in 72-77 U5 need operative (at the current stage of endoscopic) treatment (11.
It is known that for the purpose of prevention of GSU, antibiotics are widely used, which are based on clinical and pharmacological effectiveness, taking into account the growing virulence of opportunistic microflora and the wide spread of pan-resistant strains of microorganisms.
However, the above-mentioned approach to the prevention of GSU during "conditionally clean" laparoscopic operations on the pelvic organs is insufficiently effective and increases the percentage of postoperative complications.
Given that most women have a violation of the microbiocenosis of the vagina, the presence of multiresistant conditionally pathogenic microflora or pathogens that lead to the development of purulent-inflammatory diseases, the development of a rational method of perioperative preventive measures in patients with gynecological pathology is an extremely urgent problem
The need for antibiotic prophylaxis depends on the category, type and characteristics of surgical interventions. The category that includes "clean" surgery has a low risk of postoperative infections.
However, at the current stage, to reduce postoperative complications, everyone, including patients belonging to the "low" risk group, is prescribed prophylactic antibacterial therapy.
Today, many patients are diagnosed with resistance to antibiotics, the emergence of multiresistant bacterial strains, which is a problem of global importance that prevents effective prevention and treatment of an ever-increasing number of infections. In this regard, the treatment of women with resistant strains of microorganisms becomes difficult and requires the use of alternative drugs or increased doses of traditional therapy, which can be more toxic, and the treatment is costly. Large-scale
The introduction of endoscopic and minimally invasive surgical interventions into clinical practice raises the question of the necessary expediency of new approaches to antibiotic prophylaxis.
И.,The closest analogue is the work of the authors Shalimov A.A., Grubnyk V.V., Tkachenko A.I.,
Г. "Infection control in surgery, which is presented as preoperative preparation - the use of antibiotics for the prevention of GSU in patients with a moderate level of contamination (conditionally clean operations), with the aim of destroying or reducing the colonization of all types of bacteria |4|.
This work has a number of disadvantages: dysbiotic disturbances; development of polyresistant strains of bacteria; "obliteration" of the clinical picture of an infectious complication with further difficulty in diagnosis; use of higher doses of antibiotics; increasing the cost of treatment.
The basis of a useful model is the task of improving the method of preventing GSU in gynecological patients during conditionally clean laparoscopic operations on the pelvic organs without the use of antibiotics by using an intraoperative tampon impregnated with an antiseptic, which provides antibacterial, antiseptic and immunocorrective effects, which contribute to the reduction or absence of GSU, better the course of postoperative rehabilitation and long-term results of operative treatment, taking into account the improvement of biochemical and microbiological indicators, reduction of hospital stay, reduction of the cost of complex rehabilitation in this category of patients and improvement of rehabilitation efficiency.
The task is solved by the fact that the method of prevention of purulent-septic complications in gynecological patients during conditionally clean laparoscopic operations on the pelvic organs, without the use of antibiotics, through preoperative preparation, intra- and postoperative measures, according to a useful model, additionally determines the leukocyte index of intoxication ( LI), after which a cervical tampon impregnated with decamethoxine antiseptic is installed 30-35 minutes before laparoscopic surgery in "clean" gynecological patients (with | and II degree of vaginal cleanliness, without clinical and bacteriological manifestations of bacterial vaginosis, without microbial and viral associations and dysbiosis), which is removed together with 60 removed pathology of the uterus.
The method is performed as follows:
Prevention of GSU consists in preoperative diagnosis, application of intra- and postoperative measures.
Prevention was carried out in 48 women with gynecological pathology (uterine fibroids, endometriosis, etc.), which required planned surgical intervention. The age of the patients ranged from 20 to 55 years. All operations were planned, performed only laparoscopically: hysterectomy and conservative myomectomy with posterior colpotomy.
Preventive antiseptic measures begin to be carried out before putting the patient under anesthesia: 30-35 minutes before the operation, a tampon impregnated with 0.02 95 solution of antiseptic Dekasan is inserted intravaginally into the cervix, which is in place from the moment of its introduction and throughout the duration of the operation. The tampon is removed during the evacuation of the uterus or myomatous node from the abdominal cavity through a colpotomy.
Післяопераційні заходи полягають у ранній руховій активності хворих, видаленні дренажу, моніторингу частоти серцевих скорочень, температури тіла, діурезу, а також моніторингу гострофазових запальних реакцій (ЛП, с-реактивний білок).Postoperative measures consist of early motor activity of patients, removal of drainage, monitoring of heart rate, body temperature, diuresis, as well as monitoring of acute-phase inflammatory reactions (LP, c-reactive protein).
The proposed method of prevention is effective in comparison with the effect of traditional antibiotic prophylaxis and is characterized by convenience and ease of use, reduction of unwanted effects on a woman's body to antibiotics, frequency and severity of GSU.
The method is confirmed by a concrete example of use.

Thus, the effectiveness of the local application of an antiseptic solution during conditionally clean gynecological operations helps to reduce the intensity of inflammatory processes, the absence of purulent-septic complications and negative factors that are most often inherent in systemic antibiotic prophylaxis, and also helps to restore the microbiocenosis of the vagina.
