Abstract
The method of complex treatment of cervical cancer includes preoperative radiation therapy, radical hysterectomy with subsequent histopathological examination, postoperative course of radiation therapy. If unfavorable prognostic factors are detected, for example, tumor emboli in blood vessels, tumor invasion of lymphatic and blood vessels (LVSI +), low degree of tumor differentiation, postoperative courses of chemotherapy are additionally performed.
Description
Корисна модель належить до медицини, а саме - до онкології, онкогінекології, і може бути використана при комплексному лікуванні операбельного раку шийки матки (РШМ).The useful model belongs to medicine, namely to oncology, oncogynecology, and can be used in the complex treatment of operable cervical cancer (RCM).
Cervical cancer (cervical cancer) occupies one of the leading places in the structure of oncological incidence among women in Russia and Ukraine, occupying the MI ranking place in the general structure of oncological incidence of women, amounting to 5,395 |Chissov V.I., 2010). Many scientific researches are carried out in the field of studying the prognostic significance of various clinical and morphological characteristics of the tumor process and their evaluation for predicting the effectiveness of treatment of RSM.
At present, the main therapeutic methods of treatment for RSM are surgical and radiation, which are used both individually and in various combinations, including chemotherapy.
There are methods of combined treatment of operable RSM, which include radiochemotherapy and surgical treatment (1) - preoperative remote radiotherapy to the small pelvis with SOD 20-30 Gy and parallel administration of the chemopreparation - cisplatin, (21 - preoperative radiotherapy to the small pelvis against the background of chemotherapy drugs of the fluoropyrimidine series followed by radical hysterectomy and postoperative remote irradiation, and when metastatic lesions of regional lymph nodes are detected, adjuvant (postoperative) PCT courses are conducted.
These methods do not allow to completely reduce the spread of tumor cells and reduce the frequency of local recurrence, increase recurrence-free survival, since not all prognostic factors are taken into account, namely the detection of tumor emboli in blood vessels, tumor invasion of lymphatic and blood vessels (I M5I same), low degree of differentiation tumors
The closest analogue of a useful model is the well-known method of treating operable forms of cervical cancer (stage IA2-II), according to which a preoperative course of radiation therapy is applied to the small pelvis to a total focal dose (SOD) of 20-30 Gy, followed by radical hysterectomy, pathohistological examination of the postoperative material "and a postoperative course of radiation therapy, taking into account the dose of preoperative radiation.
The method allows to reduce the frequency of local recurrence, but does not allow to completely reduce the dissemination of tumor cells during surgery. This method is not
Zo takes into account prognostic factors, namely: vascular embolism (detection of tumor emboli in vessels), tumor invasion of lymphatic and blood vessels (IM5I), low degree of tumor differentiation, which leads to rapid progression of the disease.
The basis of a useful model is the task of improving the method of treatment of the operable
RSHM, in which efficiency is improved by increasing the recurrence-free period and prolonging the time until the disease continues due to additional postoperative courses of polychemotherapy.
The task is solved by the fact that in the method of complex treatment of cervical cancer, which includes preoperative radiation therapy, radical hysterectomy followed by histopathological examination, postoperative course of radiation therapy, according to a useful model, when adverse prognostic factors are detected, for example, tumor emboli in vessels, invasion with a tumor of lymphatic and blood vessels (utrpomazsciagzraseiipmaziop, I M5IZH), a low degree of tumor differentiation, postoperative courses of polychemotherapy are additionally carried out.
The method is carried out as follows:
При стадії ІВ2-ПА застосовують внутрішньопорожнинну брахітерапію: РОД - 7-8The proposed method of combined treatment of operable cervical cancer IV2-II (T1r2gmomo, T2amomo, tTagrMoOMoO) includes preoperative radiation therapy, which is carried out in 2 variants. Intracavitary brachytherapy is used at IV2-PA stage: ROD - 7-8
Гр, СОД - 14-16 Гр, з подальшою операцією, яку виконують протягом 24-48 годин після опромінення (варіант 1).Gy, SOD - 14-16 Gy, with subsequent surgery, which is performed within 24-48 hours after irradiation (option 1).
When diagnosing IV stage, option 2 is used, which includes combined radiation therapy, which consists of remote to the primary tumor and regional lymph nodes ROD - 2 Gy, SOD - 30 Gy and intracavitary radiation therapy (brachytherapy) ROID 7-8-10 Gy, SOD 14 -16-20 Gr. Depending on the diagnostic and treatment equipment, the technique of remote radiation therapy is divided into conventional (A-pictures, CT scans, 20-2.50 gamma therapy device) and conformal (CT/MRI, Z0, accelerating complex with a multi-petal collimator). Radiation therapy with radiomodification is allowed. Three weeks later, an operation is performed - radical hysterectomy (IS type operation). In young patients (up to 45 years), it is possible to perform transposition of the ovaries to preserve their function with further pathohistological examination. If one of the factors of high risk of progression is identified, such as tumor emboli in vessels, tumor invasion of lymphatic and blood vessels (I1M5/Izh-), a low degree of tumor differentiation, additional courses of postoperative polychemotherapy are carried out in the postoperative period before the appointment of a postoperative course of radiotherapy.
Chemotherapy is carried out according to standard and generally accepted cisplatin-retaining regimens. In the future, postoperative combined radiation therapy is carried out taking into account the dose of preoperative radiation therapy, which consists of remote radiation therapy on the bed of the primary tumor, regional lymph nodes, ROD - 2 Gy, SOD - 40-45 Gy and intracavitary gamma therapy on brachytherapy devices (irradiate the stump of the vagina ROD - 5 Gy 2 times a week to SOD 20-25 Gy). When metastatic lesions of the regional lymph nodes are identified, chemotherapy courses are additionally conducted.
