Abstract
The method of vacuum therapy of infected soft tissue wounds in combination with local antibacterial therapy involves the formation of a vacuum bandage on the infected wound with the conduct of vacuum therapy, and antibiotic solutions are injected into the tissues around the wound and due to the action of negative pressure. wounds with antibacterial drugs in the direction from healthy tissues to the affected, which helps to avoid complications, shorten the treatment time and improve functional and cosmetic results.
Description
A useful model belongs to the field of medicine, in particular to surgery, namely to purulent-septic surgery.
There is a known method of surgical treatment of infected wounds, in which vacuum drainage is performed, for which a porous drain is placed in the surgical wound, which is connected to the vacuum and created with the help of a vacuum suction device (VAC device) in a vacuum bandage (VAC bandage ), located in the wound, a negative pressure of 75-125 millimeters of mercury.
This achieves a number of advantages compared to open wound management, namely: effective aspiration of wound exudate, stimulation of granulation growth, reduction of wound size. But the method is not without disadvantages, among which: a vacuum dressing can create conditions for the development of anaerobic infection and makes it possible to introduce medical drugs into the wound.
The closest technical solution, both in essence and in terms of the achieved result, which was chosen as the closest analogue (prototype), is a method of treating infected wounds of soft tissues, which consists in systemic antibiotic therapy and the formation of a VAK bandage, for which a polyurethane sponge is placed in the surgical wound, the wound is sealed with the help of a transparent adhesive film, which is pasted on the outer side of the polyurethane sponge and on the skin of the edges of the wound. A drainage tube with a film-applicator is installed on the VAK-bandage created in this way above the sponge, which hermetically connects this bandage with the VAK-apparatus, which ensures the effect of negative pressure on the bandage and the wound.
A significant drawback of the technical solution in the known method of treating infected wounds of soft tissues is that in the presence of infection, a dense infiltrate is formed around the wound, which damages the penetration of antibiotics to the source of infection.
The basis of the useful model is the task of eliminating the shortcomings of the prototype in order to improve the effectiveness of the treatment of infected wounds of soft tissues due to the impregnation of tissues surrounding the wound through percutaneous injections around the wound with antibiotic solutions based on the results of a microbiological study with the determination of the sensitivity of selected cultures to antimicrobial drugs. In contrast to the prototype, the injection of antibiotics into the depth of the soft tissues around the wound allows not only to wash the surface of the flat wound, but due to the effect of negative pressure to impregnate the deep layers of tissues under the wound with medicinal drugs in the direction from healthy tissues to the affected ones. In this way, the effect of antibiotics on the depth of tissues around the wound is significantly enhanced, which allows you to avoid complications in the area under the wound
VAK-bandage (development of anaerobic infection, formation of abscesses, phlegmon), shorten the period of treatment and improve functional and cosmetic results.
Thus, the method of vacuum therapy of infected wounds of soft tissues in combination with local antibacterial therapy, which is claimed, meets the criterion of the utility model "novelty".
