Description
A useful model relates to medicine, in particular to surgery, namely to a method of treating experimental acute destructive pancreatitis.
The useful model "Method of treatment of experimental acute destructive pancreatitis" is based on the task of diluting the pathological secretion and achieving an anti-inflammatory effect during the period of destructive changes, which were characterized by the inflammatory exudation of the peritoneal cavity on a macroscopic examination, which was accompanied by edema, the presence of diffuse suppuration of inflammatory tissue and local hyperemia of the peritoneum.
The task is solved by the fact that according to the method of treatment of experimental acute destructive pancreatitis, according to a useful model, experimental rabbits are injected with 10 ml of 25 95 Pulmozyme (Dornase alfa) solution using 0.9 95 Massi (2.5 ml of Dornase alfa and 7.5 ml Massi) into the duct of the PZ for three days every 24 hours.
Drug Pulmozym (Dornase alfa). Release form - in ampoules of 2.5 ml. During the experiment, one experimental rabbit was injected with 10 ml of 25 95 Dornase alfa solution using 0.995 Mass! (2.5 ml of Dornase alfa and 7.5 ml of Mas!) The drug is administered, starting from the fifth day, by draining the PZ duct.
In the course of the experimental study, 20 sexually mature, healthy rabbits of the "Grey Giant" breed, weighing from 8 to 10 kg, were selected. A control group of 5 rabbits was formed to perform a comparative analysis of the results of the experiment. Acute destructive pancreatitis was simulated on the example of 15 rabbits according to the developed method (utility model patent Mo 66667). For this, experimental animals were subjected to laparotomy under combined intravenous anesthesia. At the point where the pancreas adjoins the wall of the intestine, the pancreatic duct (PJ) was isolated and its complete ligation with a suture ligature. After that, a medical solution was injected into the parenchyma of all departments of the PZ

During the first day of the experiment, no mortality was observed in the studied rabbits.
During the seven-day observation of the escalation of disease manifestations, reduced mobility, refusal of food, vomiting, pain and rigidity of the abdominal wall were noted. On the 5th day, the model was macroscopically characterized by the presence of diffuse suppuration of the inflamed tissue, swelling of the PZ, inflammatory exudation, local hyperemia of the peritoneum, moderately swollen intestinal loops, compared to the control group. On the 8th day of the experiment, according to the examination data, an improvement in the general condition of 13 experimental animals was established, which was manifested by a moderate absence of vomiting, a moderate increase in motor activity. One experimental animal kept signs of intoxication in the form of decreased appetite, moderate weakness, rare episodes of vomiting. One krill did not respond to food and tactile stimuli, frequent episodes of vomiting were noted, the animal was periodically in a state of motor excitement.
Since the purulent exudate contains a high concentration of extracellular DNA - a viscous polyanion that accumulates as a result of the destruction of leukocytes within the inflammatory process.
According to the results of the audit of the abdominal cavity of experimental rabbits, compared to the control group, a complete response to the administration of the drug was observed in 60 95 (9 experimental animals), as macroscopically it was characterized by the absence of diffuse suppuration of the inflamed tissue, the disappearance of swelling of the peritoneum, inflammatory exudation, local hyperemia of the peritoneum and swelling of the loops intestines Partial reaction to the use of the drug in 26595 (4 animals), which was characterized by a decrease in inflammatory exudation of the gastrointestinal tract tissues and its diffuse suppuration, a moderate disappearance of peritoneum hyperemia and swelling of the intestinal loops. Stabilization of the condition in 7 95 (1 animal), as the macroscopic examination revealed inflammatory exudation of the PZ, which was accompanied by edema, the presence of diffuse suppuration of inflammatory tissue and local hyperemia of the peritoneum. Progression of the disease in 7905 (1 krill), which was characterized by destructive lesions of the peritoneum, a moderate amount of hemorrhagic peritoneal exudate, necrotic foci of the parenchyma, pronounced edema of the omentum, hyperemia of the peritoneum, moderate distention of the small and large intestines.
In the process of analyzing the activity of pancreatic enzymes at different stages of pancreatitis development, it was found that 24 hours after the initiation of pancreatitis, the activity of venous blood lipase increased by 3.6 times, trypsin - by 1.4 times, and alpha-amylase - by 1.5 times.
In the course of the development of the initial necrotic lesion of the PZ (2 days) and its further progression (3-5 days), the activity of PZ enzymes significantly decreased. Thus, on the 5th day of the course of experimental pancreatitis (compared to the first day), the activity of lipase, trypsin, and amylase decreased by 6.0, 1.5, and 2.1 times, respectively. Similar dynamics were established when determining the activity of urinary alpha-amylase (diastase). The indicators on the 1st and 2nd day from the beginning of the simulation of the process, compared with the control by 2.2 and 3.8 times, respectively, significantly increased with a consecutive decrease on the 5th day compared to the 2nd day by almost 10 times.
The experiment was conducted in accordance with generally accepted international norms for conducting research in the field of biology and medicine, namely the Vancouver Convention on Biomedical Research (1994) and legislative acts defined by the Constitution of Ukraine.
The advantages of the proposed useful model are the use of a genetically engineered analog of a natural human enzyme, which does not cause an allergic process in the studied groups, effectively cleaves extracellular DNA by hydrolysis, which leads to the dilution of pathological secretions, and exhibits anti-inflammatory properties in the form of a reduction in edematous and hyperemic reactions.
This useful model proves the possibility of an effective effect of the genetically engineered variant of the human natural enzyme Dornase alpha on the pathological secretion of PZ by hydrolysis of extracellular DNA molecules, which is confirmed by the data of an experimental study, compression processing and biochemical studies.
