Виразкова хвороба дванадцятипалої кишки (ВХДІК) - одне із найтяжчих захворювань органів травлення в дітей. Результати лікування та профілактики ВХДПК недостатньо задовільні, незважаючи на синтез нових поколінь ліків, розробку і впровадження нових схем і підходів, включаючи еррадикацію Н. руїогі. На жаль сьогодні практичні лікарі все частіше реєструють випадки резистентності Н. руїогі до відомих антибактеріальних препаратів, тому активно розробляються нові схеми лікування з урахуванням всіх ланок патогенезу. Одним із істотних факторів ульцерогенезу є судинно-ендотеліальна дисфункція, внаслідок якої порушується мікроциркуляція з утворенням трофічних порушень, зниження кровотоку слизової оболонки шлунка (СОШ) та ДПК. Як правило, локальне зниження регіонарного кровотоку, розлад мікроциркуляції, що супроводжується розвитком тканинної гіпоксії, призводять до порушень регенерації СОШ з подальшою активізацією катаболічних процесів і переважанням апоптозу. Ці ж порушення зумовлюють розвиток ускладненого перебігу ВХДПК. Більшість схем лікування ВХДІПК спрямовані тільки на ерадикацію гелікобактеріозу. Враховуючи зростання поширеності ВХДІПК серед дитячого населення та резистентність до традиційних лікарських засобів, є необхідним включення в комплексне лікування патогенетично обгрунтованих медикаментозних засобів. Перспективним є включення в лікувальні схеми препаратів донаторів монооксиду нітрогену (МО).Duodenal ulcer disease (DUOD) is one of the most serious diseases of the digestive organs in children. The results of treatment and prevention of VHDPK are insufficiently satisfactory, despite the synthesis of new generations of drugs, the development and implementation of new schemes and approaches, including the eradication of N. ruiogi. Unfortunately, today, practical doctors are increasingly registering cases of N. ruiogi resistance to known antibacterial drugs, therefore, new treatment regimens are actively being developed, taking into account all links of pathogenesis. One of the essential factors of ulcerogenesis is vascular-endothelial dysfunction, as a result of which the microcirculation is disturbed with the formation of trophic disorders, a decrease in the blood flow of the gastric mucosa (SOS) and DPK. As a rule, a local decrease in regional blood flow, a microcirculation disorder, accompanied by the development of tissue hypoxia, lead to impaired regeneration of the SOH with further activation of catabolic processes and the predominance of apoptosis. These same violations lead to the development of a complicated course of VHDPK. Most treatment regimens for VKHDIPK are aimed only at the eradication of helicobacteriosis. Taking into account the growing prevalence of VKHDIPC among the children's population and resistance to traditional medicines, it is necessary to include pathogenetically justified medicinal products in the complex treatment. The inclusion of nitric oxide (NO) donors in treatment regimens is promising.

Після застосування "потрійної" терапії (амоксицилін ж кларитроміцин ж де-нол впродовж 7 днів) та препарату тівортін впродовж двох тижнів, на відміну від використання прототипу, у обстежених дітей визначалося покращення загального стану, зникав больовий та інтоксикаційний синдроми, прискорювалися процеси репарації слизової оболонки при ендоскопічному дослідженні. Клінічний приклад. Хворий, Х. Вік - 15 років. Хворіє впродовж З років. Рецидиви виразки 2-3 рази на рік (весна, осінь). Діагноз: Виразка цибулини ДПК, рецидивуючий перебіг. Хронічний гастродуоденіт (Н. руїогі -), помірна гіперацидність. Скаржиться на біль та тяжкість у пілородуоденальній ділянці, частий "нічний біль", нудоту, печію, головний біль, запаморочення, закрепи. Рівень МО» 7,2 мкмоль/л, МОз 11,4 мкмоль/л, МО2-МОз, 17,0 мкмоль/л. Призначено лікування: амоксицилін 500мг двічі на добу, кларитроміцин 250 мг двічі на добу, де-нол по 240 мг З рази на добу за 30 хв до прийому їжі впродовж 7 днів, тівортін по 5 мл З рази на добу під час прийому їжі впродовж 2 тижнів.
