Abstract
Спосіб ендоскопічної поліпектомії, при якому накладають на основу поліпа поліпектомічну петлю, яку заводять через біопсійний канал ендоскопа. Використовують ендоскоп з двома інструментальними каналами, а поліпектомічну петлю накладають на основу поліпа після фіксації останнього щипцями, заведеними через петлю.A method of endoscopic polypectomy, in which a polypectomy loop is applied to the base of the polyp, which is wound through a biopsy channel of the endoscope. Торкається зазначене і досягнень ендоскопії, які на сучасному етапі розвитку медицини перетворили ендоскопічну галузь з банальної діагностичної процедури на потужний лікувальний засіб проведення малоінвазивних хірургічних втручань. Так, все частіше ендоскопічні методи лікування доброякісних та злоякісних пухлин, виразкової хвороби шлунку та дванадцятипалої кишки, шлунково-кишкових та легеневих кровотеч, холедохолітіазу, сторонніх тіл шлунково-кишкового тракту та трахеобронхіального дерева та іншої патології стали заміщати традиційні методики відкритих оперативних втручань (1-4).Scientific and technical progress is definitely reflected in the development of medical technologies. Thus, increasingly, endoscopic methods of treatment of benign and malignant tumors, peptic ulcer disease of the stomach and duodenum, gastrointestinal and pulmonary bleeding, choledocholithiasis, foreign bodies of the gastrointestinal tract and tracheobronchial tree, and other pathologies began to replace traditional methods of open surgical interventions (1- 4).
Одним із основних методів лікування поліпів стравоходу, шлунка та товстої кишки є ендоскопічна поліпектомія. Остання виконується під час ендоскопічного дослідження за допомогою поліпектомічної петлі, що затягується на основі поліпа. Ефект пересічення тканин здійснюється струмом високої частоти, який генерується електрохірургічним блоком та подається на активний електрод у вигляді поліпектомічної петлі. Загальновідомими є технічні труднощі при виконанні ендоскопічної поліпектомії, що можуть принципово впливати на тривалість процедури, ймовірність виникнення ускладнень та результати лікування хворого в цілому. До зазначених труднощів слід віднести важкість швидкого захоплення поліпа, особливо великих розмірів, поліпектомічною петлею та затягування останньої саме у визначеному лікарем місці на основі патологічного новоутворення. В подальшому, після відсічення поліпа, виникають технічні труднощі при захопленні видаленого новоутворення з метою його екстракції із просвіту органа для патогістологічного дослідження (1-4).One of the main methods of treatment for polyps of the esophagus, stomach and colon is endoscopic polypectomy. The mentioned difficulties include the difficulty of quickly capturing a polyp, especially of large size, with a polypectomy loop and tightening the latter precisely in the place determined by the doctor based on the pathological neoplasm. Later, after cutting off the polyp, technical difficulties arise when capturing the removed neoplasm in order to extract it from the lumen of the organ for histopathological examination (1-4).
Найбільш близьким до заявленого способу є спосіб ендоскопічної поліпектомії, який виконується наступним чином: під час ендоскопічного дослідження через інструментальний канал ендоскопу до просвіту органа заводиться і розкривається поліпектомічна петля. В подальшому, поліп захвачується петлею, а остання затягується на визначеному рівні основи новоутворення. На петлю подається струм високої частоти, що при поступовому подальшому затягуванні петлі дає можливість безпечно пересікти тканини та, відповідно, відсікти поліп.The closest to the claimed method is the method of endoscopic polypectomy, which is performed as follows: during endoscopic examination, a polypectomy loop is introduced and opened through the instrumental channel of the endoscope to the lumen of the organ. В ході ендоскопічного дослідження двоканальний ендоскоп заведений до рівня новоутворення. Через просвіт петлі проведені щипці з подальшим захопленням новоутворення. По щипцям на поліп накинута петля з затягуванням останньої на рівні середньої третини ніжки. В подальшому виконана типова поліпектомія. Після відсічення поліпа, останній враховуючи фіксацію щипцями, відразу після завершення процедури видалений з просвіту кишки разом з ендоскопом.
