Description
本發明屬於一種椎體固定件,尤其主要涉及一種具階段式撐張,有效分散植入施力,及具高度調整功能,提供植入手術更具省力、快速精準不滑脫之椎體固定件創新發明者。
按,人體的脊椎易因年齡、意外、運動、長期姿勢不良或職業傷害等外力因素,使脊椎出現錯位、軟骨退化與骨質增生,尤其是年老退化性脊椎病變,嚴重者甚會造成身體其他器官功能失調等併發疾病發生,而為解決脊椎因年老退化病變或其他外力造成不適問題,常見係以施行椎間融合術或在椎間植入人工椎間盤的方式來解決。
如本國公告第M414944、M411920、M409825、M400307及M384014號等脊椎植入物,即為取代人體原有之椎間盤,植入相鄰兩脊椎(或腰椎)之間,以提供支撐與緩衝功用。然因不同人其相鄰椎體間之間距不一,而上述等植入物因皆為固定單一尺寸結構,以致在植入手術適用上會有太緊或太鬆現象發生,亦即,會產生過之或不及問題,是以上述創作仍不臻完善。
另者,如美國公告US5554191號”為一種骨融合椎籠植入物,由圖面得知,主要係在一支撐本體3具有表面設有鋸齒面312、313的兩支撐部31、32,該兩支撐部31、32於開放端分設有形成V狀開口342的斜向
導面310、320,一螺栓36穿設支撐本體3,一端凸出兩支撐部31、32開放端結合一迫張件37,使該迫張件37常態下抵止於兩斜向導面310、320,俾植入兩椎體間該螺栓36被旋動工具4旋動時,可撐張兩支撐部31、32形成適當角度外擴支撐兩脊椎間之效。
惟,此結構雖可藉由具角度擴張結構來解決每一個人兩椎體間不同間距尺寸問題,然此結構在植入手術操作上,必須先將兩支撐部31、32撐張先咬合兩椎體固定後,再繼續旋動撐張動作,然因該兩支撐部31、32皆為單片結構,造成植入手術操作上必須施以較大施力,才得以將兩支先撐部31、32先撐開咬合兩椎體,亦即操作施力上較費力,造成植入手術過程中容易因施力過大易滑動無法準確對位風險,必須再作重新調整,增加手術困難度,延緩手術時間及患者負擔等問題,而可待加以改善。
緣此,如何解決上述問題即為本發明所欲解決之首要課題,因此本案發明人乃經過不斷的苦思與試作後,終於獲致肯定成效,而首創出本發明。
本發明之主要目的,係在提供一種具階段撐張之椎體固定件創新結構,俾有效分散植入手術施力,及適合多數人椎體高調整功能,以達更省力、精準不滑脫快速植入功效者。
本發明之次一要目的,係在提供一種有效縮短植入手術時間之階段式撐張之椎體固定件。
為達到上述之目的,本發明階段式撐張之椎體固定件,係包含一本體及一穿入本體而末端設有迫推塊的螺桿,該本體相對迫推塊一側
設有兩個或兩個以上的活動支撐單元,各活動支撐單元以剖溝剖設有可受迫推塊位移而階段性撐張兩個或兩個以上的活動支撐面,俾使各活動支撐單元可形成適當角度外張支撐相鄰兩椎體定位者。
所述各活動支撐面以間隔適當距離分別設有可導引迫推塊的引導面,並以其中一對向各引導面先抵靠迫推塊內側面。
承上述各引導面為傾斜階梯狀。
承上述各引導面為斜面狀。
承上述各引導面為曲面狀。
承上述,該本體為一上、下外表面設有咬合鋸齒面的扁狀矩形體,該本體由前端往後開設一橫向槽,使相對橫向槽的上、下壁形成兩活動支撐單元,各活動支撐單元至少以一剖溝剖設至少一活動支撐面,及一框架狀活動支撐面,該活動支撐面一端連結框架狀活動支撐面內框緣,該橫向槽開口端形成一寬槽部,供迫推塊橫向穿置寬槽部與螺桿末端螺合,各活動支撐面內表面分別設有引導面連接橫向槽與寬槽部,各引導面係間隔適當距離,並以最內層各活動支撐面對向引導面抵止該迫推塊內側面,使旋動螺桿帶動迫推塊向內移位時,該迫推塊可由各導引面分段導引而階段式撐張各活動支撐面,而使上、下活動支撐單元形成適當角度外張支撐相鄰兩椎體定位者。

的活動支撐單元,各活動支撐單元至少以一剖溝剖設至少一的活動支撐面,及一框架狀活動支撐面,該本體中央設有軸孔,該軸孔前端開口形成寬槽部,供球狀迫推塊容置與螺桿末端螺合,各活動支撐面內表面分別設有引導面連接軸孔與寬槽部,各引導面係間隔適當距離,而以最外層活動支撐面之各引導面設於最前端靠抵迫推塊內側面,使旋動螺桿帶動迫推塊向內移位時,該迫推塊可由各導引面分段導引而以階段式撐張各活動支撐面,俾使徑向分佈各活動支撐單元形成適當角度外張支撐相鄰兩椎體定位者。
承上述,該本體為一扁狀矩形體,上、下外表面設有咬合鋸齒面,在側壁中段貫設一外寬內窄雙階狀的中空橫向槽,該中空橫向槽前端形成一寬槽部,供該迫推塊橫向置設寬槽部與螺桿末端螺結,該本體相對橫向槽內階面上、下壁分別以剖溝剖設有兩活動支撐單元,及上、下間隔兩固定框架,該活動支撐面一端連結框架狀活動支撐面內框緣,而各活動支撐單元以相同形狀剖溝剖設兩個或兩個以上由外往內階段縮小的活動支撐面,各活動支撐面內表面分別設有引導面連接中空橫向槽與寬槽部,各引導面係間隔適當距離,並以最內層各活動支撐面之各引導面設於最前端靠抵迫推塊內側面,使旋動螺桿帶動迫推塊向內移位時,在上、下固定框架不動下,該迫推塊可由各活動支撐面之各導引面分段導引,以階段式撐張各活動支撐面,使設於上、下固定框架中間兩活動支撐單元形成適當角度外張支撐相鄰兩椎體定位者。

相較先前技術之實質效益,本發明確能藉由各活動支撐單元剖設區隔成複數內外排列的活動支撐面,俾安裝旋動螺桿使迫推塊位移時時,該迫推塊可先撐張其中一活動支撐面外擴,再依序連續撐張其他活動支撐面外擴成適當角度,如此即可達到分有效分散植入手術施力,及適合多數人椎體高調整功能,以達更省力、精準不滑脫快速植入功效者。

本發明階段式撐張之椎體固定件係以鈦、鉭、不锈鋼、陶瓷、氣化鋁、PE、PU、PEEK、SILICON材料製造,俾具有良好生物相容性及力學特性。
請參見圖1-6,係本發明階段式撐張之椎體固定件第一實施例結構,包含一本體10及一穿入本體10而末端結合有迫推塊21的螺桿20,其中,該本體10為一扁狀矩形體,圖中以腰椎植入物為例,當不依此為限,該本體10上、下外表面設有咬合鋸齒面11,該本體10由前端往後開設一橫向槽12,形成上、下間隔的兩活動支撐單元14,各活動支撐單元14至少以一剖溝13剖設至少一活動支撐面141,及一框架狀活動支撐面142,該剖溝13及該活動支撐面141,如圖中為U形,當不依此形狀為限,而該活動支撐
面141一端連結框架狀活動支撐面142內框緣,該橫向槽12開口端形成一寬槽部121,供迫推塊21橫向穿置寬槽部121與螺桿20末端螺結,請參見圖3,各活動支撐面141、142內表面分別設有引導面1411、1421連接橫向槽12與寬槽部121,各引導面1411、1421係間隔適當距離,如圖中為一斜面,當不依此為限,並以最內層各活動支撐面141之引導面1411抵止該迫推塊21內側面;另者,各框狀活動支撐面142於橫向槽12開口端對向凸設擋緣1422,俾藉兩擋緣1422擋止該迫推塊21外側面以防止該迫推塊21向外脫落;另者,該橫向槽12內側端連通一縱向孔15,俾各活動支撐面141、142安裝撐張操作上更具省力:此外,該活動支撐面141中間鏤設有供手術中去除的自體骨贅或人造骨組織填入的中孔1412。
本發明安裝實施時,請參見圖4為例,本發明迫推塊21常態下係抵止最內層活動支撐面141之各導引面1411,是以,當本發明植入兩脊椎(未繪出)間,即可由旋動工具(未繪出)旋動螺栓20帶動迫推塊21向內位移推抵止最內層之各導引面141,而將各最內層之活動支撐面141先撐張形成適當角度撐張後,請參見圖5,當迫推塊21被旋動向內位移直至碰到最外層各活動支撐面142之各導引面1421時,請參見圖6,該各活動支撐面141、142即同步被迫推塊21頂推向外撐張,俾達各活動支撐面141、142可由內往外階段式被迫張至完全撐張狀態;亦即,本發明能藉由迫推塊21先迫張最內層各活動支撐面141撐張咬合相鄰兩椎體(未繪出)固定,使持續被撐張到預設行程時,最外層各活動支撐面142才會同步被撐張,利用各支撐面具階段式撐張結構,俾利有效分散操作施力,提供植入手術初始操作以適當施力,即能藉最內層活動支撐面141先咬合兩椎體(未繪出)間撐張固定,有效
避免植入物滑脫重新調整不便,進而提供更省力、精準、快速植入手術操作之實用效益。
請參見圖7、8,本發明階段式撐張之椎體固定件第一實施例各活動支撐單元14也可以溝剖13剖設成由外往內階段縮小間隔分置的三個活動支撐面142、141、143結構,以達具三階段分散施力效能目的;而各三個活動支撐面142、141、143開口端各導引面1421、1411、1431可為斜面狀導引面;或請參見圖9,各導引面1421a、1411a、1431a可為傾斜階梯狀導引面;或請參見圖10,各導引面1421b、1411b、1431b可為曲面狀導引面。

綜上各實施例所述,本發明確實可藉由各活動支撐面形成內外排列階段式分開結構設計,俾安裝旋動螺桿使迫推塊位移時,該迫推塊可先撐張其中一活動支撐面外擴,再依序連續撐張其他活動支撐面外擴成適當角度度,如此即可達到有效分散植入手術施力,及適合多數人椎體高調整功能,以達更省力、精準不滑脫快速植入功效者。

- 一種階段式撐張之椎體固定件,係包含一本體及一穿入本體而末端設有迫推塊的螺桿,該本體相對迫推塊一側設有兩個或兩個以上的活動支撐單元,各活動支撐單元以剖溝剖設可受迫推塊位移階段性撐張兩個或兩個以上呈內、外層間隔排列的活動支撐面,各活動支撐面上以間隔適當距離分別設有可導引迫推塊的引導面,俾將各活動支撐單元階段性撐張形成適當角度外張支撐相鄰兩椎體安裝定位者。
- 如申請專利範圍第1項所述之階段式撐張之椎體固定件,其中,該各引導面為傾斜階梯狀。
- 如申請專利範圍第2項所述之階段式撐張之椎體固定件,其中,該引導面為斜面狀。

- 如申請專利範圍第1至4項中任一項所述之階段式撐張之椎體固定件,其中,該本體為一圓柱體或至少四邊的中空狀長柱體,在柱體一端表面徑向以剖溝剖設間隔設置至少兩個活動支撐單元,及一固定框架,各活動支撐單元一端連結固定框架內框緣,而各活動支撐面單元以相同剖溝剖設兩個或兩個以上由外往內階段縮小的活動支撐面,該本體中央設有軸孔,該軸孔前端開口形成寬槽部,供球狀迫推塊容置與螺桿末端螺結,各活動支撐面內表面分別設有引導面連接軸孔與寬槽部,各引導面係間隔適當距離,而以最外層各活動支撐面之引導面設於最前端靠抵迫推塊內側面,使旋動螺桿帶動迫推塊向內移位時,在固定框架不動下,該迫推塊可由各活動支撐面各導引面分段導引,而以階段式撐張各活動支撐面,俾使徑向分佈各活動支撐單元階段性撐張形成適當角度外張支撐相鄰兩椎體定位者。
