Description
The invention relates to medicine, namely to otorhinolaryngology.
[1 J · 5A known method of restoring hearing in patients with conductive hearing loss by shielding the window of the cochlea [1 J · 5
25-30%.However, the known method allows to restore the patient’s hearing by only 25-30%.
ЮThe aim of the invention is to increase the degree of restoration of hearing. YU
The circuit is achieved in that in the process of restoring hearing in patients with conductive hearing loss by shielding window cochlea soglas- 5 but the invention previously implanted around window vestibule element of soft magnetic material after implantation of the implant into the cavity of the middle ear administered hollow cone 20 elastin- a confuser with permanent magnets at its narrow end so that it is firmly fixed in the vestibule window.
The method is as follows.
After radical surgery on the middle ear with complete epidermization of the cavity under local or general anesthesia using a surgical microscope, the patient is lying on his back with his head turned towards the healthy ear under the scar-epidermal tissue of the vestibule, a horseshoe-shaped element made of magnetically soft material is implanted, placing it around the vestibule window . To do this, in the area of the anterior pole of the vestibule window, an incision of scar tissue is made, retreating 0.4 0.5 cm from the niche of the window, and this tissue is separated above and below the vestibule window. 10-12 days after the implant has been implanted, a confuser is inserted into the tympanic cavity, which is strictly fixed in the vestibule window by means of ponderomotive, magnetic forces. Moreover, the confuser is placed so that its narrow end with rigidly fixed permanent magnets is strictly located along the perimeter of the vestibule window, and the perimeter of the wide opening coincides with the perimeter of the external auditory meatus. This allows ’to reliably shield the cochlear window with the walls of the confuser and to focus the sound pressure mainly in the area of the vestibule window.
Example!. Patient E., 46 years old, was admitted to the hospital with complaints of a decrease in hearing in both ears. From the anamnesis it was found that ears hurt since childhood. In 1974, a radical operation was performed on the right ear. On examination, it was found that the cavity after surgery on the right 15 uke is completely epidermized. When screening the window of a snail with a cotton-oil swab, the effect is negligible.
An element was made of soft magnetic material, 20 confuser was installed. Hearing before surgery PPad SOPspay, after surgery, the perception of spoken language (PP) from a distance of 6 m
П р и м е р 2 . PRI me R 2. Patient K., 28 years old, was admitted to the hospital with complaints of hearing loss in both ears. Ear disease since childhood. In 1972, a radical operation was performed on the left ear. On examination, the postoperative cavity is completely epidermized. zo
An element was made from soft magnetic material in the area of the vestibule window.
·After 10 days, a confuser is installed. Thresholds for sound perception through air decreased from 45 dB to 15 dB at frequencies from 500 to 6000 Hz. ·
е. на 40-50%.The proposed method allows to improve hearing to a socially suitable level, the efficiency of hearing restoration is increased by 15-20% compared with the known method, and thresholds of perception through air are reduced to 20-15 dB, i.e. 40-50%.
