Description
(54). Method of substitution of bone defect in interversional region and thigh neck
1one
The invention relates to medicine, namely to orthopedics.
1.There is a known method for replacing a bone defect in the intertrochanteric region and the femoral neck by resection of the pathological focus, followed by replacing the defect with cortical grafts 1.
However, with a known replacement method, a proximal bone fragment may be displaced.
The aim of the invention is to prevent recurrence of the proximal bone fragment.
This goal is achieved by the method of replacing a bone defect in the intertrochanteric region and femoral neck by resection of the lesion followed by replacing the defect with cortical allograft is inserted intramedullary into the distal bone fragment graft, then the channel is drilled in the oblique direction of the graft, then a channel is drilled in the proximal fragment of the thigh, after which the formed
Способ осуществл ют следующим образом .The transplant canal and the proximal femur fragment are injected with a second graft. The method is carried out as follows.
5Production of resected affected bone
Then a lamellar cortical bone graft 10–12 cm long is inserted into the medullary canal of the distal fragment.
10 so that its widest part is located in the sagittal plane. Next, install bone fragments in such a way as to restore or maintain the correct anatomical relationship of the proximal end of the femur. A channel is drilled with a drill with a diameter of 8-10 mm in the free end of the graft and the proximal bone fragment. After x-ray inspection, the drill is removed and a second graft, 6-8 cm long and 8-10 mm thick, is inserted into the channel. The formed bone marrow firmly holds the fragments of the femur without additional external or internal fixation.
Трансплантаты , уложенные по нижневнутренней поверхности, не только восполн ют дефект, но как распорки между головкой и дистальным фрагментом бедренной кости еще в большей степени увеличивают прочность креплени .The residual bone defect is filled with thin cortical transplants, which perform it completely and even create some excess tissue. The grafts laid on the inferior surface not only compensate for the defect, but, like the struts between the head and the distal fragment of the femur, further increase the strength of attachment.
У 1 больного, где патологическим процессом были поражены зоны роста, развилась вальгусна деформаци , обратна по своим характеристикам дооперационной .The proposed method operated on 9 children with tumor and dysplastic processes in the neck and intertrochanteric region of the femur. 5 children had pronounced varus deformity of the proximal end of the thigh, and in 4 patients there was no deformity. In all children, during the operation, complete correction of the deformity and firm retention of the bone fragments was achieved, preserved in the immediate postoperative period, up to restructuring of the grafts and replacing the defect with one's own bone tissue. In 8 children, the further development of the femur proceeded normally, the deformity did not recur. In 1 patient, where growth areas were affected by the pathological process, valgus deformity developed, reversed in its preoperative characteristics.
The proposed method allows simultaneous correction of the deformity, normalizing the cervical-diaphyseal angle, firmly and stably fixing the bone fragments and keeping them in the correct position for a long time. The use of thin cortical grafts contributes to the accelerated process of their transformation and the restoration of the patient's own bone tissue, and also prevents the recurrence of the proximal bone fragment.
