(54) Stomach resection technique
This invention relates to medicine, in particular to gastric surgery. Closest to the present invention is a known method of gastrectomy, including mobilizing it along the greater and lesser curvatures, cutting off the duodenum, suturing its stump, excising the affected part of the stomach, forming the gastric tube from the greater curvature of the stomach and anastomosis it with the small intestine. The disadvantage of this method is the occurrence of post-resection syndrome due to displacement, pushing up to the gastric stump of the first loop of the jejunum, moving it upward to the gastric stump, causing bending and crushing of the anastomotic loop, even restructuring the anastomosis or eliminating arefluxes. Thus, the natural shape of the stomach in its stump, as well as the angle of the stomach and the pyloric kiel, are not restored. The purpose of the invention is the prevention of post-resection syndrome. The goal is achieved by the fact that in the proposed method of gastrectomy, including mobilizing it along the greater and lesser curvatures, cutting off the duodenum, suturing its stump, excising the affected part of the stomach, forming a gastric tube from the greater curvature of the stomach and anastomosis with the small intestine , the affected part of the stomach is excised at an angle, and the gastric tube is formed into a cylindrical shape with a length of 6–8 cm and a diameter equal to the diameter of the first loop of the scab, then the gastric tube is lowered through the hole in the root eyki mezhzholon and anastomose with the first loop of jejunum at a distance of 6-8 cm below duodenal angle. The method is carried out as follows. Under general anesthesia, the abdominal cavity is opened by a midline incision, a stomach is brought into the wound, which is mobilized along the greater curvature to the last gastric branch of the right gastroepiploic artery, and along the small curvature is skeletal up to the first transverse vascular branch going to the body of the stomach from the left gastric artery . Then mark the line of the angular intersection of the body of the stomach
