Abstract
Изобретение относитс к медицине, а именно к хирургии, и может быть использовано при лечении лимфостаза нижних конечностей . Цель - снижение частоты рецидивов лимфостаза путем повышени пропускной способности шунта. 1-й этап: провод т разрез по линии Кена ниже паховой складки на 2-3 см. Выдел ют артерию и вену clrcumflexa superficial. Рубцовые ткани иссекаютс . 2-й этап: срединна лапа- ротоми , тонка кишка выводитс в рану, s проход щем свете определ етс архитектоника сосудистых аркад, наличие лимфатических узлов и их взаимосв зь с кровеносными сосудами. Намечаетс один или два св занных между собой лимфатических узла, а также питающие сосуды с учетом наименьшей травматизации брыжейки и достаточного кровоснабжени кишки после удалени лимфатического узла с сосудами. Поэтапно прошиваютс и перев зываютс кровеносные сосуды вокруг узла, последними пересекают сосуды, питающие узел. Дефект в брыжейке ушиваетс кетгутом. 3-й этап: узел укладывают в рану паховой области и анастомозируют сосуды (артерию и вену) с сосудами, выделенными в паховой области. Афферентные лимфатические сосуды узла укладываютс дистально, эфферентные , начинающиес у сосудистой ножки узла, - проксимально. После пуска кровотока провер етс проходимость анастомозов, кровенаполнение лимфатического узла. Гемостаз . Швы на кожу. Способ позвол ет достичь более стойкой редукции отека. В последующем отек распространилс на всю конечность, также отех по вилс на правой голени. Последние 3 года присоединилось рецидивирующее рожистое воспаление до 2-3 рецидивов в год.Example. Клинический диагноз: первичный лимфатический отек обеих нижних конечнстгей, аплази лимфатических сосу- дов, осложненный рецидивирующим рожистым воспалением. Учитыва бесперспективность консервативной терапии больному предложена операци аутот- рансплантаци лимфатического узла брыжейки тонкой кишки, на которую больной дал согласие. Под эндотрахеальпым наркозом произведена операци аутотран- сплантаци лимфатического узла брыжейки тонкой кишки на левую нижнюю конечность с целью воссоздани дренажа лимфы. Ко- сым разрезом слева под паховой складкой обнажено устье большой подкожной вены с притоками. Выделен приток, идущий лате- рально вверх - vena curcumflexa ill spf,, р дом с ней выделена одноименна арте- ри . Срединна лапаротоми . Послойно вскрыта брюшна полость. При ревизии органов брюшной полости патологии не вы влено . Умеренное количество прозрачного выпота. В рану выведены петли тонкой киш- ки. В проход щем свете, учитыва архитектонику сосудистых аркад, намечены два св занных между собой лимфатических узла , определены питающие сосуды. Поэтапно прошиты и перев заны кровеносные сосуды вокруг лимфатического узла, питающие сосуды вз ты на микроклипсы и отсечены . Лимфатический узел перенесен в рану паховой области. Тщательный гемостаз брыжейки. Брюшна полость послойно ушита наглухо. Под операционным микроскопом (увеличении 16-25 - кратно) лимфатический узел уложен таким образом, чтобы эфферентные лимфатические сосуды располагались дкстально, а эфферентные располагались в проксимальном направлении . Кровеносные сосуды лимфатического узла анастомозированы с a.et.v.clrcumflexa ill spf, проленом 10/00 конец в конец. Пуск кровотока. Венозный отток из узла по вилс сразу же. Проверка проходимости анастомозов . Рана послойно ушита наглухо. Эластичный бинт. Послеоперационный период протекал без осложнений.
