Description
ЈJ
The invention relates to medicine and can be used in tomographic studies and radioneurosurgery of intracranial formations.
Известны устройства, служащие дл фиксировани головы пациента и адаптировани ее относительно диагностических и терапевтических установок, состо щие или из стереотаксической рамы с жесткой фиксацией за кости черепа, например аппарат Leksel, в котором голова пациента крепитс к жесткой металлической стойке, наход щейс за теменем пациента, с помощью рамы , к которой, в свою очередь, фиксируетс голова пациента за ушные отверсти и переносицу ,Devices are known that serve to fix the patient's head and adapt it to diagnostic and therapeutic installations, consisting of either a stereotactic frame with a rigid fixation of the skull bones, such as a Leksel apparatus, in which the patient's head is attached to a rigid metal stand located behind the subject's head, with the help of a frame, to which, in turn, the patient’s head is fixed by the ear holes and nose bridge,
The closest to the present invention is a device designed to fasten the patient's head and adapt it to the proton beam during proton therapy. Device
Устройство обеспечивает крепление головы пациента на установке дл протонной терапии и адаптирование ее относительно пучка протонов путем перемещени по трем координатам.made in the form of an individual mask made of a radiolucent thermoplastic, with the help of which the patient's head is pressed against the headrest located on the L-shaped deck of the laying table. The device secures the patient's head on a device for proton therapy and adapts it relative to the proton beam by moving in three coordinates.
However, the use of the known device is limited because it is structurally associated with the deck of the laying table, which is part of the proton therapy unit. This leads to the fact that the patient in the same device can not be examined at diagnostic installations (X-ray machines or various tomographs), which dramatically reduces its functionality.

хранением высокой надежности креплени в нем головы пациента.storing high reliability of fastening the patient's head in it.
The goal is achieved by the fact that a stereoadapter containing a face mask made of rigid material additionally contains a hard X-ray transparent helmet with a triangle applied on it with a contrast agent with means for attaching it to the support and an elastic container for filling it with air. the inner surface of the helmet, and the mask is attached to the helmet and has a tray for dental impression. The drawing shows a stereo adapter, a general view.
3.The stereo adapter contains a radiolucent helmet 1, a face mask 2 and an elastic container 3.
The use of a hard helmet with an elastic container inside it allows, when the patient fills it with air, to press the patient's head to the mask that is attached to the helmet by four protrusions. In this case, the patient's head is securely fixed in the mask-helmet design. Performing a mask and a helmet of rigid material ensures high reliability of fixation. Such a fixation of the patient's head allows it to be examined at any facility for diagnostic imaging (x-ray, NMR, CT or PET scanners), and then, without fixing, treatment at any device for remote neurosurgery — a linear accelerator electron, proton accelerator or multisource cobalt installation. This greatly expands the functionality of the proposed device.
The presence of a means for attaching the device to the support allows the head to move in space, and the triangle contrasting with the diagnostic settings allows the coordinates of the intracranial formations to be determined. These features of the proposed device allow adapting the patient's head with respect to diagnostic and radiosurgical facilities, which also expands its functional capabilities.
The presence in the mask of a tray with a dental impression, which fixes directly to the bone tissue, makes it possible to combine the patient’s head, helmet and mask into a single rigidly connected design, which eliminates the movement of the mask on the soft tissues of the patient’s head relative to the helmet and turn extends the functionality of the device.
7.The stereo adapter, besides the helmet 1 with an open face, an individual thermoplastic mask 2 and an air tank 3, includes a take-up strap 4, protrusions 5 for fastening the mask, means 6 for fastening the adapter to the table, and a contrasting triangle 7.
The stereo adapter is used as follows.
A helmet 1 is put on the patient's head, the dimensions of which are chosen so that three helmets of different sizes cover all possible sizes of patient heads. Air tank 3 is partially filled with air. The patient is placed on the table of the diagnostic unit, to which the helmet is fastened with two screws. In this position, an individual thermoplastic mask 2 is made, which is put on holes on four protrusions 5 from the sides of the helmet. A portion of the plastic plastic is bent down into the oral cavity of the patient and the bite is made with an impression of the teeth of the upper and lower jaws. In this position, the mask is allowed to harden. After the mask hardens, an additional air pressure is applied to the tank until the head is firmly fixed to the helmet. In the process of conducting tomographic diagnostic studies of the patient, the diagnostician receives a picture of a tomographic slice of his skull and helmet with cross-section points of a contrast triangle. From the distance between these points, the spatial position of the obtained tomographic slice relative to the patient's head is determined.
The proposed device allows to securely fasten the patient's head, adapting it with respect to diagnostic and radiosurgical equipment. The design of the adapter allows it to be used for diagnosis and radioneurosurgery in different clinics, while the patient can be transported with a fixed mask or removed and then put on in place.
These properties of the device provide its versatility. Possibility of use on any diagnostic and therapeutic installations.
