Abstract
Изобретение относитс к медицине, а именно к нейрохирургии. Цель изобретени - снижение послеоперационных осложнений и травматичности. Способ осуществл ют смещением кнутри височной мышцы, фиксацией ее устройством, перепиливани- ем костного лоскута у основани с последующим отведением м гких тканей на ножке. Устройство содержит элементы 2, 3, выполненные в виде изогнутых пластин с пазами 4, 5 с установленными в них роликами 8,9с насечками 10, и направл ющие 6, 7 с дополнительными планками 11.12 с крючками 13. 14. При помощи крючков 13, 14 и элементов 2, 3 устройство фиксируетс в отверстии. Через пазы 4, 5 и ролики 8, 9 провод т проволочную пилу и осуществл ют распил кости . 2 ил. Отслоение надкостницы в обе стороны от разреза, трепаном наложено 5 фрезевых отверстий по линии разреза надкостницы. Кость между ними пропилена проволочной пилой после проведени под кость между соседними отверсти- ми проводника. После отслойки надкостницы в основании костного лоскута между мышцей и костью введено устройство и закреплено на кра х основани лоскута . При помощи проводника проведена пила Джигли и лоскут в основании перепилен полностью. Устройство удалено. Костный лоскут на сосудистой кожке откинут. Обнаружены в эпидуральном пространстве сгустки объемом около 60,0 мл. После удалени эпидуральной гематомы по вилась отчетлива пульсаци мозга, мозг несколько расправилс . Тверда мозгова оболочка обычной окраски, умеренно напр жена. Последн вскрыта линейным разрезом на участке в 2 см в проекции височной доли. При ревизии субдурального пространства сгустков крови не обнаружено, мозг обычного цвета, слегка гиперемирован, извилины обычные. Швы на оболочку. Гемостаз по ходу операции. Костный лоскут уложен на место, швы послойно. В подкожно-апонев- ротическое пространство установлены два резиновых выпускника. Йод, спирт, асептическа пов зка.
