Description
The invention relates to medicine, namely to vascular surgery, and can be used in the treatment of stenosis of a blood vessel.
Цель изобретени состоит в снижении послеоперационных осложнений путем исключени интраоперационных кровотечений .The purpose of the invention is to reduce postoperative complications by eliminating intraoperative bleeding.
The method is carried out as follows.
It produces an outcrop of a superficial vessel, for example, the hips. arteriotomy of this vessel, followed by dilation of the affected segment of the prosthetic vessel with a balloon catheter. The latter is made in the form of a flexible tube and an expanding balloon at its distal end.
The tube is connected to a syringe filled with saline.
The dilatation of the affected vessel segment is carried out in two stages. The first stage is produced to the calculated diameter

где Дп - диаметр дилатации сосуда на первом этапе;where Dp is the diameter of the dilatation of the vessel in the first stage;
Дн - диаметр сосуда вне сужени ;The bottom is the diameter of the vessel outside the constriction;
Ds is the diameter of the vessel at the place of constriction.
Dilatation of the affected vessel segment, produced under such conditions, does not cause its rupture. After the first dilatation stage, the catheter is removed and a tubular guide is inserted through the arteriotomy into the affected segment of the vessel. It is used to transport the prosthesis with a fixing element. The latter is made in the form of a zigzag shape of a spring rolled into a cylinder and connected to the frame of the prosthesis. The prosthesis is delivered to the affected vessel segment so that the fixing element of the prosthesis is located above the stenosis of the prosthetic vessel. After the tubular guide is removed, the denture self-locks and is placed in the stenosis area.
After placing the prosthesis in the vessel, the second stage of dilation is performed. For this, a balloon catheter is inserted into the lumen of the prosthesis, and from the lumen of the prosthesis, through the wall of the prosthesis, the dilatation of the affected vessel segment to the bottom is performed. In this case, the framework of the prosthesis is tightly connected to the walls of the vessel and turns off this segment of the vessel from the bloodstream. In case of rupture of the wall of the prosthetic part of the vessel when its strength exceeds its strength, this does not lead to the appearance of bleeding. After removing the catheter from the prosthesis, the latter is anastomized with the prosthetic portion of the vessel.
Example. Patient S., suffering from atherosclerotic stenosis of the right iliac artery, underwent an angiographic study, which showed that there was a long stenosis of 22.0 mm in length and a lumen diameter in this area of 3.5 mm with a healthy vessel lumen diameter of 9.0 mm .
According to the angiographic data, the patient is shown to replace the affected segment of the iliac artery with a synthetic prosthesis with a diameter of 9.0 mm. Dilatation of this segment to a diameter of 9.0 mm is associated with the threat of vessel rupture and bleeding. Proceeding from this, the dilatation was carried out in two stages: the first of them - to a diameter of 6.0 mm, which does not exceed the tensile strength of this section of the vessel.
Bleeding was not observed at this site. After insertion of the prosthesis with a fixing element from the inside of the prosthesis through the wall of the prosthesis, the final dilatation was made up to a diameter of 9.0 mm. Bleeding from the prosthetic site of the vessel was not observed. Postoperative angiography showed good placement and function of the prosthesis.
Thus, carrying out such a method of endoprosthesis of blood vessels with dilatation of the affected vessel segment, carried out in two stages: preliminary, carried out under the condition not exceeding the tensile strength of the vessel, and the final, after installing the prosthesis through its lumen, allows to avoid intraoperative complications caused by rupture of the vessel.
