Abstract
Изобретение относитс к медицине, а именно к отоларингологии. Целью изобретени вл етс формирование разделительного механизма гортани при удалении вестибул рного отдела гортани в блоке с подъ зычной костью. Цель реализуетс тем, что надгортанник формируют из слизисто-мышечного лоскута из корн зыка, укрепленного слизисто-хр щевым лоскутом из носовой перегородки. Преимущество способа состоит в том, что улучшаетс разделительна функци при глотании после обширной резекции гортани, способ позвол ет осуществл ть раннее кормление через рот и снизить частоту послеоперационных осложнений.The invention relates to medicine, namely to otolaryngology. The goal is realized by the fact that the epiglottis is formed from a mucous-muscular flap from the root of the tongue, reinforced by a mucous-cartilage flap from the nasal septum. The advantage of the method is that the separation function is improved when swallowing after extensive laryngeal resection, the method allows early oral feeding and reduces the incidence of postoperative complications.

Способ осуществл ют следующим образом .The method is carried out as follows.
Под местной инфильтрационной анестезией 0,5% -ным раствором новокаина накладывают нижнюю трахеостому, больного интубируют и операцию заканчивают под наркозом с искусственной вентил цией легких. Производ т П-образный -разрез кожи. Подъ зычную кость скелетируют и удал ют. Вскрывают глотку сбоку на уровне проекции большого рога подъ зычной кости на непораженной стороне. Расшир ют фа- рингостому, в рану вывод т надгортанник, который берут на шелковые держалки. Визуально определ ют локализацию и распространение опухоли, которую удал ют в пределах здоровых тканей. После этого из носовой перегородки выкраивают слизистохр щевой трансплантат. Производ т типичный разрез слизистой оболочки и хр ща носовой перегородки. Распатором отслаивают слизистую и надхр щницу со стороны, противоположной разрезу, и выкраивают слизисто-хр щевой трансплантат, форма и размеры которого соответствуют надгортаннику . По периметру слизисто-хр щевого лоскута иссекают полоску хр ща шириной 2мм. В результате край слизистой оболочки трансплантата оказываетс свободным. Из ткани корн зыка соответственно форме и размерам слизисто-хр щевого трансплантата выкраивают слизисто-мышечный лоскут толщиной 5 мм. Последний отворачивают кверху и на его раневую поверхность укладывают слизисто-хр щевой трансплантат хр щевой поверхностью внутрь. Слизистые оболочки мышечного лоскута из корн зыка и трансплантата сшивают край в край, формируют надгортанник, который состоит из хр щевой основы, покрытой с обеих сторон слизистой оболочкой.
