Abstract
Изобретение относитс к медицине, а именно к кардиологии, и может быть использовано дл лечени сердечной недостаточности. Цель изобретени - сокращение сроков компенсации недостаточности кровообращени . Это позвол ет повысить эффективность лечени , снизить летальность больных с сердечной недостаточностью, особенно на фоне острого инфаркта миокарда. Сущность изобретени состоит в непрерывном длительном введении больным раствора нитроглицерина, причем концентрацию последнего увеличивают, а количество вводимой жидкости уменьшают до уровн , преп тствующего повышению объема циркулирующей крови. Клинические испытани показали высокую эффективность предложенного способа лечени по сравнению с традиционными.The invention relates to medicine, namely cardiology, and can be used to treat heart failure. Пример 1. Больна 3., 72 лет поступила в блок интенсивной терапии с жалобами на интенсивные раздирающие боли за грудиной , удущье. При поступлении состо ние т желое. Кожные покровы влажные, цианоз губ. Одышка в покое до 30 в 1 мин. В легких жесткое дыхание, среднепузырчатые хрипы с обеих сторон. Сердце перкуторно расширено влево на 2 см. Аускультативно: 1 тон на верхущке ослаблен, акцент II тона по левому краю грудины. ЧСС - 100 в 1 мин, АД 150/90 мм рт. ст., ЦВД 22 см водн. ст. Печень не увеличена. Отеков нет. На ЭКГ остра стади трансмурального переднего распространенного инфаркта миокарда. Рентгенографи грудной клетки - отек легких .Example 1. Начата инфузи 0,1%-ного раствора нитроглицерина с помощью аппарата ДЛВ-1 со скорости 1 мл/ч, через 10 мин достигнута оптимальна скорость введени 3 мл/ч (50 мкг/мин), при этом АД снизилось до Ь25/80 мм рт. ст., ЦВД снизилось до 15 см водн. ст., ЧСС составл ло 92 в 1 мин, ОЦК оставалс на стабильном уровне. Через 1 ч после начала введени показатели составл ли 120/80, 14, 86 соответственно , число дыханий уменьшилось до 20 в 1 мин, резко уменьшилс цианоз в количество влажных хрипов в легких, которые полностью исчезли через 2 ч, диурез 500 мл.
