Abstract
Изобретение относитс к рентгенодиагностике. Цель изобретени - снижение осложнений. В трахеостомическое отверстие ввод т интубационную трубку с раздувной манжетой на дистальном конце и катетером, проведенным через отверсти , выполненные в стенке интубационной трубки. Устанавливают интубационную трубку в трахее таким образом, чтобы конец катетера находилс в трахее, а проксимальный конец, через который ввод т контрастное вещество, - наружной поверхности. Перед введением контрастного вещества резиновой манжетой перекрывают нижние дыхательные пути, контрастное вещество в трахею ввод т независимо от искусственной вентил ции пациента. Способ позвол ет избежать такого осложнени , как гипокси , и обеспечивает возможность длительного контрастировани верхнего отдела трахеи на уровне поражени .This invention relates to X-ray diagnostics. Перед проведением трахеографии пациенту удал ют из трахеи трахеостомичес- кую канюлю и вставл ют в нее интубационную трубку, подобранную индивидуально по размеру отверсти -стомы, чтобы стома плотно облегала трубку. Интубадионна трубка должна быть с резиновой манжетой , или на любую м гкую трубку заранее одевают резиновую манжету. Затем приступают к самой трахеографии через катетер , конец которого устанавливают под контролем рентгена на нужном рассто нии, ввод т любое контрастное вещество. За про- движением контраста след т под рентгенологическим контролем при различных поворотах головы, щей, туловища пациента наблюдают за заполнением контрастом верхнего отдела трахеи и гортани. При этом скоростью продвижени контраста управл ют как скоростью надавливани на порщень шприца, так и придава положение телу то вниз головным концом, то вверх. В процессе заполнени контрастом или при полном тугом заполнении отрезка дыхательных путей контрастом производ т рентгенограммы в любых заданных проекци х . Пациент при этом спокойно дыщит или ему вентилируют легкие аппаратом искусственной вентил ции легких. Общее состо ние ребенка средне-т желое, не очень активен, в лый, в физическом развитии отстает от сверстников. В контакт вступает хорошо. Кожные покровы и слизистые оболочки бледные, органы дыхани : кашель влажный, небольшое количество сзи- листой мокроты. Грудна клетка обычной формы, обе ее половины одинаково участвуют в дыхании. Носовое дыхание затруднено , дышит через стому,. голосовое дыхание одинакого проводитс с обеих сторон. Границы легких в пределах нормы. Аускуль- тативно - дыхание жесткое, крупнопузырчатые влажные хрипы справа. Сердце без особенностей. Предварительной диагноз: рубцовый стеноз трахеи. Осуществлена пр ма поднаркозна л рингоскопи . Гортань слизиста , геперемирована, голосовые св зки сближены, утолщены, просвета между ними нет. При пальпации жестким отсосом за голосовыми св зками - плотна ткань. Заключение: Стеноз гортани III степени. Прот женность его на трахею необходимо вы вить трахеографией. Произведена ретроградна трахеографи 60%-ным раствором триом- браста с крахмалом. На трахеограмме в верхней трети трахеи определ етс конусовидное сужение, заканчивающеес слепо у гортани. Контрастное вещество не проникло за раздуваемую манжету и в бронхиальном дереве не определ лось. Несмотр на тугое заполнение оно не прощло и через голосовую щель. Рентгенологическое заключение: Стеноз посв зочного пространства трахеи III степени. Произведена трахеотоми , рассечение рубцов трахеи и гортани, протезирование Т-образной трубкой.The general condition of the child is average, not very active, but in physical development it is lagging behind its peers in physical development. A retrograde tracheography was made with a 60% solution of triom-brast with starch. On the tracheogram, in the upper third of the trachea a cone-shaped constriction is determined, ending blindly in the larynx. Contrast material did not penetrate beyond the inflated cuff and was not detected in the bronchial tree. Despite the tight filling, it did not forgive even through the glottis. Radiological conclusion: Stenosis of the trachea of the III degree trachea. Tracheotomy, dissection of trachea and larynx scars, T-tube prosthetics.
На данном примере показано, что применение предлагаемого способа диагностики позволило до операции получить полное и точное представление о характере патологического процесса, а именно о локализации, прот женности и степени стеноза трахеи, а так же избежать осложнений, св занных с использованием рентгеноконтраст- ного вещества, которое при трахеографии другими способами, как правило, попадает в бронхиальное дерево. Вентил ци во врем исследовани не прекращаетс This example shows that the application of the proposed diagnostic method allowed us to obtain a complete and accurate picture of the nature of the pathological process, namely, the location, extent and degree of stenosis of the trachea, as well as avoid complications associated with the use of a radiopaque substance, which in tracheography in other ways, as a rule, enters the bronchial tree.
