Перед операцией провод т, клинико-лабораторное обследование. В случае отмены стероцдной терапии за 2-5 мес до хирургического лечени гидрокортизон ввод т один раз по 1мг/кг до операции. При по влении признаков острой адренокортикальной недостаточности зтот препарат ввод т (Л и во врем операции. В случае отмены стероидной терапии менее чем за 2мес до операции гидрокортизон вво.д т до, во врем и после операции. а При проведении стероидной терапии гидрокортизон ввод т один раз до операции по 1 мг/кг. Пример 1. Больна С-ва М.В. 13 лет. истори болезни № 750523, диагноз: ревматический артрит, суставно-висцеральна форма. Больной проведена артопластика правого коленного сустава. Продолжительность операции 2 ч 5 мин. До операции в течение 5-6 лет регул рно полегчала курсы стероидной терапии большими дозами глюкокортикоидов. За 3 мес до операции глюкокортикоиды были отменены без повышени активности процесса и ухудшени самочувстви больной, Клйнико-лабораторное обследование перед операцией на вы вило признаков недостаточности функции коры надпочечников. За 1-3 дн до операции глюкокортикоиды не назначали . За 2 ч до операции внутримышечно введен гидрокортизон в дозе 1 мг/кг массы тела. В течение операции и первых 18-20 ч осуществл ли выжидательную тактику. В течение всего указанного периода гемодинамические показатели у больной стабильно удерживались на уровне 110120/70 мм рт.ст, частота сердечнык сокращений 90-108 уд./мин, что позволило не вводить дополнительно . глюкокортикоиды.У больной отсутствовала повьшенна кровоточивость тканей во врем операции, кровопотер составила 9% от расчетного объема циркулирующей крови (ОЦК), не отмечены и другие побочные влени или осложнени , св занные с введением глюкокортикоидов дл профилактики острой адренокортикальной недостаточности .Example 1. ПоследНИИ курс стероидной терапии-бьш закончен за 5 мес до опера1Д1И. Клйнико-лабораторное обследование больного перед операцией не вы вило признаков недостаточности функции коры надпочечников. За 1-2 дн до операции глюкокортикоиды не назначали. За 2 ч до операции внутримышечно введен гидрокортизон в дозе I мг/кг массы тела. В течение операции осуществл ли выжидательную тактику. Гемодинамические показатели перед введением в общую анестезию и в течение первой половины операции стабильно удерживались на нормальном уровне: артериальное давление в пределах I 10115/60 мм рт.ст,, частота сердечных сокращений 92-96 уд/мин. В середине операции после сн ти жгута артериальное давление снизилось до 90/40 мм рт.ст., увеличилась тахикарди . Введно внутривенно капельно 25 мг гидрокортизона (0,6 мг/кг массы тела), после чего артериальное давление и частота пульса нормализовались. Дальнейшее течение операции и анестезии, а также ближайшего послеоперационного периода гладкое. У больного отсутствовала повышенна кровоточивость тканей, кровопотер составила 21% от расчетного ОЦК, отсутствовали какие-либо осложнени , которые можно было бы св зать с введением глюкокортикоидов.Example 2.
