СПОСОБ ПРОФИЛАКТИКИ ПОСЛЕОПЕРАЦИОННОГО ТРОМБОЗА ГЛУБОКИХ ВЕН НИЖНИХ КОНЕЧНОСТЕЙ путем внутривенного введени гепарина за 15-20 мин до операции и одновременного воздействи на конечности давлени 40-50 мм рт. ст. с чередованием его и расслаблением конечностей, отличающийс тем, что, с целью уменьшени числа тро.мбозов , предварительно в крови больного оппредел ют антитромбин Ш и при его содержании ниже 80% ввод т внутривенно за 1 - 2 дн до операции, во врем операции, в первый, третий и п тый дни после операции донорскую плазму в количестве 150- 200 мл, гепарин ввод т в течение всей операции , на 5-6 сут после операции в дозе 9-11 Ед/мин, а также в последующие 14- 20 сут подкожно, а воздействие давлени на конечности осуществл ют во врем всей операции и в последующие 5-6 суток еже€ дневно в продолжение 9-10 ч. Изобретение относитс к медицине, а именно к хирургии. Известен способ профилактики послеоперационного тромбоза глубоких вен нижних конечностей у больных путем введени гепарина 1. Однако при применении данного способа возникает необходимость частого подкожного введени гепарина, который в местах инъекций вызывает образование кровоподтеков и нередко болезненных инфильтратов. Кроме того, при подкожном введении гепарин неравномерно всасываетс из депо, в крови больного не удаетс создать и поддерживать посто нно безопасную его концентрацию. Известен также способ профилактики послеоперационного тромбоза глубоких вен нижних конечностей, при котором гепарин ввод т за 15-20 мин до операции, во врем операции и в течение не менее 5-6 сут после операции в дозе 13-15 единиц и одновременно на конечности воздействуют давлением 40-50 мм ртутного столба в течение 45-55 с, череду его с расслаблением конечностей в течение 60 с 2. Однако указанный способ не эффективен у больных, у которых снижено количество антитромбина Ш. Цель изобретени - уменьщение числа тромбозов. Эта цель достигаетс тем, что согласно способу профилактики послеоперационного тромбоза глубоких вен нижних конечноетей путем внутривенного введени гепарина за 15-20 мин до операции и одновременного воздействи на конечности давлени 40-50 мм рт. ст. с чередованием его и расслаблен11ем конечностей, предварительно в крови больного определ ют антитромбин 111 и при его содержании ниже 800/0 ввод т внутривенно за 1-2 дн операции, во врем операции, в первый, третий и п тый дни после операции донорскую плазму в количестве 150-200 мл гепарин ввод т в течение всей операции, на 5-6 сут после операции в дозе 9-11 Ед/ мин, а также в последующие 14-20 сут подкожно, а воздействие давлени на конечности осуществл ют во врем всей операции и в последующие 5-6 сут ежедневно в продолжении 9-10 ч. Способ осуществл ют следующим обрааом . п До операции определ ют содержание антитромбина III в крови больного и при содержании антитромбина ниже 80% за 1-2 дн до операции переливают 150- 200 мл донорской плазмы внутривенно 1-2 раза. Перед операцией внутривенно ввод т катетер, соединенный со стерильным реf Vу зервуаром, содержащим раствор гепарина. На нижние конечности больного накладывают манжеты, которые соединены шлангом с устройством, подающим перемежающеес давление. Во врем операции больному переливают 150-200 мл донорской плазмы, производ т перемежающуюс компрессию нижних конечностей продолжительностью 15-20 с давлением мм рт. ст. В следующий за циклом сжати конечностей период продолжительностью 60 с давление в манжетах отсутствует. Гепарин ввод т внутривенно за 15-20 мин до операции , в течение всего времени операции и 5-6 сут послеоперационного периода. В последующие 14-20 сут гепарин ввод т подкожно по 5000 Ед через 8-Г-12 ч. Механическую перемежающуюс компрессию нижних конечностей в указанном режиме продолжают после операции по 9-10 ч ежесуточно , включа день операции, на прот жении 5-6 сут. На 1, 3 и 5 сут послеоперационного периода переливают по 150 200 мл донорской плазмы внутривенно, Пример. Больна Б., 76 лет. диагноз рд тела матки. Содержание антитромбина Ш в плазме крови 70%. За I сут до операции перелито внутривенно 200 мл донорской разморожен ной плазмы. За 15 мин до начала наркоза катетер, вставленный в вену, начали а ельное вливание 440 мл 5%-ного раст р .юкозы с 2 мл раствора гепарина JQQQQ д g соедини устройство (тройник) перелито jgo л размороженной донорской плазмы, „ конечности больной наклады ют манжеты, в которые подаетс перемежающеес давление. Во врем цикла сдав .„„ конечностей давление в манжетах повысилось до 42 мм рт. ст. и стабильно удерживалось на этом уровне в течение 20 с. После окончани периода сжати конеч„ стей давление в манжетах снижаетс до О, прот жении 60 с давление на конечности не подаетс . Затем цикл работы устройства повтор етс . Внутривенное вливание гепарина происходило во врем операции , в течение 5 сут после нее, а затем в течение 18 сут гепарин вводили подкожно по 5000 ЕД 3 раза в сут (через 8 ч).
