Abstract

Surgical instrument which can be connected by adapter 1 to stereotactic instrumentation for implantation/ transplantation of biological material, tissue, cells, pharmacological and radioactive substances or the like in the brain. The instrument consists of an outer cannula 2 through which an inner cannula 4 is introduced to the target site in the brain. Since the inner cannula 4 contains a plunger 9 which can be locked in various positions via piston 10 and sleeve 12, a quantity of implant of defined length and volume can be suctioned up into the inner cannula 4. The plunger 9, the piston 10 and the sleeve 12 can be locked, after introduction, in the longitudinal direction by means of a securing shoulder 14 on the adapter 1, but can be released in relation to the inner cannula 4 by means of the securing pin 11 being turned from the locked position in one of the transverse grooves 8 of the longitudinal groove 7. By now simultaneously withdrawing the inner cannula 4 and outer cannula 2 over the plunger 9, the material suctioned into the inner cannula (implant/transplant material) can be deposited in the tissue. The scope of the invention lies in medical, surgical, clinical and scientific activities. <IMAGE>

Description

468 270 transplant dopamine-producing brain cells to central parts of the brain of patients suffering from Parkinson's disease. The construction is also intended for use in other transplants or implants of tissue materials, pharmacological substances and radioactive material in the treatment of disease, for example in others degenerative central nervous disorders and tumor diseases. New with the design is that the implant is passive (without positive overpressure) very precisely deposited on a stretch with the shape of a cylinder whose length can be adapted to the current anatomy. The material to be implanted is first sucked up into the lower part of a piston-equipped cannula (hereinafter referred to as inner cannula) to the desired length (and volume). In this mode the piston is fixed while the inner cannula is inserted to the target point via a previously inserted cannula (hereinafter referred to as outer cannula). By the position of the piston in height is maintained while the needles are extended over the piston until the lower surface of the piston corresponds in height to the lower surface of the inner cannula opening, the implant material will be automatically deposited at a specific place in the tissue.

Kolven (9) inuti innerkanylen (4) fixerad i bottenläge genom att dess piston (10) med hylsa (12) fixerats med fixationstappen (11) invriden i nedre tvärgående skàran på innerkanylens krage (5).Description of drawings Fig. 1 shows a section through the composite instrument, with outer cannula (2) inserted through the adapter straightener (1) and the inner cannula (4) inserted through the outer cannula. The piston (9) inside the inner cannula (4) fixed in bottom position by fixing its piston (10) with sleeve (12) with the fixing pin (11) turned in in the lower transverse notch on inner cannula collar (5).

Fig. 2 visar adaptor-riktare (1) med centrerad kanal (15),dels från sidan och dels uppifrån, med i höjdled ställbar fixationsklack (14) med làsanordning (16) samt ytterkanyl (2) med krage (6) och punktionsmandrin (3).Fig. 2 shows adapter rectifier (1) with centered channel (15), partly from side and partly from above, with height-adjustable fixing lug (14) with locking device (16) and outer cannula (2) with collar (6) and puncture mandrin (3).

An embodiment of the invention is shown schematically in the accompanying drawings (Figs. 1-3). Fig. 2 shows an upper and a lower directional conductor (1) for the needles, which also act as adapters to stereotactic instrument. The outer diameter of these adapters can adapted to holders of various stereotactic instruments. Through a channel (15) through these adapters-direction conductors a mandrin (3) provided outer cannula (2) further into the tissue so that its tip will end up in the predetermined target point in the brain. Dä the softly rounded tip of the mandrel (3), with a few mm, protrudes outside the transversely ground tip of the outer cannula (2) can puncture and insert the cannula through the brain tissue is made without any real or complete insignificant impact or damage to the tissue (this technique is earlier used in surgery). In the current construction, the inserted one is used the outer cannula (2) as a conductor for the insertion of another cannula whose construction is new and is shown in Fig. 3. This cannula, designated inner cannula (4) can be inserted into the brain via the outer cannula (2) after the mandrel (3) has been extracted. Because the inner cannula (4) is provided with a piston (9) which closes tightly against the walls of the inner cannula (4) can tissue materials, cell suspensions or other implants intended for implantation, sucked up into the inner cannula (4), to the desired length from the opening in the tip of the cannula, before inserting the cannula through the outer cannula (2). The distance of the inner cannula (4) which is thus filled can be adapted to its length (and volume) in that the piston (9) is fixedly connected via a piston (10) with a sleeve (12) provided with fixing pin (11), as through turning into one of the transverse notches (8) in the collar of the inner cannula (5) locks the piston (9) in a determined and desired position. After inner cannula (4) with piston (9), piston (10) and sleeve (12) containing the material to be deposited in the brain tissue is inserted, one is swung fixing lug (14, Figs. 1 and 2) with locking device (16) over the sleeve (12) to prevent the piston from dislocating. By now turning the sleeve (12), its fixation pin (11) will be released from the transverse one the cutter (8) in the collar (5) of the inner cannula and be located in the longitudinal one skàran (7). This means that both the inner cannula (4) and the outer cannula (2) can displaced together upwards in the longitudinal direction while the position of the piston (9) do not disturb until the lower surface of the piston (9) corresponds to the cannula opening. Through this, the implant will be deposited within desired area in the brain tissue. After fixing the heel (14) removed, the entire instrument can now be removed. The different parts of the instrument is detachable for cleaning and the instrument is manufactured in materials that make it sterilizable by autoclaving.
