Description
15 20 25 30 35 ON GJ CD ... x LN 2 The device according to the invention includes a three-way valve 10 of a conventional type. The valve 10 has a valve housing, an inlet 11 and a first and a second outlet 12 and 13, respectively. In the valve housing, a valve body, as indicated at 14, is tightly mounted and rotatable by means of finger grips 15. The valve body has the necessary channels for connecting the inlet to the two outlets and also the two outlets with each other. To the first outlet 12 a cannula 18 is detachably connectable and to the second outlet 13 a riser 16 is connected.
Detachable, tight connection of the cannula 18 to the outlet 12 of the valve member 10 can be performed in many different known ways, one of which is illustrated in the drawing.
The outlet 12 thus has a conical portion, which can be inserted into the corresponding opening in the end 19 of the cannula 18, sealing being effected by a sleeve 17 rotatably mounted on the outlet 12 with its internal thread engaging a flange 20 on the end of the cannula 18 to 17 rotation tension the conical portion of the outlet against the walls at the cannula end. The riser 16 is made of plastic material and is approximately 7-8 cm long and attached to the second outlet 13 in a suitable, conventional manner.
The cannula 18 has a longitudinal bore 25, which extends from the cannula end 19 and the receiving opening for the conical portion 26 arranged therein, respectively, on the outlet 12, and extends through the cannula to a place at some distance from the opposite, free end 23 of the cannula where it opens into a side opening 22. The bore 25 is delimited on the side facing the end 23 of a concave wall 24. The end 23 of the cannula is rounded and the opening delimiting the wall 24 is concave and inclined forwards / outwards, as is clear from Fig. 2. Both are important, as will be seen below. these features of the inlet 11 of the valve 10 are designed in a suitable, conventional manner for receiving the dispensing end of a liquid dispensing means, preferably a syringe.
The inlet 11 may thus have an internal thread or a tapered portion cooperating with the syringe tip or a 15 15 35 25 35 35: W L 'I C13 .-.' \ L 'I 3. The inlet can of course also have external means for engaging the liquid supply means, the main thing is that liquid can be supplied to the inlet 11 in a suitable manner for further transport to the riser 16 after the second outlet 13 is connected to the inlet 11 by adjusting the valve body 14 by means of finger grips 15.
The device also includes a catheter 27, preferably a so-called epidural catheter, of a commercially available type. This is made of plastic material and has an outer diameter of a few millimeters and a suitable rigidity. The catheter 27 is intended to be passed through the bore 25 of the cannula 18 and out through the outlet opening 22. For this purpose the valve body 14 has a straight through opening which rectilinearly connects the inlet 11 to the outlet 12, whereby the catheter 27 is movable in a substantially rectilinear path from the valve inlet 11 via the valve body 14, the valve outlet 12 and the cannula 18 all the way to the opening 22, where it is deflected laterally upon encountering the concave, inclined front wall 24, as illustrated in the drawing.
The device according to the invention is used in the following manner for performing intrapleural anesthesia. First, a very small incision is made in the skin between the fourth and eighth ribs. A suitable syringe, the outlet of which is connectable to the valve inlet 11, is filled with a saline solution or a local anesthetic. By means of the finger grips 15, the valve body is rotated so that the inlet 11 is connected to the second outlet 13 and the contents of the syringe are pushed up in the riser 16 to a predetermined level therein. The front end 23 of the cannula 18 and the side opening 22 are now inserted into the incision to a position under the skin, after which the valve body is rotated so that the first outlet 12 and thus the cannula 18 is connected to the second outlet 13 and the riser 16. The cannula 18 is slowly pushed through the the soft part of the chest wall and its rounded end 23 pushes away the lung a few millimeters .Iër 10 15 20 G \ OJ 4 without damaging it. When the side hole 22 of the cannula 18 enters the pleural space, the negative pressure prevailing therein causes the fluid level in the riser 16 to drop slightly, which indicates that the cannula is in the correct position. the valve body is now rotated so that both the inlet 11 and the two outlets 12, 13 are closed and the syringe connected to the inlet 11 is removed. Thereafter, the valve body 14 is rotated again by means of the finger grips 15 to a position in which the inlet 11 is connected to the outlet 12 via the straight through opening in the valve body 14. The catheter 27 is then placed in the inlet 11 with the tip next to the valve body 14. Then the front of the catheter 27 is inserted end of the pleural space via the opening in the valve body 14, the valve outlet 12 and the bore 25 of the cannula 18 under control by means of the concave wall 24. The catheter is thus inserted into the pleural space in a very simple, gentle manner with minimal risk of lung damage and minimal risk of air intake. in the alveoli. When the catheter end is in the desired location, the cannula is carefully pulled out while holding on to the catheter 27, so that it is not included. The cannula 18 with associated parts can now be removed and the catheter 27 fixed to the skin and connected to a supply of local anesthetic, which can then be administered continuously or intermittently.
