Abstract

The authors proposed a clamp for fixing the cysts of the hyoid salivary gland. This clamp has the form of two intersecting jaws, finger rings, a rack for fixing the jaws in a predetermined position and two working jaws, one of which is presented in the form of a circle, and the second has the shape of a ring with longitudinal milling notches. The concept of its use is that after opening the mucous membrane of the cyst, the proposed clamp is introduced into it. One of the proposed sponges serves as a screen, protecting the surrounding tissue from laser exposure. The second sponge in the form of a ring clamps the front and upper part of the cyst and mucous membrane. Then, with a laser scalpel along the inner edge of the ring, the mucous membrane and the cyst along the perimeter are excised. As a result of compression of the clamp and laser exposure, laser welding occurs.

The urgency of the problem lies in the fact that until this time during the surgical procedure, there was no special fixative on the sublingual salivary gland, which combined an instrument for adequate retention of the cyst and, at the same time, as an organ-preserving function.

The prototype of the alleged tool, according to the authors, is Kocher clamp 16.5 cm long, which consists of two working parts. The clamp has crossed jaws connected by a hinge with finger rings and a rack for fixing the jaws. On the other hand, the branches are made in the form of two flattened parallel-going rods 3.7 cm long, ending with working jaws. The width of the rod at the hinge is 1.5 mm, and near the working jaws is 0.4 mm. The rods have a flattening on the inside on which there are transverse milling notches with a depth of 0.1 mm. One of the working jaws is a hook located in the middle of the plate, bent at a right angle to it. The hook width is 2.5 mm and the thickness is 3 mm. The second working jaw consists of two hooks located at the edges of the plate and bent at right angles to it.

The width of the hooks is 2.5 mm and the thickness is 3 mm, with a gap between the hooks of 1 mm. The use of this clamp was reduced to the fact that after making access to the cyst, and opening the cavity of the cyst, the upper machine was fixed with this clamp. Then, the cyst was separated with a tuffer, followed by removal by excision of the sublingual salivary gland without using a laser. One of the working sponges serves as a screen, protecting the surrounding tissue from laser exposure, and the second in the form of a ring clamps the front and upper part of the cyst and mucous membrane. Then, with a laser scalpel along the inner edge of the ring, the mucous membrane and the cyst along the perimeter are excised. As a result of the compression of the clamp and the impact of the laser scalpel, laser welding occurs, which allows not to stitch.
